RESUMEN
El espectro clnico d el hipotiroid ismo es va ria ble. A d iferencia d e los pa cientes con
hipotiroidismo ma nifiesto, con presencia de signos y sntoma s de la enfermeda d, los que
pa d ecen d e hipotiroid ismo subclnico tienen niveles norma les sricos d e tiroxina y
triyod otironina , presenta nd o nica mente una leve eleva cin d e los niveles sricos d e
tirotropina . Ta les pa cientes pueden ser identifica dos por el gineco-obstetra media nte ex menes
de rutina , ta nto en el emba ra zo como en el clima terio, o bien, en el curso de una eva lua cin
de sntoma s no especficos ta les como a ltera ciones menstrua les.
Los riesgos y beneficios potencia les de la tera pia con tiroxina pa ra el hipotiroidismo subclnico
son discutibles. La s venta ja s posibles esta ra n da da s por la prevencin del hipotiroidismo
clnico, mejora de la dislipidemia , prevencin ca rdiova scula r, a livio de sntoma s clnicos y
reduccin de riesgos perina ta les. Los riesgos a socia dos a l tra ta miento son la a pa ricin de
osteoporosis y a rritmia s.
El uso de tiroxina en el ma nejo del hipotiroidismo subclnico sera beneficioso en a quella s
pa cientes que rena n a l menos una de la s siguientes condiciones: a nticuerpos a ntiperoxida sa
positivos, niveles de TSH srica ma yor de 10 mU/ L, emba ra zo, disfuncin ovula toria e
infertilida d. En pa cientes cuyo beneficio sea la reduccin de los sntoma s o reduccin de los
niveles de colesterol srico debiera ha cerse un ensa yo tera putico pa ra va lora r los beneficios en
ca da ca so en pa rticula r.

SUMMARY
There is broa d clinica l spectrum hypothyroidism is va ria ble. Unlike the pa tients with ma nifest
hypothyroidism, with presence of signs a nd symptoms of the disea se, those tha t suffer of
subclinica l hypothyroidism ha ve norma l serum levels of thyroxine a nd triiodothyronin,
presenting solely one slight eleva tion of the serum levels of tirotropina 1 . Such pa tients ca n be

identified by obstetrics a nd gynecologist by mea ns of routine exa mina tions, a s much in the
pregna ncy a s in the clima cteric, or, in the course of a n eva lua tion of nonspecific symptoms
such a s a ltera tion of the fema le genita l blood flow.
The risks a nd potentia l benefits of the thera py with thyroxine for the subclinica l hypothyroidism
a re deba ta ble. The possible a dva nta ges would be given by the prevention of the clinica l
hypothyroidism, improvement of dislipidemia , ca rdiova scula r prevention, lightening of clinica l
symptoms a nd reduction of perina ta l risks. The risks a ssocia ted to the trea tment a re the
a ppea ra nce of osteoporosis a nd a rrhythmia s.
The use of thyroxine in the ha ndling of the subclinica l hypothyroidism would be beneficia l in
those pa tients who reunite a t lea st one of the following conditions: positive a ntiperoxida se
a ntibodies, level of serum TSH grea ter tha n 10 mU/L, pregna ncy, a novula tion a nd infertility.
In pa tients whose benefit is the reduction of the symptoms or reduction of the serum cholesterol
level ha d to be ma de a test thera peutic in order to va lue the benefits in ea ch ca se in pa rticula r.

Hipercolesterolemia.

miento12, mientras que otros tres grupos8,14,43 sugieren generalmente iniciar terapia en pacientes con
hipotiroidismo primario subclnico, especialmente si
hay anticuerpos antiperoxidasa positivos. Las excepciones potenciales incluyen a ancianos8, pacientes
con enfermedad cardiaca que tienen leves elevaciones
de TSH6, pacientes con niveles de TSH srica <10 mU/L,
y por ltimo, aquellos en que los anticuerpos antiperoxidasa son negativos14. Estos pacientes que no
recibirn terapia debieran ser controlados en forma
rutinaria al menos una vez al ao.
Considerando las evidencias ya sealadas y en
relacin a la alta frecuencia de conversin de HS a
hipotiroidismo manifiesto cuando existen anticuerpos
antiperoxidasa (antimicrosomales) presentes, sera
aconsejable tratar pacientes asintomticos con hipotiroidismo subclnico aun con niveles normales de
colesterol srico. Por otra parte independientemente
de la presencia de anticuerpos antiperoxidasa, en
aquellos pacientes con elevado nivel srico de TSH
(>10 mU/L) la terapia con tiroxina estara indicada por
el alto riesgo de progresin a hipotiroidismo manifiesto. Es tambin razonable tratar hipotiroidismo subclnico en mujeres embarazadas y en mujeres que tienen
anovulacin e infertilidad (Figura 1).

Un ensayo teraputico para hipotiroidismo subclnico se debe realizar si los pacientes tienen sntomas
uniformes con la presencia de hipotiroidismo leve,
hipercolesterolemia, o un bocio. Aunque la superposicin de sntomas entre pacientes con hipotiroidismo
subclnico y pacientes eutiroideos hace difcil predecir
quien tendr una respuesta al tratamiento, algunos
pacientes tienen un mejoramiento notable en sus
sntomas con la terapia con tiroxina. Los hallazgos
positivos en algunos pequeos estudios clnicos29,30,33
tambin apoyan el uso de terapia en pacientes
sintomticos, y el reemplazo con tiroxina siempre
puede interrumpirse si no hay beneficio evidente.
Una dosis oral inicial de tiroxina de 50 g/da a 75
g/da es suficiente habitualmente para normalizar
los niveles sricos de TSH29-31. Pacientes con enfermedad coronaria debieran recibir menores dosis
iniciales (por ejemplo 12,5 a 25 g/da). Los niveles
