AFFIDAVIT
I, PO1 Thomas D. Co, Filipino, of legal age, and with residence at 2311
Leon Guinto St., Malate, Manila after having been duly sworn in accordance with
law, hereby depose and state that:
(a) I am the desk officer on duty On 2 nd of February 2016, at Pasay City
Police Community Precinct 2 located at Sen. Gil Puyat Avenue corner
Dominga St., Pasay City (PCP2-Pasay).
(b) On or about Ten (10) Oclock in the morning, a certain Mr. RUFINO
SAMUEL R. MANTOS, Filipino, of legal age, and with residence at No.
365 B. F. Ortigas St. Brgy. New Zaniga Mandaluyong City, personally
went to PCP2-Pasay to report an incident identifying himself as the
victim and one Mr. Willie R. Wong as the sole and primary suspect.
(c) I immediately took Mr. Mantos's statement in a police blotter as part of
our standard procedure and alerted PCP2-Pasay Patrol team to
coordinate with the Makati Police to make an inquiry of a spot report
related to Mr. Mantos's ordeal the night before.
(d) A copy of Mr. Mantos's statement is attached hereto in the form of
EXTRACT COPY OF POLICE BLOTTER marked as Annex A.
(e) As of this writing, copies of progress reports and a final report are
being procured from the Makati Police.
(f) I am executing this affidavit to profess the truth.

Narration of Facts:
(a)

On February 1, 2016, at around 5:00 in the evening, I am with my friend

KRISTINE LARA G. DEFENSOR, my high school classmate, at Vibers Bar
Pasay to drink and celebrate her return from the USA.

(b)

After we finished our first bucket of beer, a stranger approached us and

introduced himself as Willie R. WONG, he told us that he wanted to drink
with us. . Because of being a little drunk and for us not to be rude to a
stranger, we agreed to let him join us.

(c)

At around 11:00 in the evening, Lara, being drunk went outside the bar to
call his driver and at the same time I also went to the comfort room. Upon
my return to our table, Mr. Willie R. WONG told me that Lara Already left.

(d)

Thereafter, I told Mr. Willie R. WONG to finish our drink, he opened the
last two bottle of beer and I saw him put something in my beer, but I did
not manage to bother because I was already drunk that time.

(e)

While I was finishing the bottle of beer I felt very dizzy and blacked out.

(f)

On February 2, 2016 at around 6:00 in the morning I woke up both my

hands tied up with a telephone cable and my feet with the blanket. I also
heard Mr. Willie R. WONG taking to his Phone while leaving the room.
Then I realized that I was in a hotel room.

(g)

Thereafter, I felt body pains most especially on my lower body where my

butt is very swollen and I am sure that something was inserted in it and
found out that it was a TV remote control. I cannot move, I cannot sit, I
could not walk, I cant even shout because of body pain and whenever I
try to move the pain becomes more intolerable but after a few tries, I
managed to get up and dressed and went down to the reception and
asked some help.
