The Science of Negative Pressure Wound Therapy


Intermittent Therapy Recommended for Best Results
A strong and growing number of studies have found that intermittent negative therapy increases
blood flow to the wound site more effectively than continuous negative pressure and that proliferation of new
granulation tissue is significantly higher (63.4%) over continuous negative pressure. One theory regarding
increased blood flow is that this may be due to the fact that during higher pressure settings there is increased
perfusion of blood flow to the wound with a hypoperfused halo at the wound edge.
As pressure levels decrease this halo decreases as blood flow disperses into the surrounding tissue
minimizing possible ischemic effects.

Low Levels of Continuous Pressure Recommended


Recent research indicates that lower pressure may be beneficial especially in soft tissue to minimize ischemia
in the hypoperfused zone surrounding the wound edge. Research done at the Lund University Medical Center
recommends levels of -80mmHg for soft tissue and slightly higher for muscle tissue.

Basic Mechanisms of Negative Pressure Wound Therapy


The responses of wounds to NPWT are the result of controllable and equally distributed negative pressures, not
from the particular dressing and drain combinations used to apply negative pressure therapy. Three
leading mechanisms of action are thought to contribute to the wound healing benefits of NPWT therapy.

Vacuum Assisted Drainage


The removal of excess exudate is the first mechanism. The fluid removed contains microorganisms,
debris, proteolytic enzymes, and other components which can prolong inflammation and edema, and
delay healing. Removal of this exudate results in antimicrobial control and maintenance of a moist
environment important for healing.

Reduction of Interstitial Fluid


Removal of interstitial fluid reduces edema, which decompresses the tissues. As a result, capillary blood
flow is increased, allowing for improved perfusion, angiogenesis, and increased supplies of oxygen,
nutrients and growth factors important for healing. Increased blood flow and perfusion also improves
access for infection-fighting cells.

Mechanical Effects on Blood Flow, Cell Growth and Cell Proliferation


NPWT produces mechanical stress and a pressure gradient across tissue that results in surges of blood
flow to the wound area. In addition, NPWT places physical strain on cells, stimulating them to interact
with soluble growth factors in the extracellular matrix. This effect is often referred to as cell stretch.
The stretched cells begin proliferating, dividing, and maturing, leading to the growth of new vessels,
collagen deposition, and granulation tissue formation.

VPT Variable Pressure Therapy


The literature has confirmed that intermittent negative pressure increases blood flow to
the wound site more effectively than continuous negative pressure and that proliferation
of new granulation tissue is significantly higher. But until now, pumps available on the
market were not able to provide practical, comfortable intermittent therapy. New Prospera

In VPT, two different negative pressures are used,


but negative pressure is always maintained, never
dropping to 0 mmHg. VPT offers the advantages of
intermittent NPWT, while avoiding potential for
displacement of the dressing and loss of an airtight
seal associated with pressure drops to 0 mmHg.

Variable Pressure Therapy (VPT)

Lower Pressure
Prospera recommends negative pressure levels in the range of -80 mmHg, lower than the commonly
recommended -125 mmHg. Lower pressures have been shown to reduce pain, while offering comparable effects on blood flow, wound contraction and volume reduction. Lower pressures also reduce the
risk of tissue ischemia at the wound edge.
Standard Black Foam, Thin Foam, & White Foam Dressings
DeRoyal offers all the necessary foam dressings needed for Negative Pressure Wound Therapy today.
Our standard Black Foam dressings are customizable to any wound size and wound type, while out
Thin Foam dressings are available for shallower wounds. Our White Foam dressings are also available

AMD Gauze Dressings
Antimicrobial gauze provides protection against a wide-range of bacteria, and does not require
changing if negative pressure is lost for more than 2 hours, as do some foam-based systems. Moist
AMD gauze under negative pressure will not adhere to the wound, is natural and biodegradable,
and will not allow in-growth into the dressing medium virtually eliminating pain during use and with
dressing changes. It is easy to apply and remove in one piece and the white, absorbent nature of
gauze makes it easy to monitor exudate at the wound site.

Indications for Use


The PRO Series Negative Pressure Wound Therapy
Systems are indicated for patients who would benefit
from a suction device, particularly as the device may
promote wound healing.

Contraindications
When used for wound healing, the PRO-II is
contraindicated in the presence of:




Necrotic tissue.
Unexplored or non-enteric fistulas.
Untreated osteomyelitis.
Wounds containing malignant tissue.
Exposed arteries, veins, or organs.

Precautions
Precautions should be taken in the presence of:
Anticoagulation or active bleeding.
Difficult wound hemostasis.
Close proximity of blood vessels, organs, muscle,
and fascia requiring adequate protection.
Irradiated vessels and tissue.

Therapy today. Our standard Black Foam
dressings are customizable to any wound size
and wound type, while out Thin Foam dressings are available for shallower wounds. Our
White Foam dressings are also available for
wounds with exposed bones, tendons, vessels, and tunneling/undermining. *For Silver
Dressings, DeRoyal offers our Dermanet Ag
Contact Layer, which provides protection from

AMD Gauze Dressings


Antimicrobial gauze provides protection
against a wide-range of bacteria, and does not
require changing if negative pressure is lost
for more than 2 hours, as do some foam-based
systems. Moist AMD gauze under negative
pressure will not adhere to the wound, is natural and biodegradable, and will not allow
in-growth into the dressing medium virtually
eliminating pain during use and with dressing changes. It is easy to apply and remove in
one piece and the white, absorbent nature of
gauze makes it easy to monitor exudate at the
wound site.

Dome Kit Application


Step-by-Step Process to Applying Dome Kits

Step 1
Gather required supplies.

Step 2
Measure the wound and cut the
foam to the size/shape of the
wound dimensions.
Do not cut the foam over the
wound, as small fragments may fall
into the wound bed.
Dressing size should allow for the
dressing to fill wound cavity without
overlapping on the intact skin.

Step 3
Gently place the foam dressing into
the wound cavity, covering the entire
wound bed.
Do not force or forcefully push in
the foam dressing. Potential damage
to the wound may occur.

Step 4
Size and trim the Transeal to cover
the foam dressing.
Proper Transeal application is the
key to less alarm issues and loss of
seal issues.

Step 5
Peel off the release liner.
Gently press down the exposed
adhesive to the skin.

Step 6
Gently press the top of the Transeal to
ensure adhesive contact to the skin.

Step 7
When a proper seal is secured,
completely remove carrier layer
of Transeal.

Step 8
Gently lift the Transeal with your
thumb and forefinger.
Cut out a 2 - 2 cm hole in center of
foam using either scissors or scalpel.
NOTE: Do not cut a slit in the
Transeal. When NPWT is applied
slit may collapse and block the
pressure.

Step 9
Remove release liner from the dome
connector by holding the blue tab
and pulling the paper release liner.

Step 10
Place the center of the pad directly
over the cut hole or custom hole of
the foam dressing.

Step 11
Once a proper tight seal has been
accomplished, peel off the clear
backing liner from the connector.

Step 12
Peel off the blue tabs.
Pat around the dome pad to ensure
complete adhesion.

Step 13
Connect the dome pad tubing
to the end of the connector
tube attached to the canister.
NOTE: Every dome connector kit will
have an adapter included.

Step 14
Turn on the NPWT system, make
sure that the pressure is set at proper
mmHg or per doctors order.

Step 15
Once the NPWT is turned on, the foam
dressing, dome pad, and Transeal will
start to compress.
A compressed look will mean that there
is a proper seal and that the negative
pressure is reaching the wound.
