Reading Record Sheet Level Two

(for readers of chapter books)
Please use this list to record the titles of the books that you have read or listened to between April
1st of one year and March 31st of the following year. Please have a parent/guardian or teacher sign
the bottom of the list. Return this list to your media specialist or Reading Across Broward contact
person immediately after April 1st.
There are four different award levels, depending on the number of books that you have read or
listened to.
Participation Award (10-24 books including 5 award books)
Silver Award (25-34 books including 5 award books)
Gold Award (35-49 books including 5 award books)
Superintendents Honor Roll Award (50 or more books including 5 award books)
You may read any books of your choice. Do you want to know which books are good? Ask your
media specialist, public librarian, parents, and friends. To make reading more fun, we suggest that
you read different kinds of books (genres). Some examples of different genres are:

Fiction
Adventure
Fantasy
Historical fiction
Humorous fiction
Mystery
Realistic fiction
Science fiction
and others

Nonfiction
Fairy tales
Biography
Folk tales
Geography
Legends
History
Myths
Humor
Tall tales
Poetry
Science
Sports
and others

It is important to read books that have been recommended for awards or have won awards,
so please be sure to read at least five (5) award books. Award books include Sunshine
State, Newbery, Coretta Scott King, and others. Please list the award books that you have
read on the first five (5) lines of this list.
