PETITION
PETITIONERS, by the undersigned counsel, respectfully state that:
1.

Petitioners are husband and wife, Filipinos, both of legal age with residence and

postal address at _____________;

2.

They have no legitimate children/descendants and hereby desires to jointly adopt

a minor, named _____, five (5) years old, legitimate child of __________ and
__________. Attached herewith is a machine copy of the birth certificate of
__________ marked as Annex A;

3.

The biological parents of the minor, _____ are jobless and they can no longer

sustain the needs of their five children, thus they decided to have their child,___ be
adopted by petitioners in order for her to have a better life and future. Attached
herewith is a machine copy of the affidavit of consent to adoption executed by Spouses
_________ marked as Annex B;
4.

The father of the said minor is a brother of petitioner, _______;

5.

The Department for Social Welfare and Development has issued a certificate of

attendance certifying that petitioners attended a pre-adoption seminar on August __,

2015 at Traders Hotel, Pasay City. Attached herewith is a machine copy of the certificate
marked as Annex C;

The petitioners are qualified to adopt the said minor and have complied with the

requirements under the Domestic Adoption Act of 1998 (R.A. 8552), and will be able to
financially and morally bring up and educate such minor properly and adequately;

PRAYER
WHEREFORE, it is respectfully prayed that after due notice and hearing,
judgment be entered adjudging that the minor child_____ be declared for all legal
intents and purposes, the child of the herein petitioners______ and ___________,
and that her name be changed to ________.
Other relief and remedies are likewise prayed for.
