working in different locations within and outside your hiring company.
We have set your personal employment terms and conditions in Appendix A and the
general employment terms and conditions in Appendix B.
This letter agreement and its appendices (collectively, the Agreement) contain the
entire terms and conditions of your employment and supersede any prior verbal or
written agreement, understanding and representations. This Agreement may only
be modified in a document signed by the parties referring specifically to this
Agreement.
You are required to fully and faithfully abide by and comply with the Employees
Handbook, Code of Conduct, relevant staff dealing rules, other policies and
regulations currently and hereinafter implemented by the Company from time to
time, laws, government regulations, and all orders and instructions of the duly
designated managers and other authorized officers and agents of the Company
(collectively, the Rules and Regulations) and the Agreement. Carefully review these
documents which are available at the Human Resources Department and ensure
that these accurately reflect our agreed terms and conditions and you
understanding.
This Agreement should be treated with strictest confidence. Should you have
queries thereon, please contact your local Human Resources Representative. Please
indicate your acceptance by signing and returning this Agreement to the Human
Resources Department not later than [insert date]. You may retain the enclosed
duplicate for your files.

By signing this Agreement, I represent and affirm that I have fully read and
understood, and agree as a continuing condition of employment to fully comply
with, the Agreement and the Rules and Regulations. I also affirm that I have
willingly, voluntarily and with full knowledge of my rights under the law or with full
opportunity to consult a lawyer in connection therewith, executed this Agreement.
Unless I notify the Human Resources Department in writing of my change of
address, all notices to me shall be deemed sufficient if sent to my address stated
above.
By your signature, you attest to the truthfulness and authenticity of all your
submitted papers and references and you expressly authorize the Company to
conduct the necessary background verifications.
Acceptance and Code Affirmation
Upon signing this letter I accept the terms described above. I also affirm that I have

condition of employment to comply with the Code as amended and revised from
time to time.

Appendix A Personal Employment Terms and Conditions

Your employment shall commence on the Commencement Date and, subject
to satisfactory completion of your probation period, shall continue until your
retirement age of sixty (60) or unless earlier terminated in accordance with
law or paragraph 4 of Appendix B.

4. Remuneration

The payment for the gross monthly remuneration shall be pro-rated to the
number of days of work you actually rendered in the month, taking into
consideration your approved leaves of absence and work schedule.
5. Probation
Your probationary period will be for [insert] months from the date of hire
subject to the standards of regularization which you acknowledge as having
been made known to you on or before your Commencement Date and which
form part of the Rules and Regulations. You agree that in case you do not
receive on or before the last day of your probationary or extended
probationary employment a regularization notice from the Company, your
employment with the Company shall automatically be terminated for your
failure to meet the standards of regularization. You are not allowed without
express written instructions from the Company to report to the Company for
work upon the termination of your probationary or extended probationary
employment, and in case you report and are made to work after such

termination, the same shall be considered as a casual employment on a dayto-day and as-need-may-arise basis.
The standards of regularization shall include (but shall not be limited to)
those enumerated in Paragraph 1.3 of Appendix B.
6. Annual Leave
You will be entitled to [15] working days of annual leave with full pay for
every of service rendered, which is inclusive of the legally-required five-day
service incentive leave.
Not later than January of the year, you shall submit to the Human Resources
Department your intended leaves dates. The Company has the exclusive
prerogative to approve, schedule, re-schedule or change your intended leave
dates, taking into consideration those who timely submitted their intended
leave dates, the reason for the leave, and the demands of the business of the
Company, particularly (but not limited to) where exigencies so warrant.
APPENDIX B Personal Employment Terms and Conditions
1. Employment
1.1Your employment is contingent and subject to your obtaining and
maintaining a valid applicable work permit and/or such other
permission, licence or registration as may from time to time be
necessary to enable you to effectively carry out and continue your
duties and employment, and to your representation and warranty that
you are not subject to any restriction or prohibition that would prevent
you from performing your duties or relevant/applicable and assigned
services.
1.2You will not undertake or engage in outside employment or activities
with or without remuneration, conflicting or not with the business of
the Company, unless you have the prior written approval of the senior
management of the Company or its delegate. Outside activities shall
include (but are not limited to) activities as a director, officer, partner,
sole proprietor, consultant, employee or controlling stockholder of any
business.
1.3Your employment and continuation thereof is subject to:
(a) the satisfactory completion of all pre-employment screening
processing, including various background checks, medical
requirements and examinations by the Company designated
physical or medical service provider, execution of any other
forms necessary for employment, and the submission to the
Human Resources Department of the following documents within
seven (7) days from Commencement Date: employment
references, character references, passport size photograph, birth
certificate, marriage certificate (if applicable),
childrens/dependents birth certificate (if applicable), SSS Form
E-1 and NBI/local Police Clearance;
