OBJECTIVES
To provide direction during data recording practices, preparation of test reports and
data checking practices on activities carried out so as to ensure the basic of our products
are commonly being reported on comprehensive, accurate, clear and unambiguous manner.

SCOPE & REFERENCES

2.1 SCOPE
Applies to all Test Reports being prepared by respective inspection personnel of
ITECSOLUTIONS QUALITY GROUP PTE LTD

3.0

PROCEDURE
This procedure for preparation of test report consists of three stages:
Test Recording and Reporting
Test Worksheet
Test Reports
3.1 TEST RECORDING AND REPORTING
The quality manual and related quality documentation shall specify the
laboratory's policies and operational procedures established to meet the
requirements of SAC SINGLAS, which cover key matters including the
recording and reporting practices including details of worksheet and report
formats and data checking practices
In order for the test result to be reported accurately, it is important to record all
Raw Data in this practice and shall be recorded at the time they are made into a
worksheet or personnel record book. All recording shall be made in ink to
ensure that results, value or data obtained are not altered or change.
Any correction or changes in the data sheet shall be strike out with the correct
value entered alongside and the signed by the respective personnel making the
connection, at no time shall these data be corrected with liquid correction fluid.

3.2 TEST WORKSHEET

This test worksheet is used in the preparation of test report and is important that it
shall has the following:
3.2.1 CLARITY
As the majority of test reports are first prepared in writing by the
respective inspection personnel, it is essential that they are clearly
written, checked against all raw data work sheets and laid out so
that the risk of misreading is entirely eliminated if not reduced.
3.2.2 ACCURACY
Prior to submitting a report for typing, the inspection personnel
shall thoroughly check the body of the report are accurately
reflected on all the activities carried out with reference to the
purpose and all necessary substantiating documentation,
calibration certificate, radiographs, raw data ,etc. are attached.
3.3

TEST REPORT
With the information from the worksheet the test report is prepared
onto the relevant format as in the appendices together recording but not
limited to the following:

3.3.1 DATA CHECKING PRACTICES

Managers shall then review for correctness of record against raw
data, or worksheet and or for conformity to reference purchase
order / specification prior to forwarding to Client.
Test reports shall be in triplicate of which one original plus a
copy inclusive of all necessary attachment shall be hand carried over
to client, while the third copy with all necessary attachment inclusive
of all raw data, acknowledged with dated signature and if possible
stamped by client for record, filing and traceability.
3.3.2 TRANSMIITAL OF TEST REPORT
Test reports and or results shall not be electronically transmitted
either by facsimile, telephone or telex, this is to ensure the clients
confidential and proprietary rights.
3.3.3

HEADING AND CONTENT

In order to maintain uniformity and to provide accurate detail of
activities, the attached APPENDICES are sample formats but not
limited to the contents shall be the accepted manner of reporting.

3.3.4

MATERIAL AMENDMENTS
Material amendments to a Calibration certificate, test report or test
certificate after issue shall be made only in the form of a further
document, or data transfer including the statement "Supplement to
Calibration Certificate [or Test Report or Test Certificate] , serial
number... ... [or as otherwise identified]"
or equivalent form of
wording. Such amendments shall meet the relevant requirements
of the preceding and relevant paragraphs of this document.

3.4

CONTRACTUAL VARIATIONS
Any variations to the above requirements will be indicated in the
assignment instruction issued for the work activities.

RECORDS
Test report / test worksheet shall be kept and retained by the Operation Manager for at least
five years.
