Engine 6 arrived at 8 Street and 5th Ave to find street lights out on 8th street at the 8th,
7th, 6th, and 5th ave intersections. A manhole midblock on the east side of 8th Street was
producing small amounts of white smoke. Engine 6 asked dispatch for an additional
Engine and Aerial to help secure the area and to look for additional hazards. Enmax and
Atco Gas were requested as well. The entire block was taped off using "fire Tape" anc
access was denied. Investigation in the block found several other manholes producing
minimal amounts of white smoke. After no additional hazards were found in the area
Engine 49 and Aerial 6 were cleared. Engine 6 crew removed the manhole cover that was
producing light smoke to visually inspect the area. Engine 6 Captain ordered the manhole
to be put back on to restrict fire spread.

smoke and heat to other vaults.
The minimal smoke production continued as CFD waited for Enmax to cut the power off

area. Engine 6 personnel continued to restrict access to the area. Wind was light from the
north. Approximately one hour after CFD arrival, a large explosion was heard
underground from the manhole that was producing the most smoke on 8th Street. Large
volumes of black smoke and purple flame erupted from the whole. Battalion Chief.

'of Engine 6 became "Operations". A second alarm was called. Shaw
cable arrived on scene.
As the fire grew in intensity, CFD expanded traffic and crowd control with the aid of CPS.
Evacuation crews were mobilized in the immediate area to aid people coming out of
building in the immediate area where smoke created a safety concern. The ground level
and sublevel areas of all residential buildings from I Ith Street to 5th Street, and from 5th
Ave to 8th Ave SW were investigated to ensure no hazards were found. The Kirby centre
had a designated crew investigate the welfare of the occupants and no problems were
found, other than the main power was off and backup power alternatives were functioning.
A member of ATCO Gas was accompanied by CFD to 840 - 6 Ave SW to inspect the
mechanical room. The backup generator was shut off due to unsafe heat and gas
accumulations. During the process the ATCO worker suffered 2nd degree burns to the
hospital. Enmax eventually turned off the power, after approximately two hours from the
initial call, during which time approximately 12 - 18 explosions were heard underground
as the fire intensified creating large volumes of black smoke. Once confirmation was
made four times, with ENMAX that the power was off, CFD applied water and foam into
the underground vault. After approximately 30 minutes the fire was extinguished. Hazmat
found minima] hazardous readings in the area. Once the fire was extinguished, air quality
was safe in the area. After ENMAX had inspected the area, approximately 30 minutes
after the initial extinguishment, another loud explosion was heard and District Chief
stated a torch of flame erupted from the manhole, perhaps 20 - 25 feet in height. It
was stated by ENMAX on scene that they had mistakenly read a map and reenergized the
area that had been on fire originally. Once again the power was switched off and CFD
applied water and foam until the fire was extinguished and the area had been cooled.
Apparatus was returned as tasks were completed until eventually only 6 Engine and
Command remained on scene. Engine 6 cleared the scene after a plan was put into place
for the systematic investigation of all residential building in the grid that had lost power.
Please note: Engine 6 advised dispatch of the need for Enmax within two minutes of their
arrival on scene. Exact times would need to be produced, however it was just under two
hours from CFD's arrival when Enmax s?ave confirmation that the power was turned off to
the area involved in the
fire.

crew was enroute and they would get the power turned off upon their arrival. Immediately
after the power was turned off, a fire attack crew, lead by Rescue 8, applied water and
foam to the underground vault involved in the fire. They continued to cool the area with
water even after the fire was extinguished as requested by Enmax. A vacuum truck sucked
all of the foam and water from inside the underground vault to expose the area of origin.
After the fire was extinguished, cooled, and cleaned out by the vacuum truck, CFD crew
witnessed another explosion in the vault and a large flame erupt from the manhole which
was unexpected. Enmax stated a error had been made and Enmax had turned the power

back on in the area involved in the fire. Once again CFD extinguished the fire and cooled
the area in the underground vault, after it was confirmed the power was off. This
secondary event certainly created an additional situation which could have been life
threatening to crews on scene. No warning of such a potential explosion or rekindle of fire
condition was received from Enmax.

Central Chief arrived at 5th Ave and 8th Street at 2046hrs. On arrival found smoke co rung
from an undergroung electrical vault. After size up, found Engine 6 had secured the scene,
contacted Enmax and began searching buildings downwind from incident.
Moved command to the CDC Van and monitored the situation. CDC assumed command
and Captain
took the role of Operations. Reported incident conditions to
Battalion Chief
While standing by, waiting for Enmax to shut the
power down, observed explosions and fire growth from the underground vault. Additional
resources were requested to provide a safety team. ISO was established with West District
Chief

arrived on scene.
About an hour and a half after CDC arrival, observed power outage to many buildings in
the area. At 23:36, command was moved to theFire Command Unit.
After Enmax turned of the power, crews extinguished the fire. Crews were establishec
which checked buildings in the area for hazardous conditions. Power status, elevator
status, emergency power and fire alarm systems were checked . Type of occupancy was
also checked and noted.
After the fire was originally extinguished and water was applied to aid in the cooling
process, a vacuum truck removed all water and foam from the vault. A additional
explosion and fire occurred, with no one in the vault. Once it was confirmed the power
was off once again, CFD extinguished the fire once again.
Under the direction of EOC, assigned crews to check occupancy and building status, o|f all
buildings in the affected area.
Releaved by day shift at approximately 09:00 on the 12th.

At 2015 hours on Saturday October 11, 2014 we were dispatched to a fire. Thirty-five
units were assigned to this incident. Seventy-five personnel responded. We arrived on
scene at 2019 hours and cleared at 0041 hours. The incident occurred at On 5 AV SW at 8
ST SW, CALGARY. The local station is 06. The general description of this property is
street or road in commercial area. The primary task(s) performed at the scene by
responding personnel was extinguishment. No mutual/automatic aid was given or received.
One civilian casualty was associated with this incident.
Alarm number 4073075 has been assigned to this incident.
Hazmat 4 responded to scene on second alarm. Initially tasked with contacting Alberta
Environment, as they were contacted by dispatch previous to arrival. No adverse affects to
the environment were noted, other than smoke from the fire, Which is not a reportabie
occurance.
Hazmat 4 was reassigned to checking businesses and buildings in affected area for people
and air quality. No significant change in air quality was registered on our ITX monitors.
Retasked throughout the event to monitor air quality in buildings directly affected by the
fire and assist individuals in returning to there dwellings in the immediate area.
A hazardous waste disposal permit was issued to Enmax as their own Vactor unit was used
to empty vault of fire fighting foam mixed with water.
