Utility Box Art Program Details
A Reflection of Your Community

Theme
Those interested in submitting artwork to be considered for the Utility Box Art Program are encouraged to use the Citys flower,
Geranium in the artwork. Your artwork may highlight your ideas about what represents your surroundings, the City of Monterey Park or
what you hope for the future of Monterey Park.
Eligibility
Entrants must be Monterey Park residents or a student attending Monterey Park schools.
All entries must be the original design and artwork of the entrant(s).
Each entry must be sent with a completed official entry form and release of liability to be considered.
Entries and all forms need to be submitted together as a single package.
Please do not permanently secure forms to artwork.
Entries can be dropped off at Monterey Park City Hall, 320 W. Newmark Ave., Monterey Park, CA 91754.
Medium & Specifications
Each side of the utility box design must be represented by one sheet of paper 8 x 11 or larger. Painting, colored pencils, pen and ink,
are all acceptable media.
Every submission should be paper clipped together.
Important: Artwork using a majority of dark colors cannot be considered due to the potential for overheating the utility boxes.
Entries that use a copyrighted or trademarked image will not be accepted.
Entries must not be plagiarized, stolen, or copied from any other source. Entries must not defame or invade the rights of any person,
living or dead.
Entries not adhering to these guidelines will not be considered.
Judging
A panel of judges selected by the Community Participation commission will evaluate entries based on interpretation, composition and
originality.
Awards
Artist recognition via your signature on the utility box (no larger than 3 x 5).
Detailed discussion of the artist(s) name, artwork title, and photo of the artwork may be featured in the City of Monterey Parks
Cascades Newspaper, cable channel and other promotional materials.

LIABILITY WAIVER
In consideration of being permitted to participate in UTILITY BOX ART PROGRAM-, and all activities
associated with such participation, I agree, on behalf of myself and my minor child (if any) to the following:
1.
I agree and represent that I have inspected, or immediately upon entering the public property will
inspect, the premises and facilities. My, and my childs, entry onto the public property for all purposes
associated with UTILITY BOX ART PROGRAM- (Program), including, without limitation, participation
or use of any facilities or equipment, constitutes an acknowledgment that such premises and all facilities
and equipment were inspected and that I find and accept them as being safe and reasonably suited for
the purposes of my participation in this Program.
2.
I release, waive, discharge, and covenant on behalf of myself, my minor child (if any), heirs,
executors, and administrators and for all of my family members, not to sue the City of Monterey Park, its
elected and appointed officials, agents, volunteers, and employees (Releasees) from all liability to me,
or my child, for any loss or damage, and any claim or demands on account of personal or property injury
or because of my, or my childs, death, whether caused by Releasees negligence or otherwise, while I,
and my child, participate in the Program.
3.
I agree to indemnify and hold the Releasees harmless from and against any and all claims arising
from my, or my childs, participation in the Program, whether caused by Releasees negligence or
otherwise. I will pay all costs incident to any claim, including, without limitation, attorneys fees.
4.
I expressly agree that the foregoing release, waiver, and indemnity agreement is intended to be
as broad and inclusive as is permitted by the law of the State of California and that if any portion is held
invalid, it is agreed that the balance shall, notwithstanding, continue in full legal force and effect.
I have read and voluntarily sign the release and waiver of liability and indemnity agreement, and further
agree that no oral representations, statements, or inducement apart from the foregoing written agreement
have been made.
