Contract for Private Violin Lessons with Jos R.

(Student/Parent Teacher agreement to lesson policies)
A. Location:
Private violin lessons will be offered at Southlake Christian Academys Music Room, which is located
at Hampton Halls Lower Level. Address: 13820 Hagers Ferry Road Huntersville, NC 28078. Call Mr.
Bass direct line in order to access the building after 4:00 PM: (704) 949-2337.
B. Rates:
1. The rate for a private violin lesson with Jos R. Bas is $30 for a half hour and $40 for a full
hour. This fee can be raised at any time provided that a 1-month notice is given in writing or
by email.
2. Payments are to be made monthly, and will be expected at the student's first lesson of each
month.
3. Preferred methods of payment: PayPal (acct: basfam@gmail.com), cash, or checks payable to:
Jos R. Bas.
4. There will be a late fee of $10 per week for payments that are not made in full and on time
and a $25 penalty for returned checks.
C. Cancellation Policy:
1. There is a 48-hour cancellation policy.
2. Mr. Bas will offer lessons during regular school days beginning at 3:30 pm and will observe
the same holidays as per SouthLake Christian Academys master calendar.
3. Absences will be excused if:
a. A student stays home sick from school and a voicemail message is left at (704)
402-9003 by 9:00 am. Remember - If you do not cancel 48 hours in advance of a
lesson or by 9:00 am on the day of the lesson in the case of illness, you will be
charged for the lesson. There will be no exceptions.
b. SouthLake Christian Academys normal school activities are cancelled for
whatever reason (e.g. : inclement weather, etc.)
4. Each student will be allowed 3 total absences throughout the semester. These days will
not be carried over said period of time. Any additional absences will be forfeited without
make up or refund.
5. Unexcused absences: Any absence that does not fit under this contracts definition of an
excused absence is unexcused. Unexcused absences will be forfeited without refund. Students
who have three or more unexcused absences may lose their time slot to someone on the
waiting list.
6. Make-up times are subject to the teachers availability.
D. Withdrawal policy: If your child quits/withdraws in the middle of the month, no refund will be
issued. Students must provide four lessons notice if they intend to stop lessons or are moving out of the
area. These four lessons are to be paid for whether the student wishes to have them or not.
E. Examples of forfeited lessons:
1. Student/parent who fail to bring a signed contract.
2. Student who forgets to come to a lesson.
3. Student who forgets lesson books at home.

4. Student who is not in school for any reason except illness.

F. Additional Services available:
1. Lessons via SKYPE

G. Audio recording, video film, and/or pictures during a lesson or live performance: By signing this
agreement you are giving consent to make use of said devices for non profit and reuse them in print,
internet, or any form of media. You are also releasing Jos R. Bas and/or SouthLake Christian
Academy from all claims, demands, and liabilities whatsoever in connection with the above.
H. By signing this agreement you understand and agree that:
1. As the parent and/or legal guardian, you acknowledge to have voluntarily elected to enroll
your child in private violin lessons with Jos R. Bas.
2. SouthLake Christian Academy assumes no responsibility and will remain free from any
liability, claims or demands on any activity related to private violin lessons with Jos R. Bas.
3. You release, waive, discharge and covenant not to sue and agree to indemnify and hold
harmless Jos R. Bas, Southlake Presbyterian Church, Southlake Christian Academy, the
members of its Board of Education (in their official and individual capacities), administrators,
or employees from any and all liability, claims, costs, expenses, attorney fees, demands,
actions and causes of action whatsoever arising out of or related to any loss, damage, or
injury, including death as a result of any activity related to private violin lessons with Jos R.
Bas.
4. Music develops responsibility because it is a subject requiring discipline and diligent practice.
Because music is an applied study, students will be required to practice daily. Failure to
practice consistently will result in the cancellation of private lessons. Parents, teacher and
student must work together to insure the success of the student in the study of the discipline of
music and in preparation for and attendance of lessons.
We understand and agree to the terms of the contract for services of private music lessons. I will abide by
the terms of this contract.
