Preventing process accidents

requires vigilance. The passing of
time without a process accident
is not necessarily an indication
that all is well and may contribute
to a dangerous and growing sense
of complacency.
- The B.P. U.S. Refineries Independent
Safety Review Panel, January 2007

Final exercises:
1. Develop a preliminary list of leadership practices appropriate for each
level of the organization
2. Develop a preliminary accountability plan for critical leadership
practices

I have heard Dr. D.J. Moran speak several times in the last year. I believe
he is on the cutting edge of changing the way we work to increase worker
safe behaviors. His success in impacting behavior outside of industrial
safety has direct application in the field of worker safety. I predict that in
15-20 years, we will be discussing Dr. Moran's work as a step change in
improving behavior-based safety.

safety problems, but do not know how to define what the problems are.
Participants learn:
- Problem Statements What is a problem?
- High Risk and Hazards - Prioritizing
- Human error potential Latent vs. Active errors
- Pre-loss vs. post-loss issues

Case Studies

WHAT IS ROOT CAUSE ANALYSIS?

Root Cause Analysis (RCA) is a methodology for
finding and correcting the most important
reasons for performance problems. It is different
from troubleshooting and problem-solving as
these disciplines typically seek solutions to
specific difficulties, whereas RCA is directed at
underlying issues.
Root Cause Analysis (RCA) is designed to provide
the participant with the knowledge, skills and
tools to begin the process of root cause
assessment using selection methods and tools to
complete an exploration of factors contributing
to an incident.
Loss-producing events may have multiple causes
and require a comprehensive approach to
finding ways to break the chain of causation.
Participants will gain the ability to use RCA tools
to identify potential problems and issues,
complete an analysis, and effectively
communicate results and recommendations for
action planning.

DAY5
Data Gathering
Root cause analysis requires evidence and empirical support. In this
module, participants learn:
- How to utilize:
* Reports, inspections, & maintenance logs
* Quality & regulatory reports
- How to conduct interviews
- How to collect loss, injury, and damage data

Analyzing Data
The value of data analysis depends on the organizations question. In this
module, participants learn not only how to create, but to capitalize on:
- Pareto Diagrams
- Trend Charts
- Process Flow Charts

Managing the Process

Root Cause Analysis is not just a rear-view mirror approach for
documentation, but rather a vital procedure for ensuring safety is
improved in an organization. In this module, participants learn:
- Report writing and recommendation development
- How to assure implementation of recommendations
- How to assess unintended consequences

Case Studies

BENEFITS OF RCA:
Course participants will be able to :
Develop and implement a structured
incident investigations program
Initiate and conduct an investigation
Apply causal factor and root cause analysis
for investigating process and non-process
incidents
Develop appropriate recommendations to
address root causes at various levels to
avoid future incidents
Learn how to structure reports
Practise your new skills via actual industry
examples and workshops
