in achieving its goals for improving employee health care benefits by providing you with competent dental benefits at
a very affordable price for all your employees and their dependents.

OUR PROPOSAL
Pacific Concrete Products, Inc. has plenty of workforce that may need health benefits. However, most of these
workforce may find it difficult to avail of such comprehensive health benefits that they tend to brush these matters off.
Pacific Concrete Products, Inc. faces the possibility of employees not being able to report for work due to health
issues, especially dental related matters.
We have developed solutions to help you and your employees avoid such instances by offering your company with
different types of dental benefits that may cater to the needs of your employees. This solution may only be focused
on one aspect of the health needs of your workforce, but it is a good way to start. Most importantly, we will provide
the utmost care and service that ensures your employees will experience the quality dental care that most dental
offices may offer.

Execution Strategy
Just like with any other HMO, our execution strategy incorporates the usual dental benefits that are given to their
clients. The only difference with the usual HMO with the medical coverage will be the fact that we are only offering
dental benefits. Following is a description of our project methods, including how the project will be developed, a
proposed timeline of events, and reasons for why we suggest developing the project as described.

Technical/Project Approach
This dental benefit proposal requires a minimum of 50 clients. The clients are made to choose from three different
options of dental benefits. They are asked to fill out a general information form and will be issued an ID card with their
corresponding account number with our dental clinic. A guideline book shall also be given to the clients for further
instructions as to how they may claim their benefits. They are asked to call ahead of time to set an appointment prior
to claiming their benefits. Your company shall be notified monthly of the employees who have availed of treatments
on the said month. The benefits are renewable annually. Employees may also get dental benefits for their
dependents.

Payment Scheme
Payment in full is required to be given by Pacific Concrete Products, Inc. prior to the start of the contract. The
company shall be solely responsible for acquiring payments/premiums from their employees for these benefits. Go
Smile Dental Clinic shall transact only with Pacific Concrete Products, Inc. for the dental benefits. Shall the employee
wish to avail of the discounted rates offered with the dental benefits, payments for those procedures shall be directly
given to Go Smile Dental Clinic.

Timeline for Execution


As soon as the Memorandum of Agreement is signed, the benefits may be availed 30days from the time the benefit
was availed and may be renewed a year after that. Benefits must be availed within the year.

GO SMILE DENTAL CLINIC is a dental office that caters to all the needs of the patients. We offer quality care to all
our patients and we make sure that the patient is comfortable. We comply with DOH for the radiation safety and the
Infection Control Protocol of the City Government.

Easily accessible and readily available. We are open daily from 10am to 6pm and we are usually open during
holidays.
Strictly complies with the safety standards for patient safety
