The following message is a follow-up to [Reference: 141022-000401] [Incident: 14

1027-000117] (see original message below) for Toyota/Lexus USA and [Case: 226770
9] for Toyota/Lexus Canada:
This is a formal statement and claim for the defect of electrical wiring insulati
on in affected Toyota/Lexus vehicles that lead to electrical short circuit and t
he resulting malfunctioning airbag and seatbelt in these vehicles:
A safety-related defect complaint will be filed with NHTSA and Transport Canada
for investigation of the issues described below.
Sufficient evidence have been established to support the following claims:
Toyota Motor Sales USA Inc. and Toyota Canada Inc. knowingly acknowledge manufac
turing defects with the wiring harness that leads to unravelling of the wiring i
nsulation and exposure for electrical shortage in the electrical system of the a
ffect vehicle as evident in internal communications to its dealerships
Toyota Motor Sales USA Inc. and Toyota Canada Inc. failed to notify and address
vehicle owners the aforementioned cause of electrical shortage by faulty wiring
insulation, which results in malfunction of seat belt and supplemental restraint
systems (airbags)
Toyota Motor Sales USA Inc. and Toyota Canada Inc. require vehicle owners to pay
for the replacement of the defective parts even if vehicles was under warranty,
and ignored customer complaints that faulty wiring harness insulation is a dire
ct result of manufacturing defects
Toyota Motor Sales USA Inc. and Toyota Canada Inc. will be contacted by the resp
ective law firms representing the Class.

Hi, I am seeking reimbursement for the following repairs on my Lexus vehicle:
Vehicle year, make, model, VIN, mileage and date of purchase
2007 Lexus IS250 AWD, JTHCK262475009145, 56545 miles
A brief description of your complaint and the actions you have taken to resolve
it.
I am surprised there hasnt been a recall for this particular issue, as it pertain
s to the safety of the vehicle under NORMAL operations. Apparently this is a wel
l known manufacturing flaw on the wires to the driver s knee airbag squib. There
was an information bulletin issued in 2008 that clearly indicated poor wiring in
sulation that would come unraveled under the dash and the airbags and seat belt m
ay become inoperable in the event of a collision. The resolution offered by the
Lexus Richmond has been to replace the wiring harness for $3125 CAD. Lexus Richm
ond failed to acknowledge the defect even after speaking with the dealership man
ager. Contacted Lexus Canada corporate and redirected to Lexus USA for reimburse
ment. This is a critical SAFETY defect that Toyota/Lexus Canada and USA has fail
ed to address.

What action or remedy you believe would resolve your problem.
I am willing to accept reimbursement of the entire amount of $3125.25 CAD, which
covers replacement of the faulty wiring harness by Lexus Richmond.
