PETITION
PETITIONER, MOST RESPECTFULLY AVERS THAT:
1. I am over twenty one (21) years of age, having been born on
May 31, 1987 at Baguio City, Benguet, a Filipino Citizen and
a resident of # 4-B DPS Compound, Baguio City and with
Office address at Rm. 777, Ficticious Building, Random
Street, Baguio City;
2. I am a member of the Philippine Bar, having passed the
2018 Bar Examinations and admitted to the practice of law
on May 4, 2019 and entered in the Roll of Attorneys with Roll
No. 77777. A photocopy of my Certificate of Membership in
the Philippine Bar is hereto attached and made an integral
part of this petition as Annex A;
3. I am a member of the Integrated Bar of the Philippines
having paid my membership dues. A photo copy of the
receipt No. 777777 issued on January x, 2019 is hereto
attached as Annex B;
4. I also obtained my Professional Tax Receipt No. 777777
issued on January x, 2019, a copy of which is hereto
attached as Annex C;
5. I have not applied for an appointment as Notary Public nor
do I have a commission as Notary Public elsewhere in the
Philippines;
6. I possess all the qualifications and none
disqualifications for the Office of the Notary Public;

7. I have never been convicted of any crime involving moral

turpitude. A photo copy of said Certification is hereto
attached and made an integral part of this Petition as Annex
D:
8. I am a member of the Philippine Bar in good standing, of
good moral character and have no derogatory records as
certified by the Integrated Bar of the Philippines, BaguioBenguet Chapter. A photo copy of said Certification is hereto
attached and made an integral part of this Petition as Annex
E:
9. There is no pending criminal case filed against me as
certified by the Regional Trial Court-Office of the Clerk of
Court, which certification is hereto attached as Annex F, as
further proof, also attached hereto is a copy of my NBI
clearance as Annex G;
10. I have paid for the filing of this petition for Appointment as
a Notary Public of the City of Baguio where the official
receipt for said payment is hereto attached as Annex H as
proof of payment thereof:
11. My clients desire that I be appointed as Notary Public for
and in the City of Baguio for they need my services as
Notary Public;
12. Attached herewith and made an integral part of this
Petition are three passport
13. Attached herewith and made an integral part of this
Petition are three specimens of passport-size color
photograph taken within thirty days of application, my official
signature and true impressions of my Dry Seal as Annexes
I, I-1;I-2; J, J-1;J-2; and K, K-1;K-2 respectively.
WHEREFORE, it is respectfully prayed that I be
appointed as Notary Public for and in the City of Baguio
effective January x, 202x.
