AFFIDAVIT OF LOSS
I, FELISA A. ENRIQUEZ, of legal age and a resident of Pengue
Ruyu Tuguegarao City, after having been duly sworn to in accordance
with law, do hereby deposes and says:
1. That I am the owner of a Cloud Phone cellphone with unit
number 0917-7032637;
2. That on August 3,2015, while I was at Brickstone Mall
Tuguegarao City, I was still able to use my cellphone in calling
my son to ask him if there is still available rice for our
consumption at home;
3. There being no more supply of rice at home, I proceeded to
Don Domingo to buy one sack of rice;
4. When I returned home, I noticed that my cellphone was not in
my bag anymore so I started looking for it;
5. That despite diligent search and efforts to locate the said
cellphone, I could not find the same such that I now believe
that said cellphone is lost beyond recovery;
6. That I am executing this affidavit to attest the truth of the
foregoing allegations and for whatever legal purpose this may
serve.
