Personal History (Past or Previous Personal History)
A. Prenatal Data
Mr. Mark James P. Jimenez was born and grew up at Mandaluyong City. His mother became
pregnant to him when she was 23 years old. He was the eldest among the 4 siblings where there are two
brothers and one sister in their family. During the mothers conception there were no non-infectious
and infectious diseases occurred. She also has an O+ blood type and there were no observed allergic
condition. His mother was very healthy during pregnancy since adequate balance diet as well as
frequent checkup was observed. There were no indication of usage of drugs, alcohol, opium and etc.
There were no exposure to any forms of radiations, jarring, electric shock and traumas happened to the
mother. Finally, there were no emotional tension occurred since both parents are happy since he was
their first baby.
B. First Year of Life and Infant Feeding Problems
The subject was delivered normally at Mandaluyong Hospital with the care of Dr. Camarines.
The first sucking process by the subject was satisfactory and he was given immediate breast-feed by his
mother. He was under the supervision of his physician. The subjects mother occupation was public
teacher, she was then under leave due to her condition but shes the one who took care most of the
time of her son. There were no periods of refusal in breast-feeding. He was also taken care of his
grandmother since after 2 months his mother needs to go to work. His guardian was caring and a great
provider for his basic needs. There was no vomiting occurred in the subject. The subject also slept after
feeding by her mother all the time. He also started to weaned and bottle fed at the age of 1 yr. old since
he liked the bottled-fed milk compared to breast milk. There were no sucking of artificial nipple
happened. There were no occurrence of diarrhea and constipation as well. He started thumb sucking
and biting of nipples until the age of two and there were no occurrence of lip and nail biting at that time.
Regarding physiological development he was able to fix his eyes towards a person and look to a certain
object at the age of 4 months. At the age of 6 months he began to respond to sound, voice or people
approaching. He also began to grasp and play with objects with the age of 8 months provided with
supervision of mother. He started sucking thumb or fingers during 7 months as well as rolling at the
bed. He began sitting at 8 months. In addition, he was observed standing and walking slow without help
at 1 old. He began walking and running at the age of 2. He also began climbing and jumping at the age
of 3. During 8 months of the subject, he began his tooth eruption. At the age of 8 months he started
uttering simple words like ma-ma, pa-pa. He started speaking phrases and sentences at the age of 2. He
was not allowed to mix or to play with other children since his mother was over protective to him. At
this stage his language development was progressive since he started uttering few words during 8
months and upon 2 years of age he was able to speak phrases or sentences.

C. Two to Three Years of Life (Toilet Training)
During the second and third year of life, he wasnt rebelliously or intentionally bowel or urinate
everywhere since he was wearing diapers during that time. He also didnt find any pleasure in playing
with his feces or urine. His parents were not strict in terms of training him for proper hygiene in
defecating or urinating since his parents wants to explain things in calm manner. He wasnt stubborn or
resistive in complying with his training since he was given reward upon doing right thing. There were no
diarrhea and constipation occurred during that time. He also doesnt cry since he was taken care of by
his grandmother and father wherein he was closed to them. At this age, he was not destructive although
he was very playful and wants to lure outside of the house. Rewarding system was applied to him to
follow the rules of his parents during that time since his parents dont want to give corporal
punishments. He was able to speak phrases and sentences during this time but then there were some
difficulties in uttering words which are long or difficult to pronounce.
D. Middle Childhood (Age 3-11 years)
During middle childhood the subject was able to identify his gender being a male with the help of his
parents; they taught or help the subject to recognize things which a male or a female should use. So
with that the subject was able to used things appropriate for his gender. His parents most especially his
mother was very strict to him but she will not give any corporal punishments instead she will explain the
negative impact for the wrong doings done by the subject. During the first separation it goes out well
since the subject was amazed in his classroom as well as he gained friends. He gained many friends soon
as he grow up. But then he remained as a follower in his peers.

E. Educational history
The subject entered school when he was 3 years old. It was June 1997 when his mother sent him at holy
cross day care center for nursery. The subject was quite anxious at first since he wants to be with his
mother. He was very quiet at school but he received good marks since he was doing well academically.
He only go to school during weekdays and then will go home after during that year. There were no
truancy or escaping from school that time since the subject always follows his teacher. The subject was
ambitious for his education since he wanted to become a doctor someday so that he can help people
with diseases. His school was very near his house and there were no tuition fee since it was under a
government so his parents dont worry too much. The patient acquire desired amount of education
since when he turned 7 he transferred to a private school near their house and good remarks were
observed to the subject. The subject chose his career which is quite far from his ambition since there
was a pressure in his family and school as well since during high school most of his classmates took
accounting, so he chose that course as well even though he doesnt like it. But then he failed that course
causing him to transfer to another course. There were no quarrel with teacher and professors but there
were fight between his friends because of being lazy and want other to help him in his requirements at
school.
F. Occupational History
The subject wants to be a clinical psychologist or HR manager after graduating from school. He was
influenced by his own determination to become a doctor someday. His parents doesnt have strong
hand towards his choice of work since they want him to stand on his own since the subject was quite
indecisive at first and he always ask for other people opinion. The subject vocational aims when he had a
work someday are to become an effective and efficient professional and to enhance his capabilities
towards psychological testing. The subject was still studying now.
