Certification Application Terms and Conditions

This application is the property of International Institute of Business

Analysis and is protected by copyright.

This application is confidential once it is completed. The CBAP Application,

exam results and all other CBAP certification program-related materials
are kept private and confidential. This information will not be disclosed to
anyone other than the applicant without the applicants consent. To request
the release of an exam result to a third party, IIBA must be provided with a
written request identifying which exam result may be disclosed and the
person or organization to which the result should be disclosed. Any violation
of the IIBA Privacy Policy will be subject to disciplinary action(s) as
described in the IIBA Constitution. Requests should be submitted to the
address listed on our website at www.iiba.org. Notwithstanding any other
confidentiality obligation owed by IIBA to the applicant, in the event
that the applicants application fee and/or examination fee has been
paid by a third party (Sponsor), the applicant hereby irrevocably
authorizes and directs IIBA to release Confidential Results Information
to that Sponsor.

By submitting this application to IIBA, I understand that IIBA may contact me

regarding my application.

I consent to IIBA collecting and storing the foregoing information and

contacting my stated references to confirm any or all information on my
application form.

I understand that the CCBA application fee is a non-transferable and

non-refundable fee and that if my CCBA application is approved, I have
one year from the time I receive my approval email to take the CCBA exam
before my application lapses. If my application lapses, I must reapply for
certification and repay the application fee.
Upon successful completion of the Certification of Competency in Business
Analysis exam, I consent to the following information to be part of the
CCBA directory that is available on the IIBA website: first and last name,
city, state/province/territory, country and certification date.
By making this application for the CCBA designation, I agree to abide by the
terms of use of the CCBA certification mark as set out below:

CCBA recipients in good standing are granted a limited, nonexclusive, and revocable license and permission to use the following
certification marks and trade-marks (collectively "CCBA Marks")
for the term that the CCBA recipient is in good standing, in
accordance with the terms and conditions that follow:

The CCBA recipient agrees (1) to use the CCBA Marks in

accordance with the standards set out in theIIBA Corporate Identity
Standards Manual; and (2) to terminate use of the CCBA Marks in
the event the certification is revoked or the recipient is no longer in
good standing. The CCBA recipient agrees that IIBA reserves the
right to revoke the license herein at its sole discretion at any time.

I understand that it is necessary for IIBA to collect personal information

solely for the purpose of working with vendors, such as application service
providers.

I consent to the collection and disclosure of information that is made for this
purpose and only when necessary for working with such vendors and service
providers.
