Una conferencia de consenso multidisciplinaria del Acute Kidney Injury Network (AKIN) en
2005 desarroll otro sistema diagnostico y de clasificacin ms simple, que solo utiliza el
termino lesin renal aguda (Acute Kidney Injury AKI) con tres grados de severidad que
son parecidos a las primeras tres categoras del sistema RIFLE anterior(Mehta et al., 2007).
Teniendo en cuenta varios estudios que demostraron que elevaciones pequeas de creatinina
durante una hospitallizacin conllevan un riesgo elevado de morbi-mortalidad(Chertow et al.,
2005, Lassnigg et al., 2004, Loef et al., 2005, Coca et al., 2007), el primer grado de AKI incluye
una elevacin de creatinina srica por slo > 0.3 mg/dl (= 26.4 micromol/l). Este sistema de
definicin y graduacin es el ms reciente y ya ampliamente aceptado. Sin embargo, no
necesariamente va a ser permanente, sino puede ser cambiado o ampliado segn otras
evidencias epidemiolgicas y experimentales, por ejemplo sobre el uso de nuevas tcnicas
bioqumicas o radiolgicas de dao renal(Dagher et al., 2003, Dharnidharka et al., 2002, Adams
and Venkatesh, 2009).

Reduccin abrupta, en < 48 horas, de la funcin renal, definida por ahora (interim) como una
elevacin absoluta de la creatinina srica > 0.3 mg/dl (26.4 micrmol/l) o una elevacin
proporcional > 50% (1.5 veces sobre el nivel bsico) u oliguria < 0.5 ml/kg/h por > 6 h.
Estos criterios se deben usar en el contexto clnico adecuado y despus de reanimacin
con fluido si aplicable. Requieren por lo menos dos mediciones de creatinina srica e
incluyen cambios absolutos y proporcionales de ella para acomodar variaciones relacionadas a
edad, sexo y peso corporal. El criterio de la diuresis se incluy por su importancia predictiva,
pero teniendo en cuenta que la diuresis no siempre se mide fuera de las unidades de cuidados
intensivos. El diagnostico de AKI basado solo en diuresis requiere exclusin de
obstruccin del tracto urinario o cualquier otra causa fcilmente reversible de oliguria.

2005, Coca et al., 2007, Abelha et al., 2009, Bihorac et al., 2009). En el estudio de Chertow y
colaboradores(Chertow et al., 2005) se vio un aumento de > 0.5 mg/dl de creatinina srica en
13% de un total de mas de 9000 pacientes de varios servicios mdicos y quirrgicos de un
hospital en Boston, EEUU. Este mismo aumento de creatinina se asoci con una elevacin por
6.5 veces del riesgo de mortalidad, un tiempo de hospitaliazacin aumentado por un promedio
de 3.5 das y una elevacin del costo de unos $7500. Extrapolando esto al nmero total de
admisiones hospitalarias anuales en EEUU el costo atribuible a IRA adquirida en el hospital
sera de ms de $10 billones.
Estos ltimos estudios parecen comprobar que a pesar de que la relacin con el fallo
multiorgnico sea un factor muy importante para la mortalidad de pacientes con
IRA(Ostermann and Chang, 2002), tambin hay un componente de morbi-mortalidad
aumentada atribuible slo a la propia IRA, incluso a sus grados ms sutiles(Chertow et al.,
2005, Lassnigg et al., 2004, Loef et al., 2005, Coca et al., 2007, Payen et al., 2008). Los
pacientes no solo se mueren con sino de IRA y sus complicaciones asociadas metablicas e
infecciosas y tal vez tambin de la manera en que se trata (Brivet et al., 1996, de Mendonca et
al., 2000, Kellum and Angus, 2002, Levy et al., 1996, Liano et al., 1998, Metnitz et al., 2002,
Prough, 1996). En un estudio austriaco que comprar pares de pacientes similares en edad,
tipo y severidad de la enfermedad de base, pero distinguiendose por presencia u ausencia de
IRA control se observ una mortalidad de 63% en pacientes con y 39% en pacientes sin
IRA(Metnitz et al., 2002).
De todo esto se infiere la importancia de prevencin de IRA con estrategias como:
Reanimacin optimizada en el perioperatorio asi como en pacientes con trauma y
sepsis severa(Guly and Turney, 1990, Rivers et al., 2001, Brienza et al., 2010, Brienza
et al., 2009)
Posible empleo preventivo de fenoldopam y peptido natriuretico atrial (ANP) para
pacientes con alto riesgo de IRA, incluso en el perioperatorio, sobre todo en ciruga
cardiaca(Landoni et al., 2008, Landoni et al., 2007, Morelli et al., 2005, Nigwekar et al.,
2009b, Sward et al., 2004)
Fluidoterapia con suero salino(Mueller et al., 2002, Trivedi et al., 2003) o
bicarbonato de sodio 1.26% (Chertow, 2004, Merten et al., 2004, Hoste et al., 2010)
endovenosos y N-acetilcisteina(Kshirsagar et al., 2004, Aguiar-Souto et al., 2006,
Marenzi et al., 2006a, Kelly et al., 2008, Tepel et al., 2000, Recio-Mayoral et al., 2007)
antes del empleo de medios de contraste radiolgico en pacientes de alto riesgo.
En IRA establecida, a pesar de que los estudios epidemiolgicos siguen indicando una
mortalidad alrededor de 60% en casos severos (con dilisis o hemofiltracin) y pocos cambios
en las ltimas dcadas(Brivet et al., 1996, Kellum and Angus, 2002, Liano et al., 1998, Liano
and Pascual, 1996, Metnitz et al., 2002, Prough, 1996, Uchino et al., 2005, Vinsonneau et al.,
2006), existen algunas seales de que se puede lograr una mejora en el pronstico. En dos
revisiones de un centro de EEUU publicados en 1983 y 2002, aunque hubo un aumento de la
frecuencia de IRA en pacientes hospitalizados de 4.9% a 7.2%, se vio una reduccin en la
mortalidad de los casos mas severos (creatinina > 3 mg/dl) de 64% a 38% en los 20 aos
transcurridos(Hou et al., 1983, Nash et al., 2002). Una revisin de 1996 demostr una
mortalidad real de 49% vs. 60% pronosticada por SAPS II en 116 pacientes con IRA severa
tratados con hemofiltracin continua en varias UCIs del estado australiano de Victoria(Cole et
al., 2000). En algunos estudios pequeos de hemofiltracin y dilisis se han podido lograr tasas
de mortalidad a corto plazo relativamente bajas, sobre todo con dosis o frecuencia de apoyo
renal mas altas (Bouman et al., 2002, Nash et al., 2002, Ronco et al., 2000, Saudan et al.,
2006, Schiffl et al., 2002). Un trabajo de dos centros holandeses(Bouman et al., 2002),
comparando hemofiltracin temprana con volumen alto y tarda con volumen bajo en IRA
severa, demostr mortalidades hospitalarias de 37 y 39%. La mortalidad de todos los pacientes
hemofiltrados por IRA en los dos centros en el mismo perodo fue del 43%. Sin embargo, dos
grandes estudios multicentricos (con mas de 1000 pacientes en cada uno) recientes
reportaron mortalidades a 60 y 90 das entre 45 54%(Bellomo et al., 2009, Palevsky et
al., 2008b), SIN una reduccin con las dosis mas altas de hemofiltracin y dilisis.
Esta alta mortalidad tan persistente de la IRA demuestra que tratar de prevenirla sigue siendo
de mxima importancia y que se deben seguir investigando opciones de prevenir y tratarla
mejor para reducir la morbi-mortalidad y el costo asociado.

Otra consideracin bsica muy importante es que la creatinina en sangre no sube por
encima del lmite normal hasta que haya una reduccin de la filtracin glomerular del
50% o mas(Brenner and Brenner, 2001, Levey et al., 1999). Por todo eso, un nivel srico
de creatinina en el lmite superior de lo normal puede significar la prdida de funcin de
la mitad de las nefronas y en pacientes con niveles de creatinina normales altos ya hay
un nmero con disfuncin renal.

Diurticos
Los diurticos pueden causar hipovolemia y disminucin del gasto cardiaco con posible
hipoperfusin renal. Existe evidencia tanto en animales como en humanos que los diurticos
de asa como furosemida y osmticos como manitol pueden causar un deterioro de la funcin
renal en ciertas situaciones. Ademas, en la mayoria de los estudios y meta-analisis se ha
observado que los diurticos no reducen la incidencia, severidad o duracin de
IRA(Tenstad and Williamson, 1995, Mehta et al., 2002, Lameire et al., 2002, Lassnigg et
al., 2000, Weinstein et al., 1992, Solomon et al., 1994, Rello et al., 1989, Visweswaran et
al., 1997, Gubern et al., 1988, Ho and Sheridan, 2006)
Recientemente, se ha demostrado que los antagonistas de receptores A1 de adenosina, que
tambin tienen un efecto diurtico, pueden prevenir la reduccin de la filtracin glomerular
provocada por la furosemida en pacientes con insuficiencia cardaca(Gottlieb et al., 2000,
Gottlieb et al., 2002, Dittrich et al., 2007).