G. Home environment
The subject home life was happy since all of them are complete in their house except for his father. Hes
also happy since they have a stable financial capacity since his mother works so much His friends were
also allowed in his house since his mother was open for visitors since she knew that they will do school
requirements. When the subject was little he doesnt have any recreational activities and good time
with his friends since her mother was protecting him to get sick and bullied by people. But now he
enjoyed having a good time with his friends most of the time. The subject also given responsibility in
home which is simple chores but the he usually wants his siblings to do his work for him. His parents
were not that too strict for him but there were limitations at home. The subject was living with his
family at home up to present since he has a fear of being alone all the time. He experienced having
many friends outside. The subject got along well with his mother since all the time theyre with one
another. Usually they have a family problem relating to financial support of his father since his father
has a third party
H. Social Relationships
1. Attitude towards people
The subject has many friends since early childhood up to present. The subject likes tp go and
engage to social parties nowadays most especially when there are occasions such as birthdays or debut
of his friends at school. The subject was highly sociable towards other people since he wants to be
accompanied by those people most especially his friends. But the, there were times that the subject felt
that his family and friends doesnt like him in terms of being a party-goer type of person since other
people misjudged the subject personality as a bad-influence type of person. The subject doesnt
complaint to other people if they treat him unfairly since the subject understands the situation first if he
was the one whos committed the wrong deed. The subject doesnt complaint that there were some
enemies trying to trick him since he will know first the person if they are real or fake before engaging to
them.
2. Social Adjustments
During childhood the subject didnt obtained full happiness towards his relationship to his
playmates since his mother was very strict to him in terms of playing outside their house. Although
there were closed friends started during childhood it didnt last longer since there were some conflicts
happened and immediately the subject seeks another friend relationship towards his classmates at
school. His friends at school treated him very well since theyre very close to each other. But then the
subject took criticisms and kidding by his classmates and friends seriously since he was not unto those
things. The subject was quite self conscious in groups since he wants to be looked right unto his friends
eye since he has a fear of being alone or being thrown away by those people who were closed to him.
The subject played volleyball and other recreational activity not only to achieve popularity but also to
gain friends at school. The subject always felt happy at school when he was with his friends since he
wanted to work with them. The subject indulged in alcohol beverages but not into drugs and tobacco
during emotional distress but then in the present the subject drinks whenever there are occasions
happening. The subject was not a member of any social and professional group although he had a peer
group at school where he was satisfied and contented to them.
3. Finding Satisfactorily Outlets and Interest
The subject finds his interest towards playing sports specifically volleyball and engaging makeup
sessions. The subject like sport games such as volleyball and chess but then it only happened
occasionally since he was very busy all the time. The subject doesnt like things out of wood and metal
and in addition he doesnt collect things. The subject doesnt experience yet fishing and hunting but he
like to try it. Te subject indulged his recreational activities together with his friends at school.
4. Sexual Life
The subject doesnt have or experienced venereal diseases. His sexual power was normal since
there were instances of high desire erection and interest and there were times that he doesnt want. He
started is interest towards sex during puberty period. The subjects sexual development was normal
since he experience the desire and urge. The subject started to masturbate during 1

year high school
summer after being circumcised. The subject didnt ask his parents although he has been open minded
when his classmate at school shared about sexual information. The parents dont mind the subject
masturbation since they knew it was being done privately. Sexual information were given freely since
they had a sex education in class. During grade 5 there were some sexual information gathered by the
subject during that time. The subject doesnt felt any shyness during early social contact with opposite
sex. There were no sexual experiences which have produced the subject to have emotional distress.
There were no husband and wife role play experienced by the subject. The subject doesnt have a
relationship with opposite sex. Although, he had experienced sexual intercourse with same sex.
I. Religious Activities
T he subjects religion was Roman Catholic. The subject believed and worships his religion well
since he grew up in a catholic school. There was no pressure experienced by the subject towards his
religion. The subject was religious during elementary days since he studied in a catholic school every day
they must engage in religious activity. But then upon transferring to other school his routine change, he
doesnt go to church every Sunday and he doesnt pray every day. The subject was every since chose this
religion since his parents were both catholic so they introduced it to the subject as well. The subject was
tolerant of other religion as well since he was now studying to a university where people have different
religion so he must respect them as well even though they have differences from one another. Religious
beliefs create conflicts when both sexes have different religion and they have differences from one
another so this causes conflicts towards their relationships. The subject observes and practices his
religious work moderately since there were times he doesnt go to church every Sunday.
J. Behaviour History
The subject doesnt ever gotten into any serious jams with the law ever since because he was
afraid to be in jail. The subject doesnt experienced being arrested or summoned in the court as well.
