B. Past health History
She stated that she had not experienced any disease when she was
a child. Her immunizations were completed during her childhood.
She has no allergies in food or drugs. She was first diagnosing of
cancer this year and she had two confinements in November
because she experienced abdominal pain and her stool is black. She
went from Antique for three months.
C. Present Health History
Patient is diagnosed of Gastric Ca, confirmed by endoscopy and
biopsy last February 2012. She was admitted last November 29 due
to pallor and anemia (Hgb 5mg/dl) and was transfused with five
units of Pack RBCs. She stayed for seven days and was discharged.
One-day prior to admission, patient sought consult from her
attending physician and a CBC was requested as part of follow up
check up. Hemoglobin was noted to be decreased at 8 mg/dl and
was advised admission however the patient decided to go home.
D. Family Health History
Her parents died because of ageing. Her siblings have no hereditary
diseases. Her husband died at 42 due to electric accident. And she
has three children; all of them have their own families.

Functional Health Pattern


A. Health Perception and Health Management
Patient takes a bath twice a week stated by her daughter because
she feels lazy to take a bath. She is usually in her store. She just has
her sponge bath when told by her daughter to do. Patient sees
health care provider when she has a feeling of undesired changes or
if she feels pain in her body. She has no safety or traditional
practices being followed. She feels healthy by eating right kind of
foods. Patient started to smoke when she was 17. She smoked two
packs of cigarettes in a day and when she was 73, she smoked five
sticks a day. She occasionally drinks alcohol beverages or when she
feels difficulty in sleeping.

B. Nutritional and Metabolic Pattern
She usually eats three times a day. She likes to drink coke than
water. She only drinks water two to three glasses a day. She likes
pork meat especially fried. She also likes fatty foods. She usually
likes to eat in front of television. She lost her weight during her
confinement because of decreased appetite and she wanted to go
home. Her skin is dry and she has dentures.

C. Activity and Exercise Pattern
She considers walk as her exercise during morning at least twice a
week. She likes to watch tv or if she has nothing to do she sleeps.

D. Elimination Pattern
She defecates twice a week. After she was in Antique, she felt pain
on her abdomen and she had her black stools and had her check
ups. After five days, the stools were color light brown. She urinates

three times a day, yellow in color. Her daughter said that her mother
likes carrot juice when she has difficulty in defecating.

E. Sleep-Rest Pattern
She usually sleeps at 9pm and wakes up at 6am. When she is at
home her sleep is continued but when she is in the hospital, she has
difficulty in sleeping. If she has difficulty in sleeping she just lie
down at bed. Sometimes she takes her nap in the afternoon.
Watching tv is the relaxation for her.

She has difficulty in her vision and she wears eyeglasses. she has
hearing aid to hear properly. Due to her age, she has memory lost
sometimes she forgets where she put her things.

She thinks that there is someone who does witchcraft on her that is
why she is sick. Whenever she has problem in the family, she tells
them right away so they solve or talk it all about. She likes to talk to
people if she has time because she is usually in their house or in
store. She thinks that she is a calm person. She does not mind
anyone.

She lives with her youngest daughter with three children. She is the
grandparent there and she helps them by having her sari sari store.
If they have problem they settle it and talk things. They are usually

in the house. Whenever her daughter has free time they go out of
town.

pain in her abdomen is the change for her. Her daughter did not say
to her that she has cancer.

concerning her health. She just seek health provider if she feels sick.
