GMC PLAB blueprint

We allow reproduction of the blueprint and of the additional information about the

Additional information about the blueprint

Vertical axis
The descriptors of disorders have been updated to include the terms used in
contemporary coding. For example gastro-intestinal category is now digestive
system disorder.

In exceptional circumstances, presentations have been duplicated. For example the


presentation of chest pain appears under both cardiovascular and respiratory
disorder.
Horizontal axis
To keep the titles on this axis relatively succinct, there is some additional information
below about some of them:

Applying and integrating scientific knowledge to make a diagnosis should


include recognising acutely ill patients and ensuring that no serious condition is
missed.

Disease factors should include inherited diseases and natural history of disease.

Management plan should include pre-operative, post operative and terminal care
and where appropriate complementary therapy.

Decision making supported by evidence should include applying the principles of


risk management and involving patients and where appropriate their relatives or
carers in decisions about their care.

Good practice in prescribing should include the appropriate use of drugs, route of
administration calculation of dose and fluid requirements and production of a safe
prescription.

Effective communication skills including listening, questioning, explaining,


involving patients in management should include listening and sharing
information, recognising that patients are knowledgeable, discussing options with
patients and explaining the effects of treatment and procedures.

Awareness and sensitivity of the needs and expectations of patients regardless
of their lifestyle, culture, beliefs, ethnicity, gender, sexuality, disability, age or social
or economic status and understanding the patients’ environment and its effect on
their health.

Practising ethically and medico-legally should include understanding and


applying the principles of professional practice set out in Good Medical Practice and
supporting guidance including obtaining informed consent and apply the duties of
doctors under law including:

• Certification of sickness
• Death certification
• Dealing with the Coroner/Procurator Fiscal
• Procedures for cremation
• Statutory notifications.

Promoting, monitoring and maintaining health of individuals and groups


should include being able to audit and appraise epidemiological data.

Keep accurate medical records should include information literacy.

Performing clinical and procedural skills safely should include:

• Demonstrating competence in basic and advanced life support skills and


basic trauma support
• Using local anaesthetics
• Giving intramuscular and subcutaneous injections
• Preparing and administering intravenous medicines
• Measuring and interpreting blood pressure
• Performing and interpreting ECG
• Performing and interpreting peak flow rate
• Performing venous and arterial puncture
• Gaining intravenous access including setting up an ivi
• Complying with infection control
• Disposing of potentially dangerous instruments
• Assessing suicide risk
• Assessment of altered mental state
• Performing a cervical smear
• Dealing with aggression
• Using an ophthalmoscope
• Giving pain relief
• Using a speculum and performing a vaginal examination
• Suturing a wound
• Urine testing
• Using an auriscope.

Patient safety and risk management should include promoting monitoring and
maintaining health and safety in the clinical setting and systems of quality assurance
including governance, audit, CPD, appraisal, revalidation. Dealing with complaints
remains a separate heading under relationships with patients.

Testing method
Many of these will be assessed as part of the PLAB test. Some skills are, however,
best assessed in the workplace and will form part of the appraisal process, which will
be used as a source of evidence for revalidation. Examples of these follow:

• Taking appropriate action if one’s own health, performance or conduct or that


of a colleague puts patients, colleagues or the public at risk

• Recognising personal and professional limits and asking for help from senior
colleagues and other health and social care professionals when necessary

• Developing a portfolio of evidence that records participation in educational


and clinical teaching sessions and allows one to identify learning needs and
develop a plan to meet these

• Contributing to audits and understanding how the results of audit can improve
one’s own practice and that of others

• Responding constructively to the outcome of appraisal and performance


review

• Contributing to the appraisal and review of students with whom one works

• Working with colleagues

• Teaching and training

• Probity

• Health

• Completing legal documents.
