RELEASE & WAIVER OF LIABILITY

I, the signee below, will hereby agree to the following conditions set forth by ON VINYL MUSIC
and all affiliates including the ON VINYL MUSIC management staff, NATHALIE PHAN and
ALEX DUCK, boat rental vendor, BEACH FRONT BOAT RENTALS, all bands and performers,
and transportation provider, LONE STAR PARTY BUSES.
The event held April 18, 2015 will hereby be referred to as LAUNCH EVENT.
By signing below, I fully consent to the following conditions:
1. I understand that LAUNCH EVENT is an 18+ event and I meet this requirement.
2. I am aware of the zero-tolerance policy on underage drinking enforced by BEACH FRONT
BOAT RENTALS and LONE STAR PARTY BUSES as stated in each vendor agreement
form. I agree that I will not consume and/or bear alcohol if I am under the age of 21.
3. I acknowledge that the #1 priority of all parties involved is safety and will not act in any way
that threatens the safety of others or myself.
4. I am aware the ON VINYL MUSIC and LAUNCH EVENT is not affiliated with St. Edwards
University or any public or private college or university and its student organizations.
5. I waive my right to take legal or financial action against ON VINYL MUSIC or any LAUNCH
EVENT affiliates for any emotional or physical injury or death endured during LAUNCH
EVENT. I understand that I am liable for any physical or emotional damages that I incur to
myself, other guests, the boat crew and/or captain, the party bus driver(s), ON VINYL
management and staff, or any persons involved with LAUNCH EVENT.
6. I agree to exercise common sense and will act responsibly throughout the entire event,
including on the bus trip to the marina and back, on the marina, and on the boat. I
understand that if I cause disorderly behavior or if I conduct myself in any way that is rude,
offensive, dangerous, illegal, or otherwise disruptive, including but not limited to urinating on
marina property, causing damage to the boat, buses, and/or any equipment not belonging to
myself, being a threat to myself or others, engaging in the use of illegal substances, sexually
harassing any person that may or may not be involved in LAUNCH EVENT, verbally or
physically abusing another person, or any other behavior deemed unsatisfying or unruly by
the ON VINYL MUSIC management team, I assume responsibility of all financial and legal
penalties and/or will be ejected from LAUNCH EVENT at the discretion of the ON VINYL
MUSIC management team and BEACH FRONT BOAT RENTALS and/or LONE STAR
PARTY BUSES. If there are any fines or penalties incurred, I agree to pay them in full.
7. I am responsible for all of my own belongings and valuables and hold no one else financially
responsible for any loss or damage of my belongings.
8. I understand that I must comply with all requests made to me by the boat captain, boat crew,
bus drivers, and ON VINYL MUSIC management during the LAUNCH EVENT.
9. I will not bring any unwanted items on the buses or boat including glass, sharp objects,
highly flammable items, items that cause excessive noise, alcohol (if under 21 only) or illegal
drugs, beer bongs, jello shots, or funnels.

10. I understand that the party is rain or shine, but will be cancelled in threatening conditions
such as if there is lightning. I acknowledge that I may not be granted a refund unless the
party is fully cancelled by ON VINYL MUSIC, that all sales are final even if I do not attend
the event and that all tickets are non-transferrable.
11. I understand that I will not be allowed in the bus or boat if I appear intoxicated and will be
ejected from the party if I appear to be belligerent or disruptive. I acknowledge my own
personal responsibility for my own well-being at all times during the event. ON VINYL
MUSIC and all vendors and affiliates are not financially responsible for legal or medical fines
for existing conditions or bodily damage, including injury, or death, caused by recklessness
or negligence of myself, other guests, or any party involved with LAUNCH EVENT.
12. I acknowledge that I may be recorded, filmed, or photographed by other attendants,
vendors, or ON VINYL MUSIC management and staff. I am aware that any images
portraying me may be used commercially or non-commercially by any parties involved in
LAUNCH EVENT and may be viewable by parties not involved in LAUNCH EVENT.
13. I will not attempt any dangerous behaviors or acts in the water that may threaten the safety
of myself or others including parties not affiliated with LAUNCH EVENT and understand that
I am liable both legally and financially for any physical injury, death, or damage to equipment
and any emotional duress that I cause.
14. ON VINYL MUSIC and all affiliates reserve the right to eject anyone behaving in a disorderly
fashion without compensation or refund of ticket sale.
15. I, my family members, heirs, or any legal representatives release waive, discharge, and
covenant not to act legally against LAUNCH EVENT management, vendors, and all other
affiliates including BEACH FRONT BOAT RENTALS, LONESTAR PARTY BUSES, and all
performers, musicians, and bands for injury or death incurred by their negligence or other
acts for the duration of the event. I have read the above release and waiver of liability and
fully understand its contents. I voluntarily agree to the terms and conditions stated above.
