Safety
The control and elimination of recognized hazards
to attain an acceptable level of risk. An individuals
perception of that hazards and associated risk.
Safety = Hazard Elimination + Risk Management
Hazard: Any thing that will cause harm of any degree to
Man, Machine, Method & Money.
Risk: The chances, likeliness, odds of an event occurring.
DEFINITIONS
ERROR Mistaken belief, fault, mistakes, something done a miss.
RISK - Possibility or likelihood of danger, injury, loss etc.,
ACCIDENT - An event that takes place without ones foresight or
expectation, esp. one of an afflictive or unfortunate character, a
casualty.
SAFETY - Condition of being safe, freedom from danger or hazard.

1. Job Safety Analysis(JSA)
JSA is a procedure used to review job methods and uncover hazards
that
1. might have been overlooked
in the layout of the facility or building
in the design of the machinery
equipment
tools
workstations
processes
activities
2. might have developed after the production started.
3. resulted from change in work procedure or personnel.

2. PPE A Second Line of Defense
PPE is designed to protect the worker from health and
safety hazards that cannot be removed or engineered
out of the work environment.
PPE is used as a last resort
The use of PPE signifies that the hazard could not be
controlled by other methods
3. EDUCATION

Action taken to eliminate a detected non conformity
Corrective Action (CA)
Action taken to eliminate the causes of an existing non conformity or
other undesirable situation in order to prevent recurrence

To develop effective CAPA plans, root cause analysis of problem
is required. Developing an effective CAPA Plan is half the battle
won. Next half is implementing them, which at times is not given
due discipline and rigor as required so often.
