The applicant is previously enrolled in only one
school in her entire freshmen year.
Nursing applicants should pass the Nursing
Students Qualifying Exam and sign a waiver that
only subjects that are of the same course
description as those of SPUQC and which meets
the cut-off grade of 83% for nursing will be
credited.

Statement.
Notarized affidavit of support including bank
statement.
Scholastic records Authenticated by the Philippine
Foreign Service Post located in the student
applicants country of origin or legal services
Photocopy of Data page of the students passport
showing date and place of birth certificate or its
equivalent duly authenticated by the Philippine
Foreign Service Post.
