AFFIDAVIT OF SERVICE
I, ARNOLD C. BUCOY, of legal age, Filipino, single and a resident of Sta.
Catalina, Zamboanga City, after having been duly sworn to in accordance with
law, do hereby depose and state:
1. That I am employed as a Paralegal in Sana Law Office, with office address
at Room 206, EJS Building, Veterans Ave. Ext., Zone IV, Zamboanga City;
2. That on 24 September 2015, I personally served a copy of the Petition in
Virginia Bazan v. Gregorio Bazan for Legal Separation with Prayer of
Issuance of Temporary Restraining Order, docketed as civil case no.
6748, to the Office of the City Prosecutor. A copy of the Petition was duly
received evidenced by the signature and printed date appearing on the
last page of the filed Petition.
3. I also served a copy of the same Petition to the Office of the Solicitor
General by registered mail with explanation in accordance with Section
10 of Rule 13 of the Rules of Court. Service made is evidenced by
Registry Receipt No. RD 040 017 619 ZZ;
4. That I am executing this affidavit to attest to the veracity of the foregoing
and for other legal purposes that this affidavit may serve.
