1. This Form shall be used by the S&T official in the following cases:
a. For attending to failures.
b. For removal of interlocking gear of any kind.
c. For carrying out work in connection with lever frames, point track circuits, points, signals etc.,
involving disconnection.
2. Before commencing the work, the person in-charge of the work shall fill up the first part of the block
copy and obtain thereon the SMs signature.
3. Then, the person in-charge of the work shall fill up and sign the Disconnection Notice, detach and
hand over to SM.
4. The signature of the SM on the first part of the block is the authority for the signal branch to
commence the work.
5. During the time the work is in progress, all signals (excepting those which control movements over
lines which are not only isolated, by the setting of the points, from the line affected by the work but
are also maintained in the isolated condition by the interlocking during the passage of the train)
shall be treated as out of order and placed at ON by the signal branch staff.
6. If it is necessary to pass train or perform any shunting movement, SM shall advise the Official incharge of the work, by a memo stating in which position the points are to be set.
7. With the permission of the Official in-charge of the work, SM or other authorised staff shall arrange
to set and clamp the points to prevent any interference until the completion of train/shunting
movement.
8. SM shall observe rules 3.68 to 3.72 for allowing train/shunting movement to pass the signal/s at
ON.
9. After completion of train/shunting movement, the padlock and clamp shall be removed by the SM
or other authorised staff.
10. Then the Official in-charge of the work can resume work on the gear.
11. After the work is completed, Official in-charge of the work shall fill up second part of the block
copy and obtain thereon the SMs signature.
12. Official in-charge of the work shall also fill up and sign the Reconnection Notice, detach and hand
over to SM.
13. The signature of the Official in-charge of the work on the Reconnection Notice is the authority
for the SM to resume normal working.

the following points may be added
a. Disconnection for durations upto one hour should normally be allowed by SM on duty depending
upon trains in the section.
b. If Disconnection notice is not allowed by the SM, it should be requisitioned again by the S&T staff
and allowed by control depending upon flow of traffic on the section.
c. Otherwise the available slot may be indicated by the control to the S&T staff.
d. In extreme emergencies, if the maintenance /repair of S&T gears is urgently required to avoid
an accident, the same may be suspended with the approval of Sr.DSTE.
e. For works requiring disconnection of more than one hour, a Disconnection schedule jointly
signed by Sr.DSTE, Sr.DOM, Sr.DEN & Sr.DEE/TRD should be issued and notified to all concerned.
f. For Disconnection/Maintenance likely to last more than a day
i. Temporary working instructions must be issued.
ii. SM on duty shall be responsible for ensuring that all the points over which the train is to
pass are correctly set, clamped and padlocked.
iii. S&T staff may restrict the aspect of signals to give only yellow aspect as permitted in
Temporary working instructions. Thereafter, the SM may take off the signals after
ensuring correct setting and locking of points.
iv. Temporary engineering indicators with speed indicator of 15 Kmph may be placed at the
site.