Aminoglicsidos
La nefrotoxicidad por aminoglicsidos(Cronin, 1979, Appel, 1990) se puede reducir con buena
hidratacin y administracin de aminoglicsidos guiada estrictamente por niveles plasmticos,
dando dosis altas nicas (en vez de tres diarias) para iniciar terapia y despus solo cuando el
nivel est suficientemente bajo(Levin, 1994). Tambin hay que evaluar la necesidad del
tratamiento de sepsis con aminoglicsidos en cada caso (el balance entre el riesgo de IRA por
la sepsis misma o el aminoglicsido), as como posibles alternativas menos nefrotxicas.
Varios meta-anlisis recientes sobre el tratamiento emprico de la sepsis severa con o sin
neutropenia no han demostrado beneficio pero un aumento significativo del riesgo de
complicaciones incluso la disfuncin renal con la combinacin de aminoglicsidos con betalactmicos comparada con monoterapia con beta-lactmicos(Paul et al., 2003b, Paul et al.,
2003a, Paul et al., 2004). Por eso el uso de aminoglicsidos ya no debera ser de rutina sino
limitado a casos especficos y guiado por la severidad y el sitio de la infeccin y la sensibilidad
microbiana.

y despus cada 12 horas, con 4 dosis totales, o 150 mg/kg en 30 minutos, seguido por 50
mg/kg en las cuatro horas siguientes) y la va endovenosa para angiografas de
emergencia, con resultados positivos (Briguori et al., 2004a, Briguori et al., 2004b, Marenzi et
al., 2006a, Baker et al., 2003). Marenzi y colaboradores compararon 0.6 g y 1.2 g de Nacetilcisteina (primera dosis endovenosa, seguida por tres dosis orales) con placebo en 354
pacientes con infartos agudos del miocardio para intervencin coronaria con contraste noionico de baja osmoloaridad (Iohexol = Ominpaque)(Marenzi et al., 2006a). Se vio un aumento
de creatinina por mas de 25% en 33% de los pacientes que recibieron placebo, 15% de los que
recibieron 0.6 g y 8% de los que recibieron 1.2 g de N-acetilcisteina (p < 0.001). La mortalidad
hospitalaria fue 11% en el grupo de placebo versus 4 y 3% en el grupo de 0.6 y 1.2 g
respectivamente. Existen algunos estudios negativos sobre N-acetilcisteina y nefropatia de
contraste en pacientes con insuficiencia renal crnica(Fung et al., 2004, Goldenberg et al.,
2004), y algunos expertos han discutido la verdadera importancia de la N-acetilcisteina, incluso
en el contexto de las cifras tan bajas de nefropata por contraste que se lograron en estudios de
mas tamao y calidad con suero salino y bicarbonato(Mueller et al., 2002, Merten et al., 2004,
Kshirsagar et al., 2004). Sin embargo, varios meta-analisis han demostrado un valor
beneficioso de la acetilcisteina sobre la nefropatia de contraste(Kelly et al., 2008, Birck et al.,
2003, Kellum, 2003).
Un estudio unicentrico de 111 pacientes sometidos a coronariografa de emergencia demostr
una reduccin de pacientes con un aumento de Creatinina > 0.5 mg/dl (= > 44mcmol/l) a 1.8%
versus 21.8% (p < 0.001) con una combinacin de bicarbonato de sodio y N-acetilcisteina
endovenosos comparado con suero salino por 12 horas(Recio-Mayoral et al., 2007)
En ciertas situaciones de estres renal, antagonistas de adenosina como aminofilina
pueden llevar a una dilatacin de la arteriola aferente y constriccin de la arteriola eferente,
junto con un efecto diurtico directo por inhibicin de absorcin tubular de sodio y
agua(Gottlieb, 2001, Gerkens et al., 1983, Bidani et al., 1987). Un efecto protector de la
aminofilina o teofilina (a dosis endovenosa de 165 a 250 mg(Kolonko et al., 1998, Huber et al.,
2006, Huber et al., 2002, Huber et al., 2001, Huber et al., 2003) o 5mg/kg por via oral (Erley et
al., 1994) antes de la administracin del contraste sobre la nefropata de contraste se ha visto
en algunos estudios(Erley et al., 1994, Huber et al., 2006, Huber et al., 2002, Huber et al.,
2001, Huber et al., 2003, Kapoor et al., 2002, Katholi et al., 1995, Kolonko et al., 1998, Ix et al.,
2004), pero no se ha comprobado en otros(Erley et al., 1999, Shammas et al., 2001). El efecto
protector de aminofilina parece menos cuando los pacientes se hidratan bien con cristaloide
isotnico(Erley, 1999, Erley et al., 1999). En la mas reciente meta-analisis se vio solo una
tendencia no significativa de la nefropatia por contraste con teofilina(Kelly et al., 2008)
No se han visto resultados beneficiosos sobre la nefropata por contraste con el uso de
diurticos como furosemida y manitol(Weinstein et al., 1992, Solomon et al., 1994, Dussol et
al., 2006) y de fenoldopam(Kelly et al., 2008, Stone et al., 2003, Briguori et al., 2004a).
Estudios de Marenzi y colaboradores(Marenzi et al., 2003a, Marenzi et al., 2006b, Marenzi et
al., 2003b) han demostrado que en insuficiencia renal crnica establecida (creatinina srica
> 2 g/l), que conlleva un riesgo mayor de deterioro por radiocontrastes, la hemofiltracin
continua a solo un litro por hora por 4 8 horas antes y 18 24 horas despus de la
administracin de medios de contraste ayud a preservar la funcin renal en el 95% (vs. 50%
en el grupo de control con administracin de suero salino), reducir la necesidad de dilisis y la
morbi-mortalidad. Los medios de contraste son filtrados por las membranas de hemofiltracin y
se diluyen por el lquido de reemplazo. Adems con el fluido de reemplazo se administra (o se
genera de lactato) bicarbonato que puede contribuir a una alcalinizacin de la orina. Por ende,
la hemofiltracin puede reducir la cantidad de radiocontraste y el dao renal por el.

