Only a maximum total of three (3) Set-Top-Box (STB) may be purchased by an employee or personnel (collectively, the
Employee)
Payment for the STB may be in cash or through one-time salary deduction (the SD Term) for regular employees. All
regular employees who will avail of the SD Term must accomplish an Authority-to-Deduct Form that will be submitted to the
HR Group for evaluation. ABS-CBN reserves the right to disallow application of the SD Term payment if such will result to
violation of ABS-CBNs policies including the net take home policy.
For non-regular employees, cash payment will be the only availment option
Accomplished Kapamilya Muna Availment Forms must be submitted by the Employee to their respective HR Account
officer, who will then turnover to DTT Logistics for order processing.
DTT will send a notification to the Employee regarding the claim schedule for the ordered box/boxes.

a. Regular employees of ABS-CBN Corporation as of 2014;


b. Non-regular employees consisting of:
i. Personnel under the IJM program of ABS-CBN;
ii. Consultants;
iii. Contractuals;
iv. Project employees
v. Direct-hires
Accomplish the DTT Employee Program Kapamilya Muna Availment Form (AF) and submit to
your respective HR Account Officer for assessment and approval. All approved Availment
Forms (AF) will be submitted to DTT Logistics or Marketing by the respective HR Account
officer. Notwithstanding such approval, the DTT Group may deny or disallow any application by
an Employee for any technical reason such as but not limited to poor or absence of reception
in the address indicated in the AF.
For Regular Employees, payment options are one-time salary deduction or cash payment.
Should salary deduction be chosen, the regular employee must secure and accomplish the
Authority-to-Deduct Form from his HR Account officer which shall be evaluated by Payroll. The
company reserves the right to decline or disallow any AF if such will violate company policies
including the net-take home policy of the Company. For non-regular employees, cash payment
shall be the sole mode of availment.
Once all necessary forms are approved, the Employee is already qualified to avail of the DTT
Employee Offer:
a. Each employee can avail only up to three (3) set top boxes (STB), at the price of Two
Thousand Pesos (Php 2,000) for each STB.
b. Each STB comes with an ABS-CBNmobile Prepaid Sim Card with Fifty Pesos (P50.00)
electronic load
A notification e-mail or text message will be sent to all those whose AF have been approved
regarding the pick-up schedule of the STB.

6. The Employee has seven (7) calendar days from receipt of the STB to return or exchange the STB to ABSCBN due to inherent defects of the STB or if there is no signal in the address indicated above. ABS-CBN shall
verify and confirm the complaint prior to any acceptance or replacement of the STB.
7. The warranty card shall be submitted by Employee to ABS-CBN on the same day of STB issuance.
8. In the event of Employees separation from ABS-CBN, for whatever reason, prior to the complete payment of
the STB, Employee agrees and authorizes ABS-CBN to deduct the payment, or the balance as applicable,
against any amount due to Employee arising from Employees separation from ABS-CBN. If the amount due
to Employee is not sufficient to cover the balance, Employee agrees and undertakes to pay ABS-CBN the full
balance within five (5) days from receipt of notice and prior to the effectivity of Employees separation.
Employee acknowledges that Employee shall not be cleared from my accountabilities until and unless ABSCBN has received the full and complete payment of the Total Amount.
9. Ownership over the STB shall be transferred to the Employee only upon full and complete payment of the
Total Amount. All risks to the STB, including losses or damages thereto, shall be for the sole account of
Employee from the time of receipt of the STB from ABS-CBN. ABS-CBN shall be held free and

harmless from any claim or liability, except for the express warranties stated in the STB, in
relation to or arising from the Employees use and ownership of the STB
10. Employee acknowledges that Employee has read the terms and conditions under this Agreement. Employee
agrees that Employees signature to this Agreement constitutes Employees consent to the terms and
conditions herein including the authority of ABS-CBN to deduct the Total Amount from Employees salary in
accordance with this Agreement.
