UNDERTAKING

i.

That the information furnished in the admission documents attached with the application form by me is
correct to the best of my knowledge and believe, that incomplete and incorrect information may disqualify
me from admission.

ii.

That at any time during the course of study, if it is found that documents produced by me are fake or false
or information provided is incorrect, it shall render me ineligible for admission/continuation of studies in
the respective course besides any legal action instituted against me in accordance with law.

iii.

That in case my name is struck off from the college roll under clause (ii) above or due to poor academic
performance/failure in a professional examination or ill-discipline in the college/hostel or for any other
reason, which college academic council considers to be justifiable for expulsion from the college or if I
decide to leave the college/hostel at my own, I shall not be entitled for refund of any fee or funds paid by
me to Islamabad Medical & Dental College.

iv.

I shall attend at least 75% of the lectures, tutorials, clinical training and practical as is mandatory
requirement by the PM&DC & affiliated University for the professional examinations of the respective
course, in order to qualify for appearance in the respective university examination.

v.

I shall not be promoted to the next higher class unless and until I have cleared all the subjects of the
present class. I shall pay the full dues/fees of the college for the year if I am repeating the class due to my
failure in the university examination/detention by the authorities.

vi.

I fully understand that hostel accommodation will only be provided subject to the availability. In case of
non-availability, I will not demand any accommodation from the college authorities.

vii.

I will abide by all the college rules, regulations & instructions already enforced or hereafter to be made or
amended by the College Authorities from time to time and that I will do nothing, inside or outside the
college premises that will be contrary to the discipline of the college.

viii.

indulge in politics of any type and will not be a member of any political party/organization/students
federation, nor I will attend any meeting of such Parties/Organization/Federation. I understand that failure
to observe the above undertaking would result in cancellation of my admission/expulsion from the college
and that the decision of the Principal/Academic Council in this regard will be final. I will neither form nor
will join any student union or association or society in the college, except that which is approved by the
college authorities.

ix.

In case, I am reported to be guilty of being involved in any of the aforesaid activities during my stay in the
college, the college authorities can rusticate me from the college temporarily or expel me permanently,
and I shall not have any claim.

x.

If I am reported to be involved in any of the aforesaid activities and consequently I am rusticated, I will
not try to create a law and order situation or instigate the students of the college, or those of any other
institution against the action taken by the college authorities.
xi.
In case there is any dispute between me and the college administration regarding my involvement in a
disciplinary or political matter or regarding the imposition of any penalty or damages on me, the matter
shall initially be referred to the Principal / Dean who shall recommend the penalty or action against me to
be taken.
xii.
The decision of the competent authority of the college shall be final and binding upon me.
xiii.
Student will be dismissed from the college roll if found guilty in moral aptitude, discipline or professional
incompetence without any notice.
xiv.
In case of failure in a subject or in all subjects during academic calendar, the prescribed fee as specified
by the authorities for the subject or for the entire year shall have to be paid by the student. Failure to
comply with shall result in detention or even expulsion from the college.