Un estudio pequeo reciente demostr que permitir respiracin espontnea en ventilacin por
liberacin de la presin en las vas areas (Airway pressure release ventilation APRV = Presin
positiva en vas areas bifsica BIPAP) se asoci con mejor perfusin renal y filtracin
glomerular que ventilacin con el mismo volumen por minuto pero de manera controlada
(control de presin) sin respiracin espontnea(Hering et al., 2002). Esto comprueba evidencia
previa de una mejoria de la funcin renal con adicin de respiracin espontnea en ventilacin
mandataria intermitente sincronizada(Steinhoff et al., 1984).
Recientemente se ha demostrado que hay efectos de la ventilacin a presin positiva sobre la
funcin renal ms all de la perfuson renal disminuida y la retencin de sal y agua. En un
trabajo con animales el grupo de Slutsky observ que la ventilacin con volmenes tidales y
presiones pico altos y PEEP baja lleva a apoptosis y elevacin de marcadores de dao renal e
intestinal(Imai et al., 2003). Otros trabajos similares comprobaron una asociacin de la
ventilacin con altos volmenes y presiones con fallo multiorgnico(Slutsky and Tremblay,
1998, Ranieri et al., 1999). La reduccin del dao de otros organos que los pulmones mismos
puede haber tomado parte en la reduccin de la mortalidad en el estudio de ARDSnetwork con
volmenes corrientes y presiones pico bajos (< 6 ml/kg/min y < 30 cmH2O)(Brower et al., 2000).
Hipertensin abdominal
La presin abdominal elevada y el sndrome de compartimiento abdominal (presin abdominal
y disfuncin de rganos) se pueden asociar con insuficiencia cardiovascular y renal por
compresin de la vena cava, la aorta y los vasos renales(Prowle and Bellomo, 2010, Wauters
et al., 2009, Dalfino et al., 2008, Malbrain, 2004, Sugrue et al., 2004) (Sugrue et al., 1999, De
laet et al., 2007, Sugrue et al., 1995, Sugrue, 2005, Meldrum et al., 1997). Quirrgicamente se
puede prevenir con no cerrar el abdomen bajo tensin y colocar una cobertura temporal en
casos de trauma, hemorragia (retro)peritoneal o cualquier patologa asociada con alto volumen
del contenido abdominal (Sugrue et al., 1998, Balogh and Moore, 2004). Una vez establecido,
el sndrome de compartimiento abdominal se puede tratar con descompresin por laparotoma
o drenaje percutneo (Myers and Latenser, 2002, Latenser et al., 2002). El manejo mdico
incluye correccin de coagulopata y fluidoterapia ptima, no excesiva(Balogh et al., 2003,
Balogh and Moore, 2004) (Sandison et al., 1998), uso de relajantes musculares(De Waele et
al., 2003), reduccin de opiceos que pueden causar leo y aumentan la actividad espiratoria
de los msculos abdominales(Drummond and Duncan, 2002) o, aunque hasta ahora solo
experimental, la aplicacin de presin negativa externa al abdomen (NEXAP (Life Care - Nev
100, Respironics)(Bloomfield et al., 1999, Valenza et al., 2003). Adems, cuando hay riesgo o
presencia de hipertensin abdominal es importante no slo enfocar valores de gasto cardiacos
altos pero tambin la presin de perfusin renal (tensin arterial media menos presin
abdominal).
Rabdomiolisis y mioglobinuria
La IRA por rabdomiolisis involucra varios mecanismos: hipovolemia por secuestro de lquidos
en los msculos necrticos, vasoconstriccin renal, precipitacin de mioglobina y cido rico en
los tbulos renales y dao tubular por radicales libres de oxgeno formados por el hierro que se
libera de la mioglobina (los ltimos dos mecanismos se promueven por acidosis de la
orina)(Vanholder et al., 2000a, Vanholder et al., 2000b, Zager, 1996b, Zager, 1996a, Moore et
al., 1998). La prevencin y el manejo de IRA por rabdomiolisis incluyen: fluidoterapia agresiva
(hasta por encima de 10 litros diarios) con cristaloides, posiblemente con bicarbonato
adicional, p.ej. salino 0.45% y 75 -100 mmol/l bicarbonato de sodio(Vanholder et al., 2000b) o
hasta 200 - 400 mmol de bicarbonato por da(Druml, 2004) en adicin a otro cristaloide comenzando tan pronto que posible despus de la lesin muscular. Aadir bicarbonato y
alcalinizar la orina a un pH > 6.5 con la misma solucin endovenosa (y posiblemente la adicin
de una dosis de acetazolamida) deberia reducir la precipitacin de protenas alteradas por
descomposicin de mioglobina y la formacin de radicales libres de oxgeno(Moore et al.,
1998). Un efecto protector de la alkalinizacin de la orina se ha comprobado en experimentos
con animales(Block and Manning, 2002, Moore et al., 1998). En un estudio observacional de 20
pacientes con oliguria por mioglobinuria a pesar de fluidoterapia se vio una mejoria de la

funcin renal con la administracin de bicarbonato y manitol(Eneas et al., 1979). Sin embargo,
el numero y la calidad de estudios clnicos sobre fluidoterapia en mioglobinuria es limitado. Un
estudio observacional pequeo demuestra ninguna diferencia entre fluidoterapia con suero
salino solo o suero salino con adicin de bicarbonato y manitol(Homsi et al., 1997). El uso de
manitol como agente osmtico para reducir edema muscular, diurtico y depurador de radicales
libres de oxgeno se recomienda por algunos expertos para prevenir IRA en rabdomiolisis,
siempre y cuando sigue existiendo flujo urinario(Ron et al., 1984, Vanholder et al., 2000b). Sin
embargo, hay que tener en cuenta que el manitol a dosis alta puede inducir o empeorar dao
renal(Rello et al., 1989, Visweswaran et al., 1997). En unos pocos experimentos con animales
la aminofilina atenu el dao renal por mioglobinuria (Bidani and Churchill, 1983, Bidani et al.,
1987). Sin embargo, faltan datos de estudios humanos para comprobar un efecto de
aminofilina, adicional a buena fluidoterapia, para prevenir IRA en rabdomiolisis. La furosemida
acidifica la orina y aument la mortalidad por mioglobinuria en un estudio con animales(Bidani
et al., 1987). Algunas membranas de hemofiltracin continua dejan pasar 50% o ms de la
mioglobina (peso molecular 17 000 Dalton) en sangre. As la hemofiltracin elimina mioglobina
y la diluye con el lquido de sustitucin. Este efecto aumenta con ms volumen de ultrafiltrado y
sustitucin(Amyot et al., 1999, Timokhov et al., 1997, Nicolau et al., 1996, Winterberg et al.,
1990) y mucho mas an con membranas de hemofiltracin de flujo super alto y un tamao de
poros de 100.000 Dalton a tasas de ultrafiltracin y re-emplazo hasta 4 l/h. Con esto se pueden
remover hasta 5 gramos por da de mioglobina en comparacin a 1 g con una membrana
convencional con poros de unos 30.000 Dalton a 2 l/h(Naka et al., 2005). As se disminuye la
exposicin de los riones a la mioglobina y sus efectos nocivos. Sin embargo, sobre todo en
catstrofes con muchos casos de rabdomiolisis, como terremotos, la fluidoterapia temprana y
agresiva sigue siendo la manera ms importante para prevenir la IRA y la hemofiltracin a alto
volumen no es una solucin fcilmente practicable.

Coloides
La administracin de coloides, sobre todo en cantidades y concentraciones altas puede
inducir IRA (nefropata hiperonctica) por disminucin de la filtracin glomerular por un
aumento de la presin onctica de la sangre por encima de la presin hidrosttica
glomerular. Ademas, coloides, como otros solutos y medios de contraste, pueden asociarse
con cambios histopatolgicos en las celulas del tubulo proximal (nefrosis osmotica),
inducidas por endocitosis lisosomal intracelular de estas molculas que conlleva hinchazn de
las celulas y obstruccin tubular. Adems, en experimentos recientes se ha visto que los
almidones (mas los de peso molecular medio que bajo) pueden inducir infiltracin con
macrofagos y proliferacin intersticial en tan solo seis horas. (Bernard et al., 1996, Cittanova et
al., 1996, Legendre et al., 1993, Moran and Kapsner, 1987, Rozich and Paul, 1989, Salomon et
al., 2007, Huter et al., 2009, Schortgen and Brochard, 2009). El riesgo de nefropata por
administracin de un coloide se puede agravar por hipovolemia, deshidratacin y
administracin insuficiente de cristaloide y agua libre o por insuficiencia renal ya
existente(Dickenmann et al., 1998, Gosling et al., 2001, Boldt and Priebe, 2003). Hasta cierto
punto el riesgo de IRA asociado con coloides se relaciona tambin al tipo del coloide, al peso
molecular y a la concentracin de la solucin. La albmina, mientras no se administren
cantidades excesivas, parece asociarse con menor riesgo de dao renal y puede tener efectos
protectores para la funcin renal(Davidson, 2006, Finfer et al., 2004), aunque en pacientes
criticos se ha visto un riesgo elevado de IRA y mortalidad con albmina hiperoncotica en un
estudio observacional multicentrico reciente(Schortgen et al., 2008). Se han reportado casos y
series de IRA con todos los coloides sintticos (dextranos, gelatinas, almidones)(Davidson,
2006). Dos estudios franceses compararon gelatina de peso molecular bajo al 3% con almidn
hidroxietilado de peso molecular medio al 6% en donantes de rganos(Cittanova et al., 1996) y
en pacientes spticos(Schortgen et al., 2001) y observaron que la funcin renal estaba peor
con administracin del almidn. El estudio en pacientes spticos es algo controvertido por la
definicin de IRA usada, la cantidad de pacientes con disfuncin renal crnica previa desigual y
el hecho de que la causa de IRA puede haber sido no solo el almidn sino la administracin
reducida de cristaloide y agua(Gosling et al., 2001, Boldt, 2001). En una revisin sobre el efecto
renal de coloides sintticos se recomienda que los coloides siempre se administren con y no en
lugar de cristaloides y que los almidones se usen con cuidado en pacientes con disfuncin
renal crnica(Boldt and Priebe, 2003). En el estudio VISEP alemn se vio un exceso al doble
de insuficiencia renal aguda requiriendo hemodiafiltracin as como un aumento no significativo
de la mortalidad en 90 das en pacientes tratados con almidn de peso molecular medio
concentrado (Haes Steril 10%) en comparacin a los que solo recibieron cristaoloide
balanceado (Lctato de Ringer)(Brunkhorst et al., 2008). Eso puede haber sido relacionado al
peso molecular alto asi como a la alta concentracin (10%) del coloide usado.
Desafortunadamente, este estudio se condujo con un almidn de peso molecular medio a
concentracin tan alta, cuando ya era mucho mas comn el uso de almidones de peso
molecular bajo a una concentracin de 6%. Es posible, pero no comprobado que los nuevos

