1. Three (3) copies of 1x1 picture Not scanned, yellow-green background, white duty uniform, very
visible nurse's cap (ladies)
2. Three (3) copies of 2x2 picture Not scanned, yellow-green background, white duty uniform, very
visible nurse's cap (ladies)
3. Name, School & Year graduated at the back of the pictures
4. Application letter declaring intent to apply as a Nurse Trainee addressed to:

5. Resume with 2x2 picture (Kindly specify age, gender, religion, and civil status)
6. Birth Certificate
7. Baptismal Certificate
8. Transcript of Records
9. College Diploma
10. PRC License or PRC Claim Slip
11. PRC Board Rating
12. NBI Clearance (dated within 1 year of submission) or Police Clearance(dated within 6 months of
submission)
13 Marriage Certificate (if married)
14. Tax Identification Number (if previously employed)
15. BIR Form 2316 (if previously employed)
16. SSS Number
17. PAG-IBIG Number
18. PhilHealth Number
19. Community Tax Certificate/ Cedula
20. Recommendation from the Dean
21. Recommendation/ Certification from previous employer (if previously employed)
22. Character Certification from two other persons
23. Completely filled up Personnel Profile (will be provieded once completed requirements are checked
by NSO Personnel)

Produce one (1) clear copy of the above. Place all requirements in a long white
ordinary folder and fasten all documents securely at the top using an ordinary
fastener. Arrange all documents according to the above sequence. INCOMPLETE
REQUIREMENTS WILL NOT BE ACCEPTED . The applicant's name must be clearly
written at the side of the folder in capital letters using a black marker in the
manner shown at the right. (Last Name, First Name Middle Initial).
