I,Hemanta Kumar Sahoo , aged about 52 years,S/o Late Dinabandhu Sahoo permanent
residence of
Vill-Similipada,PO/PS/Dist-Angul in the
state of odisha do hereby solemnly affirm and state as
follows:1. That,I am the permanent of the above noted address.
2. That, Jogendara Biswal, aged about 32 yrs, S/o-Madan Biswal of the above noted address
i.e.Vill- Soubhagyanagar ,PO/PS/Dist-Angul, Odisha is going to take a new Electric
Connection to the Rented Shop
Room for Commercial Purpose.
3. That, Jogendara Biswal applied for a new electric connection for his Rented Shop Room for
Commercial Purpose.
4. That, Khata No.386 of Mouza-Angul Town, Similipada stands recorded Grand Mother Name
is Swarnalata Sahu,
W/o Late -Dinabandhu Sahu.
5.That,I have no objection against him if a New electric line will be connected in his favour,
to his Rented Shop Room for Commercial Use vide Khata No-386 Plot No-779 Area-AC-22
dec of Vill-Similipada,Po/PS/Dist-Angul.
6. That, I Swear this affidavit in order to produce before the CESU/ENZEN Authority,Angul
Division for new Electrification for Rented Shop Room Commercial Purpose on behalf of
Jogendra Biswal.
That, the contents of this affidavit are true and correct to the best of my knowledge
and belief.

2. That, Khata No-386 Plot No-779 Mouza-Angul Town,Similipada Area :-Room Rented Owner
name Hemant Kumar Sahoo ,S/O Late Dinabandhu Sahoo and the landed Property owner
Swarnalata Sahu, W/o Late -Dinabandhu Sahu.
3. That, now I am intending to avail an electric connection to the above said plot and there is
no objection from any corner & no electricity has been supplied to the aforesaid area, if the
CESU Authority,ANGUL DIVISION ,Angul provided the same to me.
4. That, if there will be pending any arrear on the same plot then the rest amount will be
added on the consumer account.
5. That, I do here by declare that I will pay the all dues for COMMERCIAL USE IN MY SHOP
ROOM.
6. That, I Swear this affidavit in order to produce before the CONCERNED Authority of CESU,
ANGUL DIVISION, for Electrification to my new Rented Shop Room.
That, the contents of this affidavit are true and correct to the best of my knowledge