renales(Huter et al., 2009, Boldt et al., 2003, Blasco et al., 2008). Un estudio de 7000 pacientes
crticos en Australia comparando hidroxi-etil almidn 130/0.4 suspendido en suero salino con
suero salino sin coloide esta empezando este ao y va a proveer mas datos clnicos
importantes.
En un estudio observacional retroespectivo (SOAP = Sepsis occurrence in Acutely Ill Patients)
no se observaron cambios negativos en la diuresis, el nivel de Creatinina, SOFA score renal y
necesidad de hemofiltracin en 1075 pacientes crticos de 198 UCIs europeos que recibieron
un promedio de 500 - 1000 ml de almidones sintticos (sin especificacin del peso molecular
medio) en comparacin a otros tipos de coloide y cristaloides solo(Sakr et al., 2007). El estudio
multicntrico australiano SAFE (Saline versus Albumin Fluid Evaluation)(Finfer et al.,
2004), en que se compar suero salino al 0.9% con albmina 4% suspendida en suero salino al
0.9% para el tratamiento de hipovolemia en casi 7000 pacientes, no se vio ninguna diferencia
en la morbi-mortalidad entre los dos grupos, incluso en el numero de pacientes que requirieron
hemofiltracin y la duracin de ella. Es importante mencionar que ambos grupos recibieron
cantidades generosas (1500 2500 ml/da) de otros lquidos (mantenimiento, nutricin) durante
el periodo del estudio y que alrededor del 50% de los pacientes incluidos tenan un puntaje bajo
en APACHE II, y posiblemente siguieron no muy graves en UCI. Estos ltimos estudios
parecen demostrar que en una mayoria de pacientes crticos mixtos, mientras se administran
cantidades suficientes de cristaloide y cantidades no excesivas de coloide, tal vez no importa
tanto el tipo de lquido usado para tratar hipovolemia y no se puede distinguir un efecto nocivo
sobre la funcin renal de los coloides. Esto no necesariamente se puede extrapolar a grupos de
pacientes en situaciones de alto riesgo de IRA. Tambin quedan abiertas cuestiones sobre
diferencias entre nuevas soluciones de coloides suspendidas en soluciones de electrolitos
MR
balanceados como Volulyte en comparacin con la suspensin convencional en NaCl 0.9%.

Vasopresina
Mientras la vasopresina es sinnimo a hormona antidiurtica, a dosis baja (0.02 0.04 U/min)
se ha demostrado un efecto diurtico en varios estudios pequeos en sndrome hepatorenal,
shock sptico y cardiognico(Holmes et al., 2001a, Holmes et al., 2001b, Patel et al., 2002,
Eisenman et al., 1999). Esto se puede explicar por un aumento de la presin de perfusin renal
con vasoconstriccin mas pronunciada de la arteriola glomerular eferente que aumenta la
presin de filtracin glomerular, pero tambin por vasodilatacin en la medula renal, mediada
por oxido ntrico(Rudichenko and Beierwaltes, 1995). Se ha establecido que el uso de anlogos
sintticos de vasopresina (ornipresina y terlipresina) en el tratamiento del sndrome hepatorenal
logra mejor diuresis y funcin renal(Lenz et al., 1989, Lenz et al., 1991, Hadengue et al., 1998,
Cardenas and Arroyo, 2003, Guevara et al., 1998). Hay evidencia observacional y de un
estudio randomizado que esto podra ser el caso con el uso de infusin de vasopresina en
shock, sobre todo sptico (Holmes et al., 2001b, Patel et al., 2002, Eisenman et al., 1999). En
un analisis post-hoc del estudio VASST(Russell et al., 2008, Gordon et al., 2010) evalu la
adicin de vasopresina a norepinefrina en shock sptico, se vi lesin renal aguda (AKI) de
todos sus grados en los 60% de 778 pacientes incluidos. En 106 pacientes con riesgo de IRA
segn la clasificacin RIFLE, vasopresina con norepinefrina se asoci con menos progresin a
IRA establecida (20.8 vs. 39.6% p = 0.03) y menos uso de hemodiafiltracin (17 vs 37.7% p =
0.02) que norepinefrina solaEl autor de este subanalisis, Anthony Gordon, est planificando un
estudio multicentrico (VANISH) comparando norepinefrina y vasopresina como primer
vasopresor en shock sptico y evaluando la incidencia de IRA (comunicacin personal).
Fenoldopam
Fenoldopam, un agonista sinttico especfico de los receptores de dopamina DA 1 que reduce
la presin arterial sistmica y vasodilata la mdula renal, ha tenido resultados algo
contradictorios en estudios experimentales y humanos(Shorten, 2001). Un estudio multicntrico
sobre fenoldopam (0.05 - 0.1 mcg/kg/min) no demostr ningn beneficio de su uso a corto
plazo (13 horas) en pacientes con insuficiencia renal crnica (aclaramiento de creatinina < 60
ml/min, 49% diabeticos) para prevenir un deterioro en su funcin renal por medios de contraste
radiolgico(Stone et al., 2003). Sin embargo, un estudio italiano unicentrico de 300 pacientes
crticos spticos con funcin renal normal (Morelli et al., 2005) demostr una reduccin
significativa a 19 versus 34% (p 0.006) de pacientes que desarrollaron un aumento de

creatinina a > 150 mcmol/l (1.7 mg/dl) o por mas de 80 mcmol/l (= 0.9 mg/dl) con
fenoldopam a 0.09 mcg/kg/min administrado por un promedio de 10 das. Adems, se vio
una reduccn significativa de das en UCI de los sobrevivientes y tendencias no
significativas a una reduccin de IRA severa (Creatinina srica > 300 mcmol/l = 3.4 mg/dl)
y de la mortalidad (35% versus 44% con placebo). Es importante destacar que en este
estudio con la dosis de 0.09 mcg/kg/min no se vio una reduccin de la tensin arterial, solo un
aumento no significativo de pacientes requiriendo norepinefrina. Esto es importante teniendo en
cuenta que fenoldopam es un vasodilatador y antihipertensivo(Murphy et al., 2001) y que la
hipotensin arterial es un factor importante en el desarrollo de la IRA(Liu et al., 2009), sobre
todo en pacientes con autorregulacin renal alterada(Palmer, 2002). Dos revisiones
sstematicas recientes por el mismo grupo de autores italianos de 13 estudios
randomizados en ciruga cardiovascular y 16 en pacientes crticos incluyendo mas de
1000 pacientes han reportado una reduccin significativa del riesgo de IRA, la necesidad
de apoyo renal extracorporeo, la mortalidad hospitalaria y los das en UCI con
fenoldopam(Landoni et al., 2008, Landoni et al., 2007). Se tendra que examinar el potencial
rol de fenoldopam en profilaxis de IRA en estudios multicentricos con grandes numeros de
pacientes con alto riesgo de IRA.

