AnapplicantwillfillForm49Aonlineandsubmittheform.
Ifthereareanyerrors,rectifythemandresubmittheform.
Aconfirmationscreenwithallthedatafilledbytheapplicantwillbedisplayed.
Theapplicantmayeithereditorconfirmthesame.
Onconfirmation,anacknowledgementwillbedisplayed.Theacknowledgementwillcontainaunique15digitacknowledgementnumber.
Theapplicantisrequestedtosaveandprintthisacknowledgement.
'Individual'applicantsshouldaffixtworecentcolourphotographswithwhitebackground(size3.5cmx2.5cm)inthespaceprovidedintheacknowledgement.
Thephotographsshouldnotbestapledorclippedtotheacknowledgement.TheclarityofimageonPANcardwilldependonthequalityandclarityof
photographaffixedontheacknowledgement.
(h) Signature/LeftThumbImpressionshouldonlybewithintheboxprovidedintheacknowledgement.Thesignatureshouldnotbeonthephotographaffixedon
rightsideoftheform.Incaseofapplicantsotherthan'Individuals',theauthorizedsignatoryshallsigntheacknowledgementandaffixtheappropriatesealor
stamp.Thesignatureshouldnotbeonphotograph.Ifthereisanymarkonphotographsuchthatithinderstheclearvisibilityofthefaceofthe
applicant,theapplicationwillnotbeaccepted.
Signature/Lefthandthumbimpressionshouldbeprovidedacrossthephotoaffixedontheleftsideoftheforminsuchamannerthatportionof
signature/impressionisonphotoaswellasonacknowledgement.
(i) Thumbimpression,ifused,shouldbeattestedbyaMagistrateoraNotaryPublicoraGazettedOfficerunderofficialsealandstamp.
(j) AADHAAR
IncaseAadhaarnumberofIndividualapplicantisenteredintheapplicationform,thenproofofAadhaaralongwithsupportingdocumentsistobesubmittedto
NSDLeGov.
IfcopyofAadhaarisselectedasProofofIdentity/Address/dateofbirth,thenitismandatorytoenterAadhaarnumber.
IncaseapplicantisMINOR,Aadhaarofminorshouldbementionedintheapplicationform.(i.e.DonotmentionRepresentativeAssessee'sAadhaarnumber)
Aadhaarnumber(ifprovided)wouldbeauthenticatedusingapplicantsdetailsasmentionedinapplicationform.

(m) Demanddraftandchequeshouldbedrawninfavourof'NSDLPAN'.
(n) DemanddraftshallbepayableatMumbaiandtheacknowledgementnumbershouldbementionedonthereverseofthedemanddraft.
(o) Applicantsmakingpaymentbychequeshalldepositalocalcheque(drawnonanybank)withanyHDFCBankbranchacrossthecountry(exceptDahej).The
applicantshallmentionNSDLPANonthedepositslip.ListofHDFCBankBranches.
