SRP Student Profile: Guidelines for Students

Please complete the SRP Student Profile by hand, in black or blue ink. Each applicant
must complete the form in their own handwriting, and in their own words. Please give
as much information as possible for each of the questions. Do not type the form itself;
however, if you want, you can type Part II: Personal/Autobiographical Statement.
Remember, there are no right answers. Please be as honest as possible.
CONFIDENTIALITY:
WUSC endeavors to respect the privacy of all applicants to the Student Refugee
Program. Some of the information provided on this application will be viewed by
WUSC Ottawa staff members and Local Committee members in Canada. Information
from Part One: Personal Data and Part Two: Personal/Autobiographical
Statement will be circulated to Local Committee members when we are trying to place
you at a post-secondary institution. If you would like to add information of a more
confidential nature to your application, include it in Part Three: Confidential. WUSC
Ottawa will try to limit the number of people involved in the sponsorship process who
have access to this part of the form.

PART ONE: PERSONAL DATA

QUESTION #1: Please give your complete name. In the Canadian system, most
people have at least two names, a given name and a family name. The family name is
the last name. Therefore, if your name is Mary Jane Stewart, you should write:
Family Name: Stewart

Given Name(s): Mary Jane

If possible, you should provide a minimum of two names. If your name cannot be broken
down to reflect this standard, put the name your friends call you as your given name. For
your family name, put your main, family name (sometimes this is your fathers name).
Alternatively, please give us your name as it appears on official documents. Please note
that you must be consistent and use the same name on all official documents
during the sponsorship and immigration process.

You need to fill out all the information on the WUSC SRP Student Profile. This guideline is to
help you with some of the questions that may require additional explanation.

QUESTION #4: Your country of Citizenship can be different from your country of
birth for instance, if you hold a passport from another country other than your
country of asylum or your country of origin.

QUESTION #7: Please mention if you have ever been married or if you are married
now.

QUESTION #11: Please provide information on your formal education (secondary


school and any University or college courses you have taken or diplomas/certificates
you have received). Provide the details starting with your secondary school.

Question #25 Please explain the source of funds used to pay for your food and
housing, as well as any other support you might receive. You can also explain what
your expenditures are.

QUESTION #26: Please describe where you live, and how you spend your time. This
will assist Local Committees to learn more what it is like to be a refugee and why you
need assistance.

QUESTION #29 and 30: If you have applied for resettlement to another country or if
you have already submitted an application to Canada, you should let WUSC know.

QUESTION # 32: Please tell us if you have family members living in Canada. This
information does not affect your chances of acceptance. If you are accepted, WUSC
may use this information to try and resettle you near your family members if
possible.

If your family has applied for resettlement elsewhere and has included you on their
application, you may wish to withdraw from the SRP program. WUSC cannot resettle
any of your family members. Your application to WUSC is only for yourself.
PART TWO: PERSONAL / AUTOBIOGRAPHICAL STATEMENT
Please print clearly, on one side of each page only. Include a description of what has
happened to you personally in your country of origin and what you have done since you
came to your country of asylum. You can provide additional information about yourself,
your family, or your friends. You can also explain your goals in life, what you do with
your time presently and what kind of activities or hobbies you have.
PART THREE: CONFIDENTIAL
If you would like to add anything to your application form but do not want the
information to be circulated to all of the members of the Local Committee, you can
include this information in this section. Where possible, WUSC will try to limit the
number of people who will read this part of the application form.
Please fill in all the questions on this form. Remember to include your
photograph.
