Narration of Facts:
Investigation shown that at around 01:30 in the morning on December 13, 2010
vehicular accident was transpired along Congressional Road, Caloocan City. Mr. Mario
Gomez personally came to this office and requested to put on record the damage on his
cart.
Based on the statements of the investigation, that on said 01:30am,
Congressional Road Hi-way, North Caloocan City, December 13, 2010, while he was
walking along the road, the vehicle coming from the opposite side was accidentally
struck the wagon. Then, before he went in the office, he tried to talk to the driver but he
was asleep.
As a result thereof, the aforementioned vehicle incurred undetermined amount of
damage.

Narration of Facts:
Based on my personally investigation from the scene of the accident occurred on
or about 17:30H, vehicle with plate# ADB-267 going to Caloocan, accidentally hit the
vehicle ahead from the right side with plate# TVN-624. I was interrogate the driver
vehicle collide and he told the truth what happened? Due to his negligence and he
drunk that time
The said accident was committed damages to the both vehicle, and the driver
vehicle with plate# TVN-624 was committed less-serious injured.
As a result thereof, the aforementioned vehicle incurred undetermined amount of
damage.
