List of Available Medications
Inform all areas including ER, Medical Ward I & II, OB-Surgical Ward that the updated list of available medications at the pharmacy is already posted in their respective areas. Nurses must review the updated list.

Defective Supplies
There will be guidelines or protocols in the returning of defective supplies. A form will be formulated and utilized to monitor incidence of defective supplies.
Pharmacists
There is already a form available to monitor complaints of items/supplies /medications. However, the said form is very broad and thus it will be modified and utilized once approved.

There will be strict implementation of hospital policies.
Nurses must secure the doctors signature in the prescription. Forgery of doctors signature is strictly prohibited.

It is recommended to clarify medication prescriptions during MANCOM meeting.
Nurses will be reminded of the need to separate drugs/medications and medical supplies/equipment.
Nurses will be reminded of the need to accomplish all of the patients information in the prescription including the case number and category (4PS, POC, NHTS, w/ PhilHealth, w/o PhilHealth, etc.)
Patient for Discharge
Home Medications
Nurses will be reminded that they should inform the Pharmacy Department promptly for MGH patients and at the same time inform the patient & SO to return ALL unused medicines to deduct from hospital bills prior accomplishment of hospital clearance.
Patient or SO must be instructed to get their home medications ASAP, as soon as patient is tagged MGH.
Instruct patients the status / availability of home medications at the Pharmacy.
Nurses are advised to prescribe separate prescriptions with at least 2-3 days medication from the prescriptions for full regimen. Proper instruction / clarification that NOT ALL home medications are dispensed by the pharmacy.
Effectiveness of Drugs / Medications
There will be a proposed meeting with the pharmacy department and the therapeutic committee regarding drugs/medications in terms of brands and its effectiveness. The meeting will also include clarification of certain medications
