I.

INSTRUCTIONAL COMPETENCE
A. Lesson Planning and Delivery
At the end of the First Semester 2014-2015, I had achieved the following:
1. Formulated/ adopted objectives of the lesson which were:
a. specific

2. Selected content and prepared appropriate instructional materials/ teaching aids which were:
a. congruent
b. relevant
c. appropriate
d. adequate
3. Selected teaching methods / strategies which
a. were suitable to learning ability of students
b. provided opportunity for greater participation of the students
c. generated teacher- learner / learner-teacher interaction
d. maximized utilization of available audio-visual materials
4. Related new lesson with previous knowledge/ skills which were

b. conducted with a review/ recall of a previous lesson

C. LEARNERS ACHIEVEMENT
1. Improved learners achievement level by 1.69% over pre-test

Most of the time, I have acted immediately on needs/ requests in accordance with the

Most of the time, I have rendered service over and above the regular functions and
even beyond the regular time.

Most of the time, I started action, projects and perform task without being told and
supervised

I always have shown polite, kind and thoughtful, behavior toward the public /clientele
in manner of speech and actuations
Human Relations
Most of the time, I integrated concern for people at work, office clientele and
supervisor-subordinate relationship into work situation.
Leadership
Most of the time, I always guided, influenced, motivated and developed confidence of
subordinates to work as a team and accomplish assigned tasks, leading the
organizational unit to achieve its goals and objectives enthusiastically.
Stress Tolerance
Most of the time, I have shown stability of performance under pressure of
opposition.
Fairness/ Justice
I always conformed to usual principles of law, is just and unbiased
Proper attire/ Good Grooming
I always dressed in accordance with proper decorum and/ or prescribed rules and
regulations and is neat in appearance

III. PUNCTUALITY AND ATTENDANCE


Punctuality
At the end of the first semester of 2014-2015, I arrived on time with not more than three (3)
times tardy / under time during the last 6 months.
Attendance
At the end of the first semester of 2014-2015, I attended in school with not more than (2)
days absent.
