Any work related to safety, health and environmental which has been carried out by any persons, or group of
persons, engaged by the employer in order to promote safety, health environmental in the place of work.
Company appoint new safety officer as OSHA 1994, FMA 1967 and Sime Darby requirements.
Regulate EHS meeting and toolbox talk once a month.
Fogging fortnightly stress at 'kongsi' and office area.
Had EHS Committee and Emergency Response Team at site. Will train the committee and team time to time.
Water bowser keep on running during hot and windy day. Worry of complaint from public and local authority.
Silt trap and wash through regularly maintained and reinstate
Safety promoting-now in progress and will raise up more signages at workplace area, site office, 'kongsi', store
and hazardous area.
Start conduct safety, health and environmental training based on Sime Darby requirements.
Took water sampling on monthly basis by our environmental consultant-Environmental Science Sdn Bhd.
Noise and air will be monitored every three month.
Workshop for lorry to do maintenance has been erected. They must change the black oil and repair at the
concrete base.
More signages have been raised at site office, 'kongsi' and working area.
Livestocks had been removed from site.
SIRIM audit had been conducted on 29-30th August 2012. This IMS audit conclude Safety & Health,
Environmental and also Quality.
DOE visit on 12th September 2012. DOE advise Condolink to make protection on slope and platform. No ncr
given.
Drowning hazard - signage has been putted at all pond area. No swimming and fishing allowed. Condolink and
Sime Darby do not take any responsibility if accident happen there.
Sime Darby EHS Department has been audit site on 4th December 2012. Observation on environmental has
been given.

10 Any outstanding matter arising from the previous report.

a. All company & subcontractor operators must have legal and valid driving license to operate the machine/vehicle.
DOSH & JPJ normally will stress on this issue.
b. Having problem with local residents who were stayed in site area. They must obey the regulation such as
open burning to prevent Condolink or Sime Darby getting summons from local authority.
c. Some of workers still did not have CIDB Green Card. Slowly to process CIDB Green card for them.
d. Lorry 'hantu'/un-licensed lorry is not allowed to operate in site. All vehicles used at construction site must be
roadworthy and registered with JPJ.
e. Silt trap must construct follow drawing by HSSI. However,the design not suit at the site. Contractor must come
out with new design and get approval from consultant.
f. Many workers has come in and we need more toilet at 'kongsi' area.
g. Sullage at bath area need to be trapped as well before entering temporary drainage.
h. Vehicle maintenance area was damaged and need to repair as soon as possible.
i. A few skid tank found at site must make proper bund to prevent spillage. Also need approval from BOMBA.

FUTURE PLANS
a. Try to achieve highest Safe Man Hours for this project. Suggest site management to make small occasion
or 'makan makan' if the team can work without accident until end of project.
b. HIRADC and EAIIA need to be revised.

12 Action to be taken on report

a. An employer or any person in charge of a place of work after receiving the report under regulation 19, shall
not later than two weeks after the receipt discuss the report with the safety and health officer.
b. An employer or any person in charge of a place of work shall countersign the report in order to confirm that
he has received it.
c. The report shall be kept in good condition at least for a period of ten years for the purpose of inspection
or investigation by the officer if necessary.
