Abstracto
Esta revisin se refiere a los efectos del ejercicio fatigante muscular general
(que involucra a todo el cuerpo) y al ejercicio muscular de fatiga local (con
la participacin de un grupo muscular, en particular) en el control postural.
Cortos e intensivos ejercicios aumenta en general el balanceo postural
cuando el gasto de energa inducida supera el umbral de acumulacin de
lactato. Ejercicio exhaustivo local afecta el control postural cuando se
genera una prdida de fuerza al menos en un 25-30% de la contraccin
voluntaria mxima. Ejercicios generales y locales no intensivos tambin
pueden perturbar el control postural cuando se prolonga el ejercicio. Ambos
ejercicios generales y locales contribuyen a alterar la efectividad de los
estmulos sensoriales motores de input y de output del control postural.
Diferentes estrategias posturales compensatorios se activan para
contrarrestar o limitar la alteracin del control postural debido a la fatiga
muscular general y local. Se proponen modelos conceptuales para explicar
cmo la contribucin central privilegia cierta informacin
sensorial y
descuida y / o compensa otra informacin y mejora el output motor del
control postural mediante el desarrollo de estrategias motoras segn el
contexto de la fatiga muscular.

1. Introduccin
El control postural es una funcin compleja que implica mantener la
proyeccin vertical del centro de gravedad (COG) dentro de la base de
apoyo. Una actitud postural nunca se adquiere definitivamente. La primera
causa de esto se relaciona con los movimientos de lquidos y contracciones
musculares respiratorias y cardiacas. Estas funciones fisiolgicas inducen al
balanceo continuo del cuerpo (Conforto et al., 2001). La estabilidad postural
puede ser cuantificada mediante la medicin de los desplazamientos del
COG, es decir, los desplazamientos de todo el cuerpo y / o los
desplazamientos del centro de presin del pie (COP), es decir, la variacin
del momento alrededor del tobillo (Caron et al., 2000). El cuerpo
constantemente sufre deformaciones. Este fenmeno contribuye a modificar
su estado de reposo e impide mantener un estado balanceado. Segn
Bouisset y Duchne (1994), las perturbaciones resultantes de los
movimientos respiratorios son dbiles y, por s solo, no son responsable de
la oscilacin postural. Por un lado, el movimiento respiratorio es al menos en
parte contrarrestados por pequeos desplazamientos angulares de la parte
inferior del tronco y las extremidades inferiores (Hodges et al., 2002). Por

mayor perturbacin de la propiocepcin que las acciones concntricas
(Givoni et al., 2007). Este dao contribuye a la perturbacin del control

(Nardone et al., 1997). El runinng, que induce a fuertes contracciones
concntricas y excntricas con la marcha, probablemente genera mayor
dao para la propiocepcin (msculos, tendones y articulaciones) y los
receptores cutneos mecano plantares. Se puede suponer que la ejecucin
provoca mayores restricciones mecnicas para los distintos receptores
sensoriales implicados en el sistema postural al caminar.
3.3. Las estrategias compensatorias
3.3.1. Compensacin sensorial
El ejercicio muscular general afecta a la sensibilidad de los distintos
receptores sensoriales implicados en la regulacin postural. La perturbacin
de un canal sensorial puede ser compensada por la intervencin de otro
canal sensorial. Por ejemplo, la perturbacin a la informacin propioceptiva
se compensa por un aumento en la contribucin de la informacin visual. La
entrada visual compensa de manera eficiente para la disminucin de la
eficacia del output motor para controlar el equilibrio (Nardone et al., 1998).
Sin embargo, parece apropiado considerar si la perturbacin a un solo
receptor sensorial sera suficiente para degradar el control postural. Por
ejemplo, se podra considerar si la alteracin de solo la sensibilidad
vestibular sera suficiente para influir negativamente en el control postural
despus del
runinng independientemente de las perturbaciones
propioceptivas y visuales inducidas. Se ha indicado anteriormente que la
cantidad de informacin visual puede ser fundamental para compensar una
reduccin en la sensibilidad propioceptiva. Sin embargo, despus de un
ejercicio para el que hay una intensa necesidad de informacin visual, tales
como el runinng, el fenmeno de compensacin visual desaparece (Fig. 2).
Por lo tanto, se podra considerar si la alteracin de la informacin visual por
s sola podra deteriorar la eficiencia del sistema de regulacin postural. Es
difcil causar el deterioro de la sensibilidad vestibular o visual a travs de
solo ejercicio muscular general. Sin embargo, el deterioro de la informacin
propioceptiva sola despus de ejercicio muscular local (un grupo muscular)
se ha investigado y obtenido en muchos protocolos experimentales (vase
la Seccin 4). Por otra parte, las alteraciones de los diferentes estmulos
sensoriales pueden estar limitadas por un mayor aporte de recursos

