US Passport Holders Minor Tourist Visa

The documents in the Checklist are mandatory for your application to be processed.
This list of documents is not exhaustive and is subject to change based on the requirements of
the Indian Embassy / Consulate.
The Indian Embassy / Consulate have the right to request for additional documentation.
2 copies for Walk-in & 1 copy for Postal applicants of signed Checklist with the boxes ticked
must be submitted with your documents.
Wherever parents sign, the signature must match with the passport signature. Applications will
be put on hold and a new application will be asked for if parents signature does not match with
the passport signature.
All copies requested should be clear and eligible.

signed pages should be duly NOTARIZED.

online application. If incorrect contact details are entered, it will cause
a delay in processing of the application.

Transfer Notices and receipt notes are not accepted

VISA MUST BE VALID FOR AT LEAST 3 MONTHS
If legal status of Parent has expired OR if legal status is not available,
CKGS may accept the application but additional proof of extension will
be required.
The decision to whether accept this application will be made by the
Embassy/Consular Officer and will be FINAL in such cases. NO
REFUNDS will be made towards any fees once the application has
been submitted to the Embassy / Consulate.

Name change documentation (if applicable)

If your name in your Passport is different in any way from the name in
the proof of address being furnished, a copy of ONE of the following
documents must be provided
Marriage Certificate showing the name change if the name
was changed at marriage
OR
Official Court Order for the name change,
OR
Endorsement of name change during naturalization process
Enclose copy of Online Payment Receipt (If applicable)
Only if payment is made online for Fees and any optional value added
services like courier, SMS, etc.
Enclose Payment Instrument (If applicable)
Money Order / Cashiers Check / Bankers Check favoring "Cox & Kings