trabajo con riones de ratas aislados se observ que con nitroprusiato sdico, un donante de
ON, despus de isquemia renal se preserv la filtracin glomerular y la absorcin tubular de
sodio(Linas et al., 1997). Sin embargo, en dos estudios recientes slo se vio un efecto protector
renal cuando se administr un donante de ON antes de una lesin renal aguda experimental,
mientras que administracin despus pudo aumentar el dao renal (Nakajima et al., 2006a,
Nakajima et al., 2006b). Faltan todavia datos de estudios humanos clinicos.
Administracin de arginina, el substrato para la formacin del ON, mejor la perfusin renal en
modelos de IRA en animales y en recipientes de transplantes renales(Schramm et al., 1994,
Schramm et al., 2002, Schneider et al., 2003, Schramm et al., 2005).
Los efectos de N-acetilcsteina incluyen un aumento de la sntesis y duracin del efecto del
ON. Adems, como la N-acetilcistena se une con el ON hay menos substrato para la formacin
de peroxinitrito, un radical libre nocivo formado de ON y oxgeno(Safirstein et al., 2000). Como
ya mencionado, la N-acetilcsteina ha demostrado un efecto preventivo en mltiples estudios
sobre nefropata por radiocontraste(Marenzi et al., 2006a, Tepel et al., 2000, Birck et al., 2003,
Safirstein et al., 2000, Kelly et al., 2008, Efrati et al., 2003, Drager et al., 2004, Kay et al., 2003,
Alonso et al., 2004, Briguori et al., 2004a, Briguori et al., 2004b, Baker et al., 2003), en un
modelo animal de isquemia renal(de Araujo et al., 2005) y en un pequeo estudio sobre
sndrome hepatorenal(Holt et al., 1999).
El balance de ON constitutivo e inducido tambin est involucrado en ciertos efectos
protectores de las statinas en modelos de IRA isquemica en animales. En estos estudios se
vio un aumento del ON endotelial constitutivo y una reduccin de la sintasa de xido ntrico
inducible, y tambin una reduccin en la formacin de radicales libres de oxgeno(Sabbatini et
al., 2004, Gueler et al., 2002, Pisani et al., 2002).
Un estudio observacional en mas de 10.000 pacientes sometidos a cirugia cardiaca no se vio
un efecto protector de tratamiento preoperatorio cronico con statinas sobre la incidencia de IRA
(12%) y en la necesidad de apoyo renal extracorporeo (1.6 en con, 2% sin statinas) en el
postoperatorio(Argalious et al., 2010)
Sera interesante ver mas datos clnicos y prospectivos en humanos sobre un potencial papel
de donantes de oxido ntrico como nitratos, N-acetilcsteina y statinas en la prevencin y el
tratamiento de IRA en el futuro.
Diurticos de asa
Como mencionado antes, los diurticos de asa pueden disminuir la funcin renal en ciertas
situaciones(Gottlieb et al., 2002, Lameire et al., 2002, Lassnigg et al., 2000, Mehta et al., 2002,
Solomon et al., 1994, Tenstad and Williamson, 1995, Weinstein et al., 1992). No hay valor
clinico del uso de diurticos de asa para prevenir de IRA, sobre todo cuando se utilizan en
pacientes no suficientemente reanimados con fluidos y/o inotropos y/o vasopresores. Existe por
lo menos un estudio y un meta-anlisis que demuestran un posible aumento de la mortalidad
con el uso de diurticos de asa(Mahesh et al., 2008, Ho and Sheridan, 2006, Mehta et al.,
2002). Sin embargo, el uso de diurticos en pacientes crticos con insuficiencia renal sigue
siendo comn. En el estudio australiano sobre dopamina mas de la mitad de los pacientes en
ambos grupos recibieron furosemida en una dosis total de aproximadamente 200 mg (Bellomo
et al., 2000) y en un estudio epidemiolgico mundial reciente de ms de 1000 pacientes crticos
con IRA se utilizaron diureticos en el 70%(Uchino et al., 2004, Uchino et al., 2005) .
Cuando todava hay perfusin renal y diuresis, las potenciales ventajas de los diurticos de asa
en IRA consisten en
1) Reduccin de sobrecarga de lquidos y de hiperkalemia. La falta de beneficio para los
riones y potencial dao de sobrehidratacin para mltiples rganos se ha observado
en algunos estudios clnicos recientes (Wiedemann et al., 2006, Prowle and Bellomo,
2010, Minambres et al., 2010, Bouchard et al., 2009)
2) Potencial conversin de IRA oligrica a polirica, que puede facilitar el manejo y limitar
el aumento de los niveles altos de potasio, urea y creatinina antes de la resolucin de
la IRA, posiblemente obviando o minimizando la necesidad de dilisis o
hemofiltracin(Sampath et al., 2002). Sin embargo, esto probablemente indica un
epifenmeno de casos menos severos de IRA respondiendo a tratamiento general y
diurtico y no un efecto beneficioso genuino de los diurticos.
3) Reduccin del transporte de cloruro y sodio (con alto consumo de ATP y oxgeno) en la
parte ascendente gruesa del asa de Henle, reduciendo la demanda de oxgeno cuando

puede ser limitada la entrega de oxgeno en la parte del tbulo renal ms vulnerable a
isquemia(Brezis and Rosen, 1995, Brezis et al., 1994, Brezis et al., 1984).
Hay evidencia que el efecto de la furosemida se logra con dosis mas pequeas y es ms
consistente cuando se administra en infusin continua (en dosis tan bajas como 1 - 4 mg/h) en
vez de bolos (Martin and Danziger, 1994, Ostermann et al., 2007). Aunque un numero de
estudios y revisiones ha demostrado que el uso de diurticos de asa en dosis alta no previene
ni mejora el curso de IRA(Ho and Sheridan, 2006, Kellum, 1997, Lameire et al., 2002, Lassnigg
et al., 2000, Mehta et al., 2002, Gambaro et al., 2002), otros demuestran que los diurticos no
empeoran el curso o pronstico(Noble, 2004, Shilliday et al., 1997, Uchino et al., 2004,
Sampath et al., 2002). Una revisin pequea de estudios clnicos sobre diurticos de asa en
IRA demostr un tiempo ms corto hasta recuperacin de una diuresis por encima de 1.5 litros
por da y un numero menor de sesiones de dilisis en pacientes tratados con diurticos de asa
(con implicaciones obvias en el uso de recursos y costo) sin diferencia en la
mortalidad(Sampath et al., 2002).
Un estudio americano multicntrico retroespectivo(Mehta et al., 2002) demostr un aumento de
la mortalidad y menos recuperacin de la funcin renal en pacientes crticos con IRA que
recibieron grandes dosis de diurticos de asa. Es importante destacar que en el grupo de
diurtico haba mas pacientes de edad avanzada y con insuficiencia cardiaca, y una tendencia
de poca respuesta diurtica a grandes dosis de furosemida. Sin embargo, en pacientes que
respondieron con buena diuresis a dosis bajas de diurtico (lo cual posiblemente implica una
forma menos severa de IRA) no se vieron efectos nocivos sobre la mortalidad y recuperacin
renal. Por eso, una de las conclusiones del estudio es que un ndice de respuesta a
furosemida, i.e. la dosis de furosemida dividida por la diuresis diaria, podra tener valor
pronostico en IRA y ayudar en elegir adecuadamente entre diurticos o hemodiafiltracin. Se
podra alegar que este estudio demuestra ms sobre el mal uso de diurticos por intensivistas y
nefrlogos que sobre los diurticos mismos, teniendo en cuenta que en algunos pacientes se
siguieron usando grandes dosis de diurticos a pesar de que permanecieron severamente
oligricos por varios das.
Lo que no se ha controlado suficientemente en los estudios y discutido en la controversia
sobre diurticos en IRA es el resto del tratamiento, como mantenimiento de la volemia y de la
presin de perfusin antes y durante el potencial uso de diurticos. La cuestin tal vez no es
tanto s o no usar diurticos sino como usarlos apropiadamente. La inclusin de diurticos en
infusin a dosis baja (furosemida 1 4 mg/h) en una estrategia teraputica que provee
volumen, gasto cardiaco y presin arterial ptimos as como sustitucin adecuada de la prdida
de lquido y electrolitos por la poliuria, es una estrategia usada empricamente para el manejo
de IRA leve o moderada en pacientes que responden con diuresis efectiva a esas
medidas(Bullingham and Palazzo, 1994) pero no ha sido sometida a estudios controlados de
suficiente tamao y calidad.
Antagonistas de adenosina
La adenosina es un mediador fisiolgico de la retroalimentacin tbuloglomerular liberado por
altas concentraciones de solutos en los tubulos renales. Niveles excesivos de adenosina son
producidos en hipoxia e isquemia por la descomposicin e insuficiente re-sntesis de ATP. La
adenosina causa constriccin de la arteriola aferente y dilatacin de la arteriola eferente y los
vasos rectos de la mdula renal. En ciertas situaciones, como hipoxia-isquemia o sobrecarga
tubular por solutos o sustancias (posiblememente incluyendo medios de contraste), esos
efectos de la adenosina normalmente protectores para los tbulos renales pueden llevar a una
disminucin severa o un cese de la filtracin glomerular por constriccin de la arteriola aferente.
En ciertas situaciones de estres renal, antagonistas de adenosina como aminofilina o teofilina
pueden llevar a una dilatacin de la arteriola aferente y constriccin de la arteriola eferente, e
as mantener o aumentar la filtracin glomerular. (Gottlieb, 2001, Gerkens et al., 1983, Bidani et
al., 1987). La adicin de dosis bajas de aminofilina u otros antagonistas especficos de los
receptores A1 de adenosina a diurticos de asa en situaciones de alto riesgo como
insuficiencia cardiaca severa ayuda a mantener la filtracin glomerular, que tiende a disminuir
con el uso de diurticos solos(Gottlieb et al., 2002, Gottlieb et al., 2000, Dittrich et al., 2007). El
antagonismo de adenosina se puede obtener con dosis de perfusin muy bajas de aminofilina,
por ejemplo 15 mg/h en adultos o 1 8 mg/kg de teofilina cmo dosis nica en neonatos(Huet
et al., 1995, Jenik et al., 2000). Adems, la aminofilina tiene un efecto diurtico propio(Brater et

