Inspector Observations: Large scale chart BA 4037 which included 'Eastern Petroleum
Anchorage' where vessel was double-banked for supply of bunkers was not used, instead
vessel had used smaller chart BA 4041 for navigating.
Initial Operator Comments: Master was reminded to use the large scale chart at all times when
there is large scale chart
available for that area.
To avoid any recurrence, this item was included in the vessels passage plan checklist.

Additional comments
Vessel was registered with 'EW Liner' for automatic supply of new editions of charts and
publications. NTM available onboard was for week No. 24 of 2014.
Vessel is fitted with bow thruster with power output of 500 kW. In addition to controls in
wheel house, bow thruster and main engine controls are also provided on bridge wings.
Does the Operator provide adequate personal protection equipment, (PPE) appropriate to the
cargo(es) being carried; are instructions for its use provided, and is it being used correctly?
Inspector Observations: Personal protective equipment like rubber gloves, apron, eyewash
were found missing from paint store. Also, MSDS for paints was not available in Paint Store.
Initial Operator Comments: We have send the MSDS for the paint and are now available in the
paint store.
The rubber glove and eye wash is place in paint locker immediately after the inspection.
We have placed an order for the apron and will place on board once received.
Master was reminded to post up all relevant MSDS in the paint store.

Other Inspector Comments: Engine room and pumproom are protected with CO2
extinguishing system. Cargo area is provided with low expansion fixed foam extinguishing
system. Paint store is fitted with water sprinkler system. Emergency fire pump is located in
bow thruster compartment. Emergency fire pump was satisfactorily tested during this inspecti

Inspector Observations: Starboard lifeboat which was designated as Rescue Boat was not
appropriately marked externally or in anywhere in near vicinity. Also, Rescue boat recovery
procedure was not available in vicinity of starboard lifeboat.
Initial Operator Comments: The rescue boat marking was originally marked but, this marking
was painted over during
the recent routine maintenance job on the rescue boat.
The marking of rescue boat is now re-marked on the starboard lifeboat.
Instructed Master to place the recovery procedures near the rescue boat.
Are the cargo pumps fitted with emergency stops and are these tested regularly?

mooring area, in pumproom & CCR and were recorded as tested prior commencement of
operation.

Inspector Observations: As per Survey Status Report with print date of 2 June 2014,
memorandum was issued by class for inspection of Forepeak Tank during each Annual Survey.
Initial Operator Comments: We have carried out some coating repair job during vessels
scheduled
docking in November 2013.
Ballast tanks inspection is part of the vessels on going plan maintenance system.
Master is instructed to inspect the forepeak tank periodically as per
vessels plan maintenance system.
And carry out some coating repair job, if necessary.

Other Inspector Comments: As per Operator's policy, cargo tanks & ballast tanks were
required to be inspected annually. Cargo tanks were last inspected during dry-docking in
November 2013. Ballast tanks were last inspected in February 2014. Coating condition of
ballast tanks and cargo tanks was reported to be "Good".
Are safe and effectve procedures in place for the effective stripping (final draining) of tanks at
the end of cargo discharge?
Other Inspector Comments: One stripping pump is provided to assist in final discharge of
cargo.
