Presentacin
Presentation

Desde su fundacin en 1919, buena parte de la

actividad investigadora y comercial de LETI ha
estado ligada al campo de la inmunologa.
Muestra destacada de esta vocacin es la
Divisin de Alergia, creada en 1941, que pone a
disposicin del especialista los ms avanzados
productos para el diagnstico y tratamiento
etiolgico de los procesos alrgicos.

LETI, en la fabricacin y estandarizacin de los extractos alergnicos, sigue las regulaciones espaolas para
productos inmunolgicos de uso humano (Real Decreto 288/1991 BOE 12/3/91), las directrices para productos alergnicos (CPMP/BWP/243/96) de la Unin
Europea y las recomendaciones elaboradas por el
Nordic Council on Medicines(1).
Los procesos y procedimientos de fabricacin y control se
ajustan a los criterios de las Normas de Correcta Fabricacin (GMP) y estn avalados por la certificacin ISO 9001.

Allergen catalogue

LETI bases the manufacturing and standardization

of its allergen extracts on the recommendations of
the Nordic Council on Medicines(1) and follows the
Spanish regulations for immunological products for
human use (Royal Decree 288/1991 BOE 12/3/91)
and the Note for Guidance on Allergen Products
(CPMP/BWP/243/96) of the European Union.

Para evaluar la potencia de los extractos alergnicos

se han empleado numerosos mtodos. Los mtodos basados en la cuantificacin de la respuesta in
vivo cutnea son los mejor documentados y los ms
fiables para determinar la actividad biolgica de
los extractos(13).
La Unidad de Alergia de Laboratorios LETI, siguiendo
las recomendaciones del Subcomit de Inmunoterapia de la EAACI(14) y de la Academia Americana
de Alergia, Asma e Inmunologa (AAAAI)(15), elabora
los estndares biolgicos de referencia basndolos
en la actividad alergnica total. Con este fin, estandariza sus extractos de referencia in vivo mediante
bioensayos dosis-respuesta y las nuevas preparaciones de extractos alergnicos se comparan con
estos extractos de referencia mediante ensayos de
bioequivalencia in vitro.

Allergen catalogue

Several methods have been used to evaluate the

potency of allergen extracts. The methods based on
quantifying the response of the skin in vivo are the
best documented and most reliable for determining
the biological activity of extracts(13).

Catlogo de alrgenos

Following the recommendations of the EAACI

Immunotherapy Subcomitee(14) and the American
Academy of Allergy, Asthma and Immunology
(AAAAI)(15), the potency of the reference standards
is based on the total allergenic activity. LETI
standardises its reference extracts in vivo by
means of dose-response bioassays and the new
preparations of allergen extracts are compared
with these reference extracts by means of in vitro
bioequivalence tests.

based on the total allergenic activity.

LETI uses the HEP unit, which is defined as:

the activity of an allergen extract is 10 HEP
(Histamine Equivalent in Prick testing) per ml when
the extract provokes a specific skin reaction in the
median sensitive patient with a wheal of the same
size as a wheal provoked by a positive reference
solution consisting of histamine 54.3 mmol/l (for
example histamine dihydrochloride 10 mg/ml),
when both solutions are administered using the
same technique (prick testing) on at least 20
individuals who are clinically allergic and cutaneously
reactive to the allergen concerned (1).

La unidad 10 HEPL se basa en la cantidad de preparacin de referencia interna (PRI) que produce in
vitro el 50% de inhibicin de unin a IgE especfica,
usando el mtodo REINA-Competicin, y en la
experiencia en su uso clnico.

The unit HEPL is based on the quantity of in-house

reference preparation (IHRP) that in vitro produces
a 50% inhibition of specific IgE binding using an
ELISA test based on antigen capture (REINACompetition) and on clinical experience.

La unidad de valoracin utilizada por LETI es la unidad

HEP, cuya definicin es:
La actividad de un extracto alergnico es 10 HEP
(Histamine Equivalent in Prick testing) por ml
cuando produce una reaccin cutnea del mismo
tamao que la que produce una solucin positiva
de referencia, constituida por histamina 54,3 mmol/l
(dihidroclorhidrato de histamina a 10 mg/ml),
cuando ambas soluciones se administran usando
la misma tcnica (prick test) en un mnimo de 20
individuos clnicamente alrgicos frente al extracto
alergnico correspondiente(1).