al., 1983, Parker and Willatts, 2001, Suzuki et al., 1992, Ng et al., 2005). En muchos
experimentos animales y unos pocos estudios humanos se ha observado un efecto protector
de los antagonistas de adenosina sobre la funcin renal, manteniendo la filtracin glomerular y
diuresis despus de hipoxemia, por ejemplo asfixia y distres respiratorio neonatal(Huet et al.,
1995, Jenik et al., 2000), isquemia(Lin et al., 1986, Lin et al., 1988), hipotensin(Barrett and
Wright, 1994), radiocontraste(Erley et al., 1994, Huber et al., 2006, Huber et al., 2002, Huber et
al., 2001, Huber et al., 2003, Kapoor et al., 2002, Katholi et al., 1995, Kolonko et al., 1998, Yao
et al., 2001, Yao et al., 2000a) y en varios otros tipos de IRA(Bidani and Churchill, 1983, Bidani
et al., 1987, Prada et al., 1986, Yao et al., 2000b, Yao et al., 1994a, Yao et al., 1994b).
Desafortunadamente no existen suficientes estudios clnicos controlados ni sobre la dosis ms
eficaz de aminofilina para una potencial proteccin renal ni sobre su valor en pacientes crticos
a riesgo de o con IRA, tal vez en combinacin con reanimacin ptima y diurticos a dosis baja.
Por esto no se puede saber si los antagonistas de adenosina prestan un beneficio propio
genuino o si los pacientes que responden a ellos como a los diurticos de asa simplemente
tienen IRA menos severa (y que si mejoraran igual solo con buena reanimacin sin
farmacoterapia adicional, como en el estudio australiano sobre dopamina).
Dilisis y hemofiltracin
Las indicaciones para tratamiento depurador extracorpreo en IRA son:
Oligo-anuria refractaria
Sobrecarga de lquidos y edema pulmonar
Hiperpotasemia severa
Acidosis metablica severa
Uremia severa y complicaciones urmicas como encefalopata, hemorragia por
disfuncin plaquetaria, derrames pericardiales y pleurales
En pacientes crticos y catablicos, sin embargo, es mas importante responder a la dinmica de
la IRA y no necesariamente siempre esperar hasta que se llegue a estos puntos extremos,
sobre todo cuando no se ve respuesta suficiente a la terapia con fluidos, inotropos,
vasopresores y, posiblemente, diurticos en un tiempo adecuado. Tambin hay que tener en
cuenta que IRA y uremia se asocian con efectos inmunosupresores y ms complicaciones
spticas. Sin embargo, tambin hay algunos pacientes que aunque cumplan con uno u otro de
estos criterios temporneamente establecen diuresis y mejoran con el tratamiento y no se
tienen que hemofiltrar o dializar.
Los metodos principales de terapia depuradora para IRA severa son dilisis y hemofiltracin,
que se pueden combinar en hemodiafiltracin veno-venosa continua(Forni and Hilton,
1997, John and Eckardt, 2007, Murray and Hall, 2000). La diferencia entre estas tcnicas
consiste en el proceso fsico principal de depuracin: difusin en dilisis y conveccin en
hemofiltracin. La dilisis puede ser por va sangunea o peritoneal, y se usa sobre todo de
modo intermitente (con excepcin en hemodiafiltracin continua), mientras la hemofiltracin
requiere un tratamiento ms continuo para proveer aclaramiento suficiente. Ambas tcnicas
depuran la sangre por va de una membrana semipermeable natural (dilisis peritoneal) o
artificial (hemodilisis/filtracion). Las membranas artificiales para hemofiltracin y dilisis son
ms o menos biocompatibles con referencia a la activacin del sistema de complemento e
inflamatorio y de leukocitos. En la mayora de las mquinas modernas de hemo(dia)filtracin
continua en UCI se usan membranas sintticas biocompatibles d poliacrilonitrilo y polisulfona.
Sin embargo, una revisin sstematica reciente en la base de datos de Cochrane no encontr
una ventaja de las membranas biocompatibles(Alonso et al., 2008)
Cuestiones importantes en el rea de apoyo renal extracorporal para pacientes crticos
incluyen
Uso de dilisis o hemofiltracin
Terapia continua o intermitente
Uso de lactato o bicarbonato en el liquido de dilisis y reposicin
Dosis de hemofiltracin y/o dilisis

citrato tambin sirviendo como fuente de bicarbonato)

Dilisis
La dilisis principalmente aclara solubles de peso molecular bajo (hasta 500 Da,
electrolitos, urea y creatinina) por difusin por una membrana semipermeable siguiendo
gradientes de concentracin entre la sangre y un liquido de dilisis al otro lado de la
membrana(John and Eckardt, 2007, Murray and Hall, 2000). La eliminacin de agua ocurre por
un grado de conveccin adicional y osmosis. Los factores que influyen el aclaramiento de agua
y solubles en la dilisis son: la superficie y el tamao de poros de la membrana, la
concentracin de solubles en el lquido de dilisis, y el flujo de sangre y lquido de
dilisis.
Hemodilisis (HD)
La hemodilisis puede ser intermitente o continua (hemodilisis o hemodiafiltracion venovenosa continua). La tcnica intermitente, mtodo convencional para el manejo de insuficiencia
renal crnica, tambin se aplica en muchos centros del mundo para pacientes con IRA.
Aplicndose flujos de sangre y fluido de dilisis altos, es un mtodo muy eficaz para la
eliminacin de agua y solubles. Su eficacia en poco tiempo tiene la ventaja de que los
pacientes no se tienen que anticoagular permanentemente y se pueden movilizar fuera de las
sesiones de HD. Las desventajas de la tcnica intermitente con sesiones de HD no muy
frecuentes (por ejemplo cada segundo da) consisten en la eliminacin brusca y extensiva de
agua y electrolitos, que puede llevar a hipovolemia, arritmias y cada del gasto cardiaco y de la
tensin arterial(Bellomo and Claudio, 1999). Este deterioro cardiovascular puede causar
nuevos insultos renales, que pueden impedir la recuperacin renal. Existen datos que con HD
intermitente convencional resultan reas isquemicas de varias edades en los riones, y que la
recuperacin de la funcin renal es menor y ms pacientes tienen que seguir con dilisis
permanente despus de su episodio de IRA tratado con HD intermitente convencional(Conger,
1990, Manns et al., 1997). En los dos estudios grandes recientes sobre dosis de apoyo renal se
vio una diferencia interesante en la duracin de insuficiencia severa y apoyo renal. En el
estudio estadiounidense en que se emple dilisis intermitente cuando los pacientes eran
hemodinamicamente estables mas del 70% de los pacientes no habian recuperado funcin
renal y seguian en apoyo renal extracorporeo en el da 28(Palevsky et al., 2008a). En el estudio
australiano, en que slo se us hemodiafiltracin continua, en el da 28 slo 12 14% no
habian recuperado y seguan dependientes de la depuracin extracorporea. Es posible, pero no
comprobado, que esta diferencia marcada por parte sea asociada a la manera intermitente o
continua de apoyo renal.
Otra complicacin de la HD intermitente es el sndrome de desequilibrio por eliminacin rpida
de urea y otros solutos de la sangre con movimientos osmticos de agua consecuentes. Esto
puede llevar a un movimiento osmtico de agua al liquido cefalorraqudeo (del cual la urea no
se elimina tan rpidamente que de la sangre) y al intersticio y las clulas cerebrales, con el

