Most common cause is if the secondary is not loaded or shorted.

There are two reasons that may cause CT failures in High voltage system.
1. Aging and eventual failure of high voltage insulation
2. Open circuiting of CT secondary during operation
That is why periodic condition monitoring of Switch gears to be done atleast once in two years. Various diagnostic
methods are available which will detect the healthiness of the equipments like CT, PT,Transformers etc

What type of CTs are you asking about? Bus bar mounted LV/MV CTs or large HV oil-filled CTs?

Open CT secondary while energized will do it. Have you had maintenance personnel inside the panel lifting wires?
Also, if this is not related to open CT secondary and is happening frequently, maybe there is some other root cause like
ferro-resonance during switching, depending on what your system configuration is.
One needs to look at all the data.

In my limited experience ,the problem has always been either age, degredation of insular value and lightning strikes.

An open secondary is probably your issue. However, it could be an open in the CT itself, before the secondary output
terminals.

Following-on to Neil Crandell's very valid points: is it possible that someone has removed or withdrawn a
instrumentation or protective relaying device without properly operating the CT secondary shorting switches or
inserting CT secondary circuit shorting screws in the terminal block?

Dear Jaydeep,
you need to be more specific while posting the queries! Can you pl share more details of the CTs? e.g. Voltage rating,
CT Ratio, metering or protection CT, VA Burden, indoor or outdoor type, mounting etc. All these factors will help to
predict the probable causes of failure. Also the failure details. if it is due to insulation failure or Ferro resonance, it
busts.
pl share more data.

When Primary bus is charged, then with CT in secondary circuit with terminals open will cause current i.e. electrons
charged particles will get buildup around the cross section of CT terminals will eventually have no space to stand and
will be converted into heat energy and will explode.