The subject drink alcohol and it started when he was in 2

year high school where he was pressured by
his friends. The subject also gambles specifically playing cards and that was during 4
th
year high school
where he was caught by his teacher. There were no vices experienced by the subject except drinking
alcohol. There was no relation to gangster s or fraternity groups experienced by the subject. There were
no accidents happened outside the house but then inside the house he got slipped in the floor and
bump his head causing bleeding. The subject doesnt experienced running away from home.
K. Latency Period
The subject was observed of increasing in curiosity towards things most especially sexual activity
when he reaches puberty stage. The subject was observed of engaging activities mostly to girls but not
into boys. He usually plays what other kids play such as tumbang preso, piko, Chinese garter and etc. In
terms of studying, he wants to study together with his mother since he knew that his mother would
greatly help him towards his school works. The subject increased in temper when he was in elementary
days most probably ages 9-10 since there were times that he get a fight with other classmates. The
subject behaviour towards his relatives and parents during that time was obeying since he must follow
his parents and elders since they have a good family integration. The subject was frank, sometimes
aggressive, playful and friendly at school but when he is at house he was quiet since his mother might
get angry towards him. In terms of parent-child relationship during that time his mother was very strict
to him when it comes to disciplining him so it end up that sometimes he get mad towards his mother.
L. Puberty and Adolescence (12-21 years old)
There were preoccupations experienced by the subject most specially his diet and his looks.
There were also thoughts of the subject of being independent and freed from parental authority but
then he knew that he wouldnt survive being alone. The subject became more sociable both opposite
and same sex when he reached adolescent. The subject was also engaged in sexual activity when he was
19 years old. The subject enjoyed in group companionship or group activities at school most specially
the peer group in his college. There were no unusual skeptsm in religion, family relationship and school
discipline. The subjects ideal person was his mother since he considered her as a role model for him.
There were changes in behavior towards authorities and family. The subject became defensive
compared before being passive type of person. The subject was a leader during adolescent since he has
knowledge that will help his group. The subject was a mixer type of person where he enjoys accompany
with other people.
Family History
A. Father
The subjects father is still living. Currently 44 years old of age. The subjects father usual illness was high
blood and liver problem due to alcoholism. The subjects father occupation was a traffic enforcer and a
businessman as well. The subjects father was highly aggressive and serious all the time. Sometimes they
had a fight with the subjects matter and he treats his siblings unfairly since he favored in his 3
rd
son. The
subjects father was very alcoholic and gambler. He attends religious ceremony occasionally.
B. Mother
The subject mother was still living. Shes currently 45 years old of age. Her usual diseases were high
blood pressure, vertigo and hormonal imbalance. The subjects mother was a high school teacher and
businesswomen as well. The subject mother personality was short tempered, moody but very
responsible and hardworking type of person. The subjects mother doesnt have any vices. She always
visits the church every Sunday. The subject was very close to her since they always bond every week
together with the subjects siblings.
C. Immediate members of the family
The subject has 2 brothers and 1 sister. The subject was the eldest among the sibling so next to him was
17 years old and hes a male. He usually lay basketball and plays computer games. He had a short
temper, lazy behavior and very playful person. He is in college now taking up BS computer engineering.
The subject subjects brother was still single and Roman Catholic. There were no diseases suffered. Next
t him was also a male 14 years of age and currently 3
rd
year students who loves playing computer games
and collecting and taking care of pets hes single and roman catholic as well. The youngest was 12 years
old and shes a female. She usually lay guitar and introverted type of person. Shes single and Roman
Catholic as well.
D. Other Relatives
There were no relative similar character traits to the subject but there were history of high blood
pressure among other relatives. There were no relatives who are influential in the family.
E. Other Data
There was a particular trouble in the family affairs specifically financial problem and third party of the
subjects father. Usually the subjects family experienced financial problem. Aside from the subject as an
alcoholic drinker the next sibling drink alcohol as well but never tried smoking and taking drugs. There
was no member of the family committed suicide. There were no members of the family who become a
serial problem. There were no diseases in any immediate member of the family member.
F. Home and Environmental Conditions and Facilities
(a) HOME
The family own a house. The structure of the house was a residential type. The rooms were
small. There are 6 people who were living in the house. The patient was living with her parents. There
were no adopted children. The in-laws live in their own house and didnt have to live with the subject.
The subject wasnt sleeping with somebody. There was only 2 toilets available. It was tidy and it had a
small space. The kitchen and the living room are joined together with the dining area. There are no
sofas. You need to sit on the dining chairs or on the floor to watch television. There was no backyard.
The room is too small and crowded so there is no room to play around. There were no recreational
facilities. The family owned a television and a laptop. The place to study would be inside the room or in
the dining area. The room is small. The room had one bed and a cabinet at the side. The room had one
big window. The room is well ventilated.
(b) ENVIRONMENTAL
The neighborhood wasnt crowded.. It is an urban neighborhood. The families that lived there
was of middle class and high class. The kind of families that have enough income to support the electric,
water, monthly, bills of their unit. There were recreational facilities like playground, basketball court,
and a clubhouse for indoor sports like tennis and badminton. There are no social or religious
organizations in the community. The subject is not a leader or a member of any organization. The
subject is not a member of a vicious gang. There are no gambling places or immoral influences in the
community.
