St., Brgy. Liboton, Naga City, having been duly sworn in accordance with law, hereby
depose and state:
1. That I have personally known the person of NILO RANDY B. IMPERIAL for
a long period of time;
2. That I know for a fact that the person by the name of "RANDY IMPERIAL"
appearing in some documents and records or specifically in his Certificate of
Registration No. 46543252 issued on July 11, 2000 and the name "NILO
RANDY BUENAVENTE IMPERIAL" as appearing in the record of Social
Security Systems (SSS) with SSS I.D. No. 33-2788872-4 and COMELEC Voter
I.D. No. 7603-0261A-I1672NBI10001-8, refer to one and the same person
and that his true and correct name is NILO RANDY BUENAVENTE
IMPERIAL;
3. As such, I execute this Affidavit to certify or attest to the truth of the
foregoing facts and for whatever legal purpose that this Affidavit may serve.