riesgo de deterioro significativo en pacientes con presin intracraneal elevada por lesiones
cerebrales o insuficiencia heptica severa. En estos casos, la HD intermitente en sesiones
cortas e intensas esta contraindicada y son preferibles tcnicas continuas de aporte renal.
Otros factores que pueden influir en el cuidado de pacientes crticos con el uso de HD
intermitente es que por lo general hay necesidad de maquinas de hemodilisis y acceso a agua
depurada como de enfermeras especializadas en hemodilisis. Eso puede implicar el riesgo de
transporte intrahospitalario de pacientes crticos a la unidad de dilisis. Este problema se puede
superar con nuevos sistemas integrales de dilisis con una unidad para preparar lquidos de
dilisis esteriles con que se llenan mquinas de dilisis mviles, que se pueden utilizar
flexiblemente para dilisis, posiblemente extendida en tiempo, en la UCI, como para HD
MR
convencional (por ejemplo sistema GENIUS de la empresa FRESENIUS)(Kielstein et al.,
2006).
Es probable que las desventajas de la HD convencional descritas explican que un meta-anlisis
de estudios comparando tcnicas intermitentes y continuas de apoyo renal encontr una
mortalidad reducida con las tcnicas continuas cuando se corrigieron los datos por factores
como cualidad de los estudios primarios y severidad de la enfermedad crtica (no haba
diferencia en la mortalidad analizando todos los estudios sin estos ajustes)(Kellum et al., 2002).
Sin embargo, un estudio multicntrico francs no pudo comprobar esta ventaja de la
hemofiltracin continua, sino demostr una mortalidad a 60 das igualmente alta de 67 y 68% y
ninguna diferencia en morbilidad o das en UCI en pacientes crticos multimorbidos con IRA
comparando hemofiltracin a dosis relativamente baja (29 ml/kg/h) con dilisis cada segundo
da(Vinsonneau et al., 2006).
En un estudio unicentrico aleman se demostr que con hemodilisis intermitente
convencional diaria se pudo lograr una mortalidad 14 das despus de la ultima dilisis de
solo 28% comparada con 46% con dilisis en das alternos(Schiffl et al., 2002). En este estudio
tambin se vio una duracin ms corta de la IRA, as como menos sangrados
gastrointestinales, insuficiencia respiratoria, sepsis y sndrome de respuesta inflamatoria
sistmica con HD diaria. En el debate sobre este estudio se ha criticado que la dosis de
depuracin en el grupo de dilisis en das alternos fue demasiado baja(Bonventre, 2008,
Palevsky et al., 2008a). Adems fue un estudio de solo 160 pacientes en un solo centro y slo
analiz morbi-mortalidad a muy corto plazo. Dosis mas altas de dilisis tambin demostraron un
efecto favorable sobre la mortalidad de pacientes con IRA con riesgo intermedio en un estudio
observacional anterior(Paganini et al., 1996).
Otra nueva tcnica dialtica prometedora que produce dosis ms alta de depuracin con mayor
estabilidad cardiovascular es la dilisis lenta a eficacia baja (SLEDD = Sustained low efficiency
daily dilisis)(Kielstein et al., 2006, Marshall et al., 2004) . Esta tcnica vuelve a la dilisis
convencional pero de tiempo extendido, por ejemplo 12 horas cada noche, con ms
aclaramiento de solutos y agua de una manera ms gradual y por ende con mas estabilidad
hemodinmica y electroltica que HD intermitente en sesiones cortas.

necesaria para establecer un balance negativo de lquidos) con una solucin balanceada de
electrolitos. Las desventajas principales de este sistema arteriovenoso espontneo fueron el
riesgo de hemorragia por la puncin arterial y la dependencia de la presin arterial del paciente
y por ende poca eficacia (aclaramiento de creatinina solo 10 15 ml/min). Se han superado
con el desarrollo de la hemofiltracin continua veno-venosa por catteres de doble luz en que
se bombea la sangre as como el ultrafiltrado. El gradiente transmembranal para la
ultrafiltracin depende de los flujos generados por estas bombas. Se bombea tambin el
liquido de reposicin, una solucin balanceada de electrolitos con bicarbonato o lactato
(que se convierte en bicarbonato en el hgado), que tambin se puede utilizar para
dilisis adicional en hemodiafiltracin. El lactato es ms estable en solucin que el
bicarbonato, pero recientemente se han comercializado soluciones sin lactato y con
bicarbonato en botellas o compartimientos que se aaden poco antes, que suelen ser un poco
mas costosos que las soluciones con lactato. Reposicin con lactato puede llevar a acidosis
metablica en pacientes con insuficiencia heptica que no pueden convertir el lactato en
bicarbonato. Algunos estudios pequeos demuestran mejor correccin de acidosis y mayor
estabilidad hemodinmica y tensin arterial en pacientes con fallo multiorgnico con el uso de
bicarbonato en vez de lactato en el lquido de reposicin(Davenport, 1999, McLean et al.,
2000).
Dosis de hemo(dia)filtracin
Para sustancias que pasan completamente por la membrana semipermeable (coeficiente de
cedazo = 1, concentracin de la sustancia en el ultrafiltrado igual a la en sangre) como urea y
creatinina, en hemofiltracin el aclaramiento equivale a la cantidad de ultrafiltrado por minuto.
Esto significa que con un ultrafiltrado de uno o dos litros por hora, dosis convencionales
para hemofiltracin, se logra un aclaramiento de 16 a 32 ml/min, y para lograr un nivel de
los riones normales de unos 100 ml/min, se requirira una tasa de ultrafiltracin de seis
litros por hora.
Solo el 20 25 % de la sangre que pasa a travs del hemofiltro se debe ultrafiltrar para
evitar hemoconcentracin excesiva con bloqueo y coagulacin del filtro. Por esto para
lograr un ultrafiltrado convencional de 1 - 2 l/ h (= 16 33 ml/min) el flujo sanguneo debe
ser de 80 a 150 ml/min, y para hemofiltracin a volumen alto con un ultrafiltrado de 6l/h
se debera establecer un flujo sanguneo de 300 a 400 ml/min. Esto requiere acceso venoso
de luz larga, bombas capaces de producir este flujo y filtros de alta superficie, as como
calentamiento de la cantidad igualmente vasta de lquido de reposicin para evitar hipotermia.
(La hipotermia se ha asociado con morbi-mortalidad mayor en un estudio de animales spticos
tratados con hemofiltracin a un volumen muy alto de 100 ml/kg/h(Rogiers et al., 2006)).
Ademas del aumento del filtrado y reposicin por hora, el aclaramiento de molculas
pequeas se puede aumentar con la adicin de dilisis en hemodiafiltracin
continua(Bellomo and Claudio, 1999, Forni and Hilton, 1997, John and Eckardt, 2007, Murray
and Hall, 2000). Una cantidad de uno a dos litros por hora del mismo lquido que se usa
para reposicin es bombeada en contracorriente al flujo sanguneo por el lado de
ultrafiltrado del hemofiltro y depura la sangre de solutos de peso molecular bajo por difusin.
As se aumenta la cantidad del flujo sanguneo que se depura pasando por el filtro de solo 20
25% (para evitar hemoconcentracin) en la hemofiltracin. El 75 80% de la sangre que no se
ultrafiltra se depura por dilisis. Para molculas eliminadas muy eficazmente por dilisis, como
urea y creatinina, la adicin de un flujo de 1 - 2l/h de lquido de dilisis aumenta el aclaramiento
por 16 - 32 ml/min. Aandido esto a hemofiltracin a 1 - 2 l/h se logra un aclaramiento entre 50
- 66 ml/min (3000 4000 ml/h de hemofiltracon y dilisis divididos por 60 minutos).
La dilisis no elimina bien las molculas de peso molecular medio, como algunas citokinas, y el
flujo del lquido de dilisis tericamente puede oponerse a la eliminacin por conveccin y
adsorcin de tales molculas a la membrana del hemofiltro. Los estudios sobre depuracin
extracorprea en el manejo del shock sptico, implicando eliminacin de endotoxina y citokinas
por filtracin, absorcin y dilucin, han utilizado hemofiltracin a alto volumen sin dilisis (Cole
et al., 2001, Honore et al., 2000, Joannes-Boyau et al., 2004, Piccinni et al., 2006). Todava no
se ha comprobado en estudios multicntricos de suficiente tamao si la hemofiltracin a alto
volumen puede reducir la morbimortalidad del shock sptico. El estudio IVOIRE
(http://clinicaltrials.gov/ct2/show/NCT00241228?term=IVOIRE&rank=1), en curso en dos

