APPENDICES

The following are the requirements needed in order to register the partnership:
SEC REGISTRATION REQUIREMENTS
DOCUMENTARY REQUIREMENTS FOR REGISTRATION OF CORPORATIONS AND PARTNERSHIPS
as of June 30, 2013
For foreign incorporator, subscriber or director, indicate passport number for purposes of
incorporation and Tax Identification Number for amendment of the Articles of Incorporation.
REGISTRATION/RECORDING OF PARTNERS:
1. Name Verification Slip;
2. Articles of Partnership; and
3. Joint affidavit of two partners undertaking to change partnership name, provided in its
Articles of Partnership or as amended thereafter, immediately upon receipt of notice or
directive from the Securities and Exchange Commission that another corporation,
partnership, or person has acquired a prior right to the use of that name or that name
has been declared misleading, deceptive, confusingly, similar to a registered name, or
contrary to public morals, good customs, or public policy (not required if Articles of
Partnership has provision on this commitment).

BIR Form 1903 - Application for Registration for Corporations/ Partnerships (Taxable/Non-
Taxable)
Documentary Requirements
For Corporations/Partnerships
Copy of SEC Registration and Articles of Incorporation/Articles of Partnerships, as the case
may be;
Copy of Mayors Business Permit or duly received Application for Mayors Business Permit, if
the former is still in process with the LGU
Other documents for submission only if applicable:
a) Contract of Lease;
b) Certificate of Authority if Barangay Micro Business Enterprises (BMBE) registered entity;
c) Franchise Agreement;
d) License to Do Business in the Philippines, in case of resident foreign corporation;
e) Proof of Registration/Permit to Operate with BOI, SBMA, BCDA, PEZA.

SSS REGISTRATION REQUIREMENTS
Any of the partners of a partnership firm should accomplish SSS Forms R-1 (Employer
Registration) and (R-1A (Employment Report) and submit these forms together with a
photocopy of the Articles of Partnership. The original copy of the Articles of partnership must
be presented to the SSS for Authentication purposes.

PHILHEALTH REGISTRATION REQUIREMENTS AND PROCEDURES
1. Submit the following at any PhilHealth Office:
Employer Data Record or ER1 Form (in duplicate)
Business permit/license to operate and/or any of the following:
Nature of Entity Additional Documents Required
Single proprietorships Department of Trade and Industry
(DTI) Registration
Partnerships and corporations Securities and Exchange
Commission (SEC) Registration
Foundations and non-profit
organizations
Securities and Exchange
Commission (SEC) Registration
Cooperatives Cooperative Development
Authority (CDA) Registration
Backyard industries/ventures and micro-
business enterprises
Barangay Certification and/or
Mayor's Permit


2. After processing, the employer will be issued the following:
PhilHealth Employer Number (PEN) and the Certificate of Registration
PhilHealth Identification Number (PIN) and Member Data Record (MDR) of registered
employees
3. Employers are required to display the Certificate of Registration in conspicuous area of their
offices.

PAG-IBIG REGISTRATION REQUIREMENTS

1. Employers Data Form
2. Specimen Signature Form
3. Valid ID
4. SSS Certification (if already registered with SSS)
5. Certified True Copy of Proof of Business Existence

For Partnership/Corporation/Foreign-Owned Corporation
- Securities and Exchange Commission (SEC)
Certificate of Partnership/Incorporation
- Approved Articles of Partnership/Incorporation and By-Laws
- Business Permit/Mayors Permit

Steps for business (employer) registration with the Pag-IBIG:

Step 1

Go to the nearest Pag-IBIG NCR/Regional Branch or any extension/service
office/field service center.
For NCR/Regional Branch, proceed to the marketing and Enforcement Division
servicing counter. For extension/service office/field service center, proceed to
the information counter.


Note: If verified as registered and membership record is updated, end of transaction. If not,
proceed to step 2.

Step 2


Secure Employers Data Form (EDF, 2 copies).
Accomplish and submit duly accomplished EDF together with required
supporting documents.
The Pag-IBIG Employer ID Number shall be issued as soon as available.


Employer's Online Registration System:

Pag-IBIG has also an employer's online registration system. So if you want to register online
please visit Pag-IBIG Fund's online services page at https://www.pagibigfundservices.com/

When using the online registration system, the following are the required information that
must be provided:

A. Business Name (Per DTI/SEC Registration)
B. Branch Name (if applicable)
C. Business Address
D. Business E-mail Address
E. Employer Classification
F. Start Date of Business Operation
G. Nature of Business
H. At least one (1) Contact Information
I. At least one (1) Authorized Signatory with designation