3.3.2. Compensacin de los recursos cognitivos
Durante los ejercicios musculares submximos (baja intensidad), es posible
que la fatiga inducida se compense inicialmente por un aumento en el nivel
de vigilancia (Nardone et al., 1998). Segn estos autores, este aumento
mejora la eficacia de la unidad descendente requerida para activar las
hombres al realizar el mismo trabajo relativo como los hombres. Esta fuerza
inferior absoluta implica menos compresin mecnica de la vasculatura
local y, por lo tanto, el desequilibrio entre la oferta y la demanda de oxgeno
para el msculo en ejercicio es menos pronunciado (Martin y Rattey, 2007).
Las mujeres pueden tener una mayor capacidad de ejecucin del
metabolismo oxidativo, lo que reduce la dependencia de las vas glucolticas
(Russ et al., 2005). Las mujeres exhiben menos fatiga central durante las
contracciones musculares sostenidas que los hombres. La fatiga induce una
mayor reduccin de las tasas de disparo de la unidad motora o de
recrutamiento de unidades motoras en los hombres que en las mujeres.
Sin embargo, estas diferencias entre hombres y mujeres son vlidos slo
para el ejercicio de fatiga muscular local. En efecto, los hombres
experimentan un mayor incremento en la perturbacin en el plano sagital
despus de la fatiga muscular localizada (peso taln - elevacin) que la
fatiga de todo el cuerpo (ergmetro de remo), mientras que esta
perturbacin era mayor despus de la fatiga de todo el cuerpo de la fatiga
muscular localizada en mujeres (Springer y Pincivero, 2009). Tomando en
consideracin este estudio, apareceran diferencias de gnero que existen
segn el tipo de ejercicio fatigoso (general vs local) llevado a cabo. Por
tanto, sera interesante realizar estudios adicionales sobre la naturaleza de
la fatiga (central vs perifrica) con el fin de identificar con mayor precisin
las posibles diferencias entre hombres y mujeres en cuanto a los efectos de
la fatiga muscular en el control postural.
4.7. Influencia de soportes externos adicionales
En una condicin de no fatiga, ciertos dispositivos proporcionan apoyos
mecnicos adicionales y llevan la informacin sensorial adicional al sistema
postural (Noe et al., 2009). Despus de una tarea fatigosa, dispositivos
ortopdicos para el tobillo disminuyen la alteracin del control postural
(Ochsendorf et al., 2000). Estos autores sugieren que en una situacin de
motor y / o trastornos sensoriales, los dispositivos ortopdicos influyen en el
control postural en los planos mecnicos y sensoriales. A nivel mecnico un
dispositivo externo adicional en primer lugar apoya la estabilidad del tobillo.
Tambin mejora la alineacin de los diferentes huesos que constituyen la
articulacin del tobillo, que facilita la funcin de los receptores articulares. A
un nivel sensorial, un dispositivo externo adicional proporciona estmulos
tctiles a la base del pie.
Por otra parte, durante una tarea postural, el sistema nervioso central
integra la informacin sensorial que emana de un receptor sensorial lejos
de, o cerca de, el grupo muscular fatigado. De hecho, la informacin de la
presin plantar artificial entregado a travs de la estimulacin electro-tctil
de la lengua mejora el control postural y compensa la perturbacin postural
inducido por la fatiga de los msculos extensores del tronco (Vuillerme et
al., 2008). Aplicacin unilateral de la presin circunferencial utilizando un
esfigmomanmetro aneroide peditrico mitiga los efectos adversos de la
fatiga de los msculos del tobillo mediante la mejora de la agudeza del