centros europeos, compara hemofiltracin de 35 ml/kg/h con 70 ml/kg/h por 96 horas en 500
pacientes con shock sptico y lesin renal aguda. Se analizar la mortalidad en 28, 60 y 90
das, la duracin de soporte renal extracorpreo y de ventilacin mecnica as como la
hemodinmica y la dosis de vasopresores (comunicacin en el simposio de Medicina Intensiva
en Bruselas, Marzo 2007 de Dr Patrick Honor).
Cuando se emplean altas dosis de hemofiltracin hay que tener en cuenta una prdida
significativa de electrolitos, aminocidos y de algunos frmacos por la hemo(dia)filtracin y
ajustar las cantidades administradas de tales sustancias y frmacos as como mantener
estabilidad hemodinmica y normotermia(Bellomo and Ronco, 2000, Rogiers et al., 2006).
El estudio de Ronco y colaboradores(Ronco et al., 2000) en un centro italiano de 425 pacientes
crticos con IRA demostr una reduccin de la mortalidad a corto plazo a 42% con
hemofiltracin a 35 y 45 ml/kg/h (basado en el peso documentado del paciente antes de la
enfermedad critica) equivalente a 2.5 3 l/h para un adulto de 70 kg, comparada con 59% en el
grupo de 25 ml/kg/h. El subgrupo de pacientes spticos tuvo aun menos mortalidad con la
dosis de 45 ml/kg/h en comparacin a 35 ml/kg/h. Tambin se vio una diferencia en el tiempo
de sobrevivencia a largo plazo en favor de los grupos con dosis ms alta de hemofiltracin.
Como resultado derivado se observ que en todos los grupos la mortalidad sola ser ms alta
en los pacientes en que se empez la hemofiltracin con un nivel de urea ms alto (promedio
de 20 mmol/l) que en los que se hemofiltraron ya con un promedio de urea de 16 mmol/l.
Estudios observacionales anteriores tambin vieron una mortalidad reducida con hemofiltracin
temprana(Gettings et al., 1999) (Reddy et al., 2002) y a dosis alta(Oudemans-van Stra et al.,
1999). Sin embargo, el mismo grupo holands que observ mortalidad menor que la
pronsticada (por APACHE II, SAPS II y la valoracin de severidad de IRA de Liao) con
hemofiltracin a alto volumen (promedio 4 l/h, 100 l/dia)(Oudemans-van Stra et al., 1999) no
pudo comprobar una reduccin de mortalidad con hemofiltracin temprana a alto volumen en
comparacin a temprana a bajo volumen y tarda a bajo volumen en un estudio prospectivo de
106 pacientes en dos centros(Bouman et al., 2002). Sin embargo, s se vieron tendencias de
menos duracin de ventilacin mecnica y menos das en UCI a favor de hemofiltracin
temprana a dosis alta. La mortalidad (28 das) en todos los grupos de este estudio fue baja (25
30%) comparado con 43% en todos los pacientes hemofiltrados en estos dos UCIs durante
el mismo tiempo. Esta diferencia podra indicar un efecto de estudio sobre la seleccin y el
tratamiento general de los pacientes en el estudio que tal vez dificult la deteccin de un efecto
especifico de la dosis y el comienzo de la hemofiltracin. Todos los pacientes en este estudio
relativamente pequeo estaban en fallo y apoyo cardiovascular y respiratorio avanzado, una
gran parte fueron cardioquirrgicos, factores que no se describen en detalle en el estudio de
Ronco y colaboradores. Tal vez para la mortalidad de estos pacientes especficos es ms
relevante el tratamiento en general que las particularidades de la hemofiltracin.
Un estudio unicentrico en 200 pacientes crticos demostr que la adicin de dilisis a un flujo
de 1 2 l/h (18 +/- 5ml/kg/h) a una dosis de hemofiltracin de 2 l/h (25 +/- 6 ml/kg/h) se asoci
con mayor sobrevivencia en 28 das (59 versus 39 %) y tres meses (59 versus 34%)(Saudan et
al., 2006).
Otro estudio unicentrico de 200 pacientes no pudo comprobar un beneficio en mortalidad en
UCI o a 30 das o recuperacin de la funcin renal con una dosis de hemodiafiltracin venovenosa continua con efluente combinado (ultrafiltrado y lquido de dilisis) de 35 ml/kg/h
(mortalidad 51%) versus 20 ml/kg/h (mortalidad 44%)(Tolwani et al., 2008)
Un estudio observacional multicentrico italiano reciente tampoco pudo confirmar un efecto
beneficioso de altas dosis de hemodiafiltracin o dilisis intermitente sobre la mortalidad en
UCI. Sin embargo, como en el estudio holands, se observ una duracin de ventilacin
mecnica y estancia en UCI mas corta en los sobrevivientes que recibieron dosis ms altas de
aporte renal(Vesconi et al., 2009)
La evidencia ms reciente y de mayor calidad sobre apoyo renal a dosis altas comparadas con
las convencionales proviene de dos estudios multicntricos de Estados Unidos y Australia y
Nueva Zelandia.

renal y otros fallos orgnicos. Este estudio ha sido analizado en profundo por un grupo
de expertos, que observaron algunos problemas metodolgicos significativos(Ronco et
al., 2008). El aclaramiento provisto por HD diaria (intensiva) en realidad era menos
que con hemofiltracin continua a dosis convencional de 20 ml/kg/h. Esto implica que la
diferencia entre los grupos convencionales e intensivos de apoyo renal en trminos de
dosis y aclaramiento fue menor, tal vez casi inexistente. Adems, el apoyo renal se
comenz relativamente tarde en una gran parte de los pacientes. Como ya mencionado
la recuperacin de la funcin renal en ambos grupos fue ms baja que en otros
estudios.
El estudio australiano RENAL (Randomized Evaluation of Normal vs Augmented
Level of Renal Replacement Therapy) randomiz 1508 pacientes a hemodiafiltracin
continua con un efluente combinado de ultrafiltrado y dializado de 25 ml/kg/h versus 40
ml/kg/h (basado en peso real cuando los pacientes se randomizaron). La duracin
promedia de hemodiafiltracin fue solo 6 das en ambos grupos. En 90 das
despus de inclusin al estudio la mortalidad fue igual (44.7%) en los dos grupos,
y el porcentaje de pacientes todava en apoyo renal era similar (6.8% con dosis alta,
4.4% con dosis baja). Se vio hipofosfatemia en un porcentaje mas alto de pacientes
con la dosis alta (65 vs 54%)(Bellomo et al., 2009).

esperar evidencia de un estudio de fase III de muchos mas pacientes y con tiempo de uso del
dispositivo de mas de 72 horas. Es posible que esta tecnologa ofrecer un pronstico mejor de
IRA severa en pacientes crticos en el futuro, ya que es una sustitucin ms integral de todas
las funciones del glomerulo y tbulo renal.
Resumen y conclusiones
La IRA sigue siendo frecuente y grave y conlleva una morbimortalidad elevada por su
asociacin con fallo multiorganico pero tambin atribuible a si mismo y a su tratamiento. Para
evitar esta morbimortalidad, la prevencin de IRA toma un lugar sumamente importante. La
estrategia mas importante para prevencin de IRA es la reanimacin temprana y
adecuada con fluidos, inotrpos y agentes vasooactivos junto con el tratamiento rpido
y eficaz de la enfermedad causante de IRA y fallo multiorgnico.
Existe evidencia, aunque de calidad no muy buena y en parte controvertida, que ciertas
sustancias pueden ayudar a prevenir IRA en ciertas situaciones:
Bicarbonato al 1.26%, N-Acetilcsteina y teofilina para prevenir nefropatia de contraste
Fenoldopam para prevenir IRA en cirugia cardiovascular y pacientes crticos
Peptido atrial natriuretico en dosis baja en cirugia cardiovascular
Recomendaciones y las evidencias existentes para prevencin de IRA se han sido resumidas
por un grupo de expertos de la Sociedad Europea de Cuidados Intensivos este ao (Joannidis
et al., 2010).
Una gran parte de los casos de IRA leve o moderado se resuelven con una estrategia
teraputica similar a la de prevencin, con alta calidad de reanimacin y cuidados intensivos,
sobre todo mantenimiento de gasto cardiaco y tensin arterial (si necesario con vasopresores
como norepinefrina o vasopresina), pero sin exceso de fluidos. El valor de aadir diurticos de
asa a dosis bajas sigue sin comprobacin de su efecto sobre la funcin renal y no parece influir
en la mortalidad. El uso de diurticos sigue siendo comn en la prctica clnica y puede facilitar
el manejo clnico de IRA y sobrecarga de lquidos. Sin embargo, un ensayo de diurticos a
dosis baja siempre debe seguir una reanimacin eficaz temprana y continu y nunca sustituirla.
Si el establecimiento de gasto cardiaco y presin arterial adecuada y posible adicin de
diurticos a dosis baja no lleva a una diuresis satisfactoria y el paro del aumento de potasio, in
hidrogeno, urea y creatinina muy pronto, hay que proveer apoyo renal extracorpreo eficaz
con el mtodo ms apropiado para la situacin actual del paciente y ms usado por el
personal del centro en que est. Si se usa hemodiafiltracin la dosis minima deberia ser
25 ml/kg/h de efluente, basado en el peso real del paciente al comienzo de
hemodiafiltracin. Esto se debe seguir acompaando con el tratamiento de la enfermedad
subyacente y el mantenimiento de gasto cardiaco, tensin arterial y otros aspectos importantes
de cuidados intensivos, como nutricin y sustitucin de electrolitos adecuados.
Un nuevo dispositivo de asistencia renal, que combina hemofiltracin convencional con un
cartucho con una membrana de clulas tubulares ha dado resultados prometedoras en un
primer ensayo multicentrico de 40 pacientes con IRA y fallo multiorganico y se debe estudiar en
un numero ms grande de pacientes.
