parent'sphotoidentification.Theconsularofficerreservestherighttorequire
additionalevidenceiftheconsentisnotarizedbyanonU.S.notarypublic.

Forchildrenage16orolder,thechildsignsforhim/herself(inthepresence
oftheconsularofficer),sotheparentsneednotbepresent,andthereisnoneed
topresentacopyofthechild'sbirthcertificate.
Thechildmustprovehis/heridentity.Ifthechildiseasilyidentifiablebythe
consularofficerbylookingatthepreviouspassport,noadditionalproofmaybe
necessary.However,achild'sappearancemaychangesignificantlyovertime,sothe
consularofficermayaskforadditionalproofofidentity.Pleasebringasmuchproofas
possible.Examplesofitemsthatcanbeusedtoestablishachild'sidentityare:
Primaryorpreferredmethodisgrowingagephotos(i.e.,aseriesoffamily
photographsfromvariousagessincethechild'slastpassportwasissuedfor
