APPOINTMENT CONTRACT

The HERBFFREY GENERAL MERCHANDISE hereby appoints Ms. EVA RICA

MANALO, as a contractual employee with all the duties, rights, and privileges appertaining
thereto in accordance with the rules and regulations of the establishment.
For year 2016 2017, Ms. EVA RICA MANALO will be a naturaphatic practitioner of
Las Pias Outlet with the renumeration of:
A. COMPENSATION
Daily wage of four hundred and twenty pesos (P420.00) under no work, no pay
policy;
13th month pay based on the date of employment;
Incentive based on the total sale of the month and;
Birthday bonus worth one thousand pesos (P1000.00).
B. BENEFITS

PhilHealth -P100.00
Pag-ibig -P50.00
C. OTHER PRIVILEGES
5% discount for all Healing Galing products which should be paid at the end of the
month worth P1,000.00 only.
Salary loan of P2,000.00 P3,000.00 payable in 2 months without interest.
Double pay during regular holidays.
Additional 30% of daily wage during special holiday.
This contract takes effect on January 1, 2016 and terminates at the end of the year or in
the discretion of the establishment.
