Discharge Plan
Medication
1. Encourage the patient to comply with the prescribed medication.
This prevents further development of the disease process and other
possible complication.
2. Discuss to the patient and to his watcher all the take home medications.
The exact dosage at the right time, do not exceed dose, and the action of
the drug.
This would enable them to know what are the drugs and its desired
dosage. The exact dosage and time are important to ensure the drugs
effectiveness.
3. Warn them about the side effects that may occur.
Side effects are expected aside from its therapeutic effects or action. It
is important to discuss to them the side effects to prevent confusion.
4. Instruct them that they should not give drugs, which are not prescribed by
the physician.
To avoid the ineffectiveness of the drug prescribed, and to ensure the
safety of the client.
Exercise
1. Daily ambulation for at least 30 minutes every 6:00-6:30 in the morning.

Treatment

1. Explain the purpose of each medication that is prescribed by the
physician.
To educate the patient of the therapeutic effect of the medicines for the
faster recovery of the client.
2. Encourage the patient to comply with the treatment regimen required.
To prevent further complications and discomfort brought about by the
disease process.
3. Inform the significant others that they should be involved in the treatment
of the client.
To determine the coping ability of the significant others with disease and
treatment of the client.
4. Instruct the patient to refer immediately any abnormalities.
It supports continuity of care and lessens critical conditions and early
treatment of interventions could be done.
HYGIENE
1. Encourage proper hygiene care.
Hygiene is important to prevent acquiring other disease or infections.



OUT PATIENT ORDERS
1. Continue prescribed medicines and its right dosage.
To attain the therapeutic effect of each medicines towards the client.
2. Follow up with appointments with physician.
To evaluate the progress of the treatment and condition.
3. Encourage the patient to have adequate rest and sleep periods.
These aid faster recovery from the illness and to have enough strength
in performing activities of daily living and range of motion exercises.
4. Encourage the patient to perform deep breathing and coughing exercises.
To promote lung expansion and prevent post operative pneumonia and
atelectasis.
DIET
1. Encourage patient to eat foods rich in protein, vitamin c and fiber.
To support immune system and to promote infection resistance
