Incident Title: RBO side glass broken






SUMMARY:- RBO side glass broken.

DESCRIPTION:- On 05.07.14 at around 08:00 PM, Atlas RBO while going in 290 mRL south drive, at
the same time scissor lift was coming in the opposite direction, due to glaring caused by the head
light of scissor lift, RBO operator wasnt able to see the projected rod and got hit with a rod in front of
P-8 Stope.
INVESTIGATION FINDINGS:-
1) RBO wiper was not working properly.
2) There was no water in the washer tank of wiper.
3) To open the water valve which is at a certain height, rod was being used.
4) A rod was projecting 1.5m from the main drive.
5) Humidity was more hence there was an obscure condition.
6) Dipper light was not used by the scissor lift operator.

b) Rod was left in a projected condition after completion of work by someone.
