To whom it may concern,

I was asked to write and explanation letter regarding to the Medication Error that
happened to patient Manuel, Rosalina. It was April 27, 2016 late night. When Dr. Sherwin
Guevarra, rounds the patient. And so the orders have been made. I was carried out the order and
transcribed it on the medication sheet. The other medications transcribed correctly but the one
medication that I transcribed, which is the ceftriaxone (VEXON) q12, in the order but I
transcribed it in q8. So I time it in (8pm-4am-12pm). To be honest, I was not in my mood to
work on that night. I have lots of problem popping in my mind. And I know when in work you
have to focus on the patient and set aside the problem. Im so sorry for the things that Ive done.
I will take the full responsibility for the damaged that Ive done. Thank you and Good day!
