FACTS OF THE CASE:

On cited TDPO, this office received a complaint from one Mrs. Elinita
Dimanakawan nee Moo, that there was a Drug Laboratory in their Compound at #45A
Duralite Comp. The element of this office is to verify the complaint by the use of
conducting Test Buy, Surveillance and Casing. After operation conducted in cited TDPO
and the complaint is (positive).This Office coordinated to RPPO R4A and Rizal
PAIDSOTF. Which is the cause of the arresting the suspect Mr. Ian Tan y. Go, 35 years
old, married, Sales Director, native of Isabela Province whos violating R.A 9165 Sec. 8.
The suspect was brought to Rizal Provincial Police Office for proper disposition likewise
together with the following evidence of 50 KILOS of Crystalline Substance, Shabu and
other Paraphernalia was brought to (RPCLO) Rizal Provincial Crime Laboratory Office.

2. In connection with the above reference please be informed that 00A 1:30pm of July
11, 2014, the complainant of one Mrs. Elinita Dimanakaw nee Moo, 36 years old,
married, house wife, native of Samar and presently residing at #45C Duralite
Compound Brgy. Maly San Mateo Rizal personally appeared to this station complaining
that there was a Drug Laboratory in their compound at #45A Duralite Compound.
Allegedly Ian Tan y Go Manufactured Illegal Drugs.
3. After receiving the complaint P/Insp. Santos Station Commander of PCP 5,
commanding PO3 Amor and PO1 Cruz to verify the compliant by the use of
surveillance, test buy and casing.
4. Furthermore, arrested suspect together the confiscated pieces of evidence 50 KILOS
of Crystalline Substance or suspected Shabu and other Paraphernalia was brought to
(RPCLO) Rizal Provincial Crime Laboratory Office.

With legal age, Police man after having been duly sworn in accordance with the law,
Hereby depose and state that we are Bonifide Members of San Mateo Police Station
with tour duty of broken time.
That OOA 1:30am of July 11 2014 the one Elinita Dimanakawan nee Moo 35 years old,
married, house wife, native of Samar, presently residing #45C Duralite Comp. Brgy.
Maly San Mateo Rizal. Complaining that there was Drug manufacturing at #45A Duralite
Comp. Brgy. Maly San Mateo Rizal, Allegedll one Manong Joke manufactured illegal
drugs.
Base on information given by the complainant to SPO4 Joe Cats, informed our station
commander to conduct the following operation, (surveillance, test buy and casing) to
verify the complaint of one Elinita Dimanakawan nee Moo riding with the following
vehicle (Black Mitsubishi Van with plate # GMA 023, white KIA SUV with plate # CBN
00, Green Honda , Van with plate # RNP 009, Blue Chevrolet L300 with plate # IBC
013) together with the complainant the team of SPO4 Joe Cats proceed to the area
unfortunately the target was not in the area therefore the operatives back to station.
That on July 11 2014 OOA 1000H to 1300h casing operation was conducted to verify if
the complainants tell the exact location of target area and the result of casing is positive.
That on July 11 2014 OOA 1700H to 1900H the team of SPO4 Joe Cats conducted a
Surveillance to the suspected area to verify that Manong Joker was in the area after a
while the result of operation is positive.
That on July 11 2014 OOA 1400h to 1600H a Test Buy operation was conducted to the
area to verify that there was a illegal drugs on the target area, On that point PO2 Liza
was perform an undercover to be a poseur buyer immediately ready a (500) pesos bill
with serial No. PLE 081965 and marked CB on its back upon test buy operation PO2
Liza go to the area and few minutes unknown person exit to the door of target area and
tell PO2 Liza that Pare meron ako dito kaliwaan lang lowest 500 pesos. PO2 liza
grab the items and give the marked money to the person and the said items transparent
plastic was brought to RPCLO for verification,
According to the Rizal PCLO the result of Laboratory Examination was Positive the
substance inside in transparent plastic is Methamphetamine Hydrochloride.
That I SPO4 Joe Cats immediately prepared proper documents in accordance with the
communication of other Law Enforcement in PDEA and PAIDSOFT.
After the preparation of document, exactly 1:00am of July 12 2014 the operatives of San
Mateo Rizal Police Station /PAIDSOFT/ and PDEA conduct a drug operation by the
virtue of Search warrant and Warrant of Arrest at #45A Duralite Comp. Brgy. Maly San
Mateo Rizal 350 meters from the station.
Upon operation OOA 1:30Am of July 12 2014 Manong joker was captured by the
operatives and informed the purposed of the arrest and the search warrant for his
property inside his house and immediately brought to San Mateo Police Station.
The suspect was informed his Constitution Rights and subsequently brought to Amang
Rodriguez Rizal Hospital.
That w executing Affidavit of arrest that the veracity of forgoing statement, that the
affiant saying naught.

That I am a Bonifide Member of San Mateo Rizal Police Station assigned as

investigator, with a tour of duty of a broken time.

That on OOA 1:30am of July 11 2014 the one Mrs. Elinita Dimanakawan nee Moo, 35
years old, housewife, nature of Samar, and presently residing at #45C Duralite
Compound Brgy. Maly San Mateo Rizal. Personally appeared to this office complaining
about drug manufacturing at #45A Duralite Compound Brgy. Maly San Mateo Rizal.
Allegedly Manong Joker manufactured prohibited drugs.

That base on the positive result of the following operation that we conducted this station
decides to provide document in accordance with the communication with the law
enforcer agency RPAIDSOFT likewise to PDEA R4A.

That exactly July 12 OOA 1:00am the operatives of RPAIDSOFT and PDEA registered
their incoming drug operation in #45A Duralite Compound Brgy. Maly San Mateo Rizal.

That OOA 1:30am the operatives was captured Manong joker attempting to escape with
his Honda wave blue motorcycle with plate number 6013 NT. The suspect was brought
to San Mateo Police Station for proper disposition.

That during confrontation the identity of suspect was revealed, one Ian Tan y Go 35
years old, sales director, native of Isabela Province and presently residing at #45a
Duralite Compound Brgy. San Mateo Rizal.

The suspect was apprised his Constitutional Rights, and brought to Rodriguez Hospital.
