SUMMARY:- Technician struck by a projected channel over EOTC platform .
DESCRIPTION:- On 5 Aug 2014, One of the technician of Electrical Dept was climbing monkey ladder to reach Reagent area EOT crane platform. While he entered the platform his left leg was hit by the Stopper. He was about to fell but sub sequentially he balanced himself.
INVESTIGATION FINDINGS:- 1) EOT Crane Stopper is obstructing entry to the platform. 2) Railing at the same spot is not proper. 3) Gap available over EOT crane platform. 4) Cable tray laid for lighting cables obstruction the walkway. 5) Railing height for crane where Electrical Panel is kept is low. 6) Iron rods and scrap metal found in crane platform area. 7) Stand for DSL maintenance is not available.
KEY FACTORS Conclusions of the investigation team are, 1. PHYSICAL FACTORS:
a) Stopper obstructing entry to the platform. b) Railing at the same spot is not providing proper safety.
