1. SCOPE
The Sper-53 form, herein included and consisting of 3 sheets (A, B, C), provides a means of
organically reporting, in a simple and concise way, information concerning well test
operations carried out during drilling, completion, workover, and production activities.
It is a standard form, whose aim is giving information feedback to the departments involved.
2. INSTRUCTIONS
The form is supplied by the Company in multiple copy carbon paper to allow the availability
of copies of the form when a Xerox is not installed on the site of operations.
However it is recommended the utilization of Xerox machine in order to have always good
quality copies of the original.
The form is filled out by the Company Representative, who makes sure that the data
included are correct.
The form organically reports the sufficient information to offer to the reader (who, in any case
will be an expert) a complete view of the operation.
Sheet A is for general data reporting and sheet C is for test string composition reporting.
These sheets will be filled out once for each well testing operation.
Sheet B is for test data reporting; it will be updated daily and transmitted daily by telefax as
attachment of daily operation report (SPER 41).
3. RESPONSIBILITY
The Company Representative will be responsible for correct filling out of the forms and
signing for approval.
The Company Representative will be responsible for the first stage of distribution.
The Drilling & Completion Manager ("Responsabile Area Pozzo di Distretto" for Agip
domestic activities) will be responsible for ensuring the forwarding of a copy to STAP
department at Agip Headquarters in San Donato Milanese.
4. DISTRIBUTION AND FILING
The Company Representative transmits the form via telefax to the Operating Base: in
particular he will send sheet B every morning updated as attachment of the daily operation
report (SPER 41).
He files a copy and sends the original copy to the Operating Base (or District).
The Superintendent in District (or Operating Base) files the original copy and forwards a copy
of the form to STAP.
STAP receive the copy, file it in SIDAS (electronic-optical archive) and place the paper copy
on file.
Fig.1 shows the distribution chart.
5. COMPILATION FREQUENCY
This form must be compiled on occurrence and forwarded at the end of well test operations.
